Knowing the child's destiny, the doctor replaces his missing arms with sword arms, attaches a repeating rifle to one arm, and a cannon to one leg. The game begins with the now-adult Hyakkimaru beginning his quest to reclaim his body parts from the Fiends. Soon joined by the child-thief Dororo, the pair embark on a quest against ancient Japanese demons and the monsters they create.

- The game takes place on a "steampunk" type of world. The Dark Acolytes, a mysterious organization of cyborgs and robots, is trying to find the five legendary Sacred Flames, and bring chaos to the World in the process. Meis, a womanizing "Spirit Blacksmith" with the ability to make magical weapons, finds himself chosen to find the flames before the Dark Acolytes do. However, to increase the power of his weapons, Meis must have the help of a woman, and must increase his 'intimacy level' with her by going out on dates. Along his journey, he meets a colorful cast of characters, including girls to date and allies to join his party.
- It has been 300 years since humanity collapsed in the wake of a great war. Nearly all of civilization destroyed, only small groups continue to live on in the wasteland. However, in the wasteland, there are strange towers that have appeared. Seemingly sentient, nobody knows who built them or why, only that anyone who ventures near will know only death. From these towers comes a mysterious substance called diablosis, which corrupts those who touch it, turning them into murderous killing machines.
