This is achieved in that the claimed cutting catheter comprising a guidewire and a cutting instrument is provided with two guide members connected at one end to a cylindrical connector and at the other end to an annular device, wherein the outer part of said annular device is a bag made of hydrophilically coated nylon, and the inner part is an annular microsurgical blade, said annular microsurgical blade being provided with at least three protective inserts made of atraumatic nylon, and said hydrophilically coated nylon bag having a plurality of holes.

Description

Cutting catheter
The invention relates to medical equipment, namely cutting catheters, and can be used for thromboextraction and expansion of passages in vessels (artery, vein), destroying the elastic and fibrous part of the plaques for further stenting of the vessel.
One of the most common heart diseases is atherosclerotic cardiovascular disease, caused by the formation of lesions or plaques on the inner walls of blood vessels. For example, various lesions may be associated with atherosclerosis, including lesions of the aortic ostium. Lesions of the aortic orifice lead to lesions of the orifices of the great vessels branching from the aorta. This can lead to partial or even complete blockage of the artery.
Thrombosis is a dangerous condition in which a blood clot forms in a vessel. Such a blood clot can grow and clog a vessel (artery, vein) until it completely blocks. Under certain conditions it can cause it to “break off”, becoming a thromboembolus. Due to the dangers associated with such blockages, various methods and procedures have been developed to treat vascular blockages.
01.08.2018 г./. The closest analogue in technical essence to the claimed invention is a radiopaque balloon catheter with a cutting tool, which contains a catheter and a guide for delivering the balloon to the treated area. The catheter has a shaft containing a balloon with a pair of first radiopaque markers located at a distance from each other in the longitudinal direction to indicate the ends of the cylindrical portion of the balloon. The cylindrical section is located between the conical sections of the cylinder and provides a working surface. The conductor for delivering the balloon to the treatment area includes at least one pair of second radiopaque markers, and at least one pair of second radiopaque markers corresponds in location to a pair of first radiopaque markers on the balloon, so that one of the first radiopaque markers on the balloon is aligned with one of the second radiopaque markers on the conductor and another of the first radiopaque markers on the balloon is combined with another of the second radiopaque markers on the conductor /RU 2663072 C2, publ. 08/01/2018/.
The disadvantages of this analogue include the rigidity of the cutting cylinders. This limits the flexibility and ease of delivery of the balloon as it moves through the tortuous paths of a vessel or other body lumen. In addition, cylinders equipped with cutting blades and edges may be more difficult to deflate and fold. This may make it more difficult to remove balloons from the vascular system. The objective of the invention is to create a new, improved design of a cutting catheter with improved technical characteristics.
The technical result is to improve flexibility, increase safety and ensure effective removal and treatment of fibrotic lesions.
The ring-shaped microsurgical blade is made by combining two cutting tools in the shape of a semicircle, and the conductors are made with the ability to set the ring-shaped microsurgical blade in motion.

The invention works as follows.
Example.
A cutting catheter is used before stenting the blocked artery. The catheter is advanced using fluoroscopy along the guide wire so that the microsurgical blade is distal to the stenosis. Next, tightening the catheter, atheromatous plaques are removed (excised) with a microsurgical blade, and the removed plaques are collected in the bag. Accordingly, the claimed solutions use a bag with a microsurgical blade, which is designed to excise the stenosis during the procedure.
