Serum homocysteine levels show interindividual variation and are determined by nutritional factor... more Serum homocysteine levels show interindividual variation and are determined by nutritional factors, such as B-vitamin intake, and by age and genetic influences, such as the genotype of the methylenetetrahydrofolate reductase (MTHFR) gene. Recently, the relation between one-carbon and lipid metabolism has been shown. Therefore, we hypothesized that folic acid supplementation would not only decrease homocysteine concentrations but also affect lipid metabolism. The aim of the present study was to evaluate the impact of short-term folic acid supplementation on homocysteine and lipid metabolism in Polish women older than 60 y with different C677T MTHFR genotypes. One hundred twenty-two volunteers were supplemented with folic acid 400 μg/d for 8 wk. Folate intake was assessed using a food-frequency questionnaire. The serum homocysteine level was analyzed using high-performance liquid chromatography. Serum biomarkers were measured with a Vitalab Flexor biochemical analyzer. MTHFR genotyping was performed using the polymerase chain reaction restriction fragment length polymorphism method. In the studied group, the MTHFR genotype frequencies were 0.14 for TT, 0.43 for CT, 0.43 for CC carriers. At baseline, the average folic acid and homocysteine concentrations were 12.16 ± 0.23 ng/mL and 7.94 ± 0.3 μmol/L, respectively. Folic acid supplementation lowered the serum homocysteine concentration. However, the dietary intervention also led to an increase in glucose concentrations (P < 0.01). The T-allele carriers had a larger waist circumference (P < 0.05) and a higher waist-to-hip ratio (P < 0.01). In elderly women, a short-term, low-dose folic acid supplementation lowered the serum homocysteine level but also increased glucose concentrations. The C677T MTHFR polymorphism affects the waist-to-hip ratio and lipid metabolism.

The reactions of the methionine/homocysteine pathway are mediated by several enzymes, including p... more The reactions of the methionine/homocysteine pathway are mediated by several enzymes, including phosphatidylethanolamine N-methyltransferase, cystathionine β-synthase, and betaine-homocysteine methyltransferase. Homocysteine homeostasis is regulated by these enzymes. We hypothesized here that the protein and folic acid content in the maternal diet affects methionine/homocysteine metabolism in the progeny. To test this hypothesis, pregnant rats were fed a diet with normal protein and normal folic acid levels (a modified casein-based AIN-93G diet), a protein-restricted and normal folic acid diet, a protein-restricted and folic acid-supplemented diet, or a normal protein and folic acid-supplemented diet. The progeny were fed either the modified AIN-93G diet or a high-fat lard-based diet. Progeny were analyzed for expression of the phosphatidylethanolamine N-methyltransferase, cystathionine β-synthase, and betaine-homocysteine methyltransferase genes in the liver and for serum homocysteine concentration. Interactions between prenatal and postnatal nutrition were also determined. The progeny of the dams fed the diets supplemented with folic acid showed decreased expression of all 3 genes (P < .001). An interaction effect between the protein and folic acid content in the maternal diet contributed to this down-regulation (P < .001), and the postweaning diet modified these effects. Serum homocysteine concentrations were approximately 15% higher in the male rats (P < .01), but neither prenatal nutrition nor the postweaning diet affected it significantly. We conclude that maternal diet during gestation has an important effect on the transcription level of these 3 genes, but changes in gene expression were not associated with significant changes in progeny homocysteine concentrations.

A 45-year-old male, with symptoms of many years standing of gastro-oesophageal reflux disease, wa... more A 45-year-old male, with symptoms of many years standing of gastro-oesophageal reflux disease, was subjected, under general anaesthesia, to laparoscopic fundoplication. Tracheal intubation yielded no problems but great difficulties were encountered during tube insertion into the oesophagus. After surgery, aphonia developed. Laryngological examination demonstrated paralysis of the left vocal cord. Voice strength returned to the pre-operative status after 3 months, and laryngological examination confirmed normal mobility of both cords. The possible cause of the complication was damage to the left recurrent laryngeal nerve which occurred during insertion of the tube into the oesophagus. Gastro-oesophageal reflux disease causing 'acid laryngitis' can create conditions favouring this type of complication.

Tomato pomace (TP), obtained as a residue of tomato processing, was used to enrich rye bread (RB)... more Tomato pomace (TP), obtained as a residue of tomato processing, was used to enrich rye bread (RB). The sensory profile of this functional bread (RB+TP) was characterised, and its fat absorption and lipid metabolism properties in high-fat-fed rats were studied. Intake of the HF diet containing RB, RB+TP, or TP alone increased faecal energy and fat excretion, but did not affect animal growth or visceral fat weight. Both RB and RB+TP diminished the negative impact of the HF diet, lowering the atherogenic index of plasma (AIP) and the total liver lipid contents by 31.6% and 24%, respectively. The experimental diets had no effect on liver S-adenosylhomocysteine (SAH) concentrations or on the S-adenosylmethionine (SAM) to SAH ratio, though the lowest SAM levels were observed in the HF+TP group. No significant differences were detected in blood homocysteine, triglycerides, glucose or insulin levels. Although RB+TP incorporated into a HF diet may lead to a decrease in AIP and total liver lipid content, this effect does not depend on the components of TP, but rather on the RB ingredients. However, pure TP, in the doses used in this study, may potentially play a role in the energy balance via faecal loss of lipids. © 2014 Society of Chemical Industry.
