After the performance Justin Michael Terry gave as The Skull King in 'Anthrax: Blood Eagle Wings', where he was entirely hidden behind a mask and elaborate costume, director Jack Bennett asked him to play the lead role in "Monster at the End" as a thank you, thinking it would be a good opportunity for Terry to show his face and get some recognition. When the part was offered to him, the first thing Terry asked was if he could play the Monster instead.
