Abstract
Description
The invention relates to medicine, and is intended to extract stones from the ureter.
There are known extractors for extracting stones from the ureter, which are a flexible tube to the end of which a nylon thread is attached, drawn through a side hole into the tube and removed from it through the lower end (I.F. Novikov “Ureter stones” (non-surgical treatment methods) p. 38 , fig. 3). The extraction of stones is as follows. The extractor is held above the stone and the thread is tightened, forming a loop at the end of the extractor. When the extractor is removed, the stone, falling into the loop, is gradually reduced to the bladder. The main disadvantages of extractors of this type are the great complexity and duration of the process of extracting stones due to the difficulty of capturing their loops and due to slipping of the loop.
Also known is a metal extractor, taken as a prototype and containing a supporting cable with a working part in the form of a basket of steel wires, a conical conductor with olive at the end, a flexible tubular body, a clip with a collet control handle (AS USSR No. 227513 M. cl 4 A61M 25/00). In the folded state, when the working part is inside the tubular body, the extractor is inserted into the ureter to the stone. Then the collet of the collet handle moves into the housing and pushes the working part out of it. Under the action of elastic forces, the wires diverge, forming a basket. When the extractor is removed, the stone enters the basket and is firmly fixed in it.
The main disadvantage of the prototype is that when removing large stones (close in size to the lumen of the ureter), there is a high probability of injury, for example, a rupture of the ureter.
The aim of the invention is to reduce injuries by providing the ability to extract large stones in size close to the lumen of the ureter.
This goal is achieved by the fact that in the extractor for extracting stones from the ureter containing a supporting cable with a working part, a tubular flexible body, a control handle, the working part is made of material with a shape memory having an initial martensitic transformation temperature from 37 ° to 42 ° C and when heated above the temperature of the onset of martensitic transformation, the shape of a conical spiral.
Of the distinguishing features in the known solutions, the use of shape memory elements in catheters was found (EPO 0199870 M. cl. 4 A61M 25/00). However, in the known solution, the elements with shape memory have a different purpose than in the inventive extractor. So according to patent EPV 0199870 thermoactive elements with shape memory are located in the distal end of the tubular body and, changing their shape, they deflect the distal end of the catheter in the right direction, that is, are designed to control the catheter. In the claimed extractor, the implementation of the working part of the carrier cable from a material with shape memory provides the ability to capture stones and their large size, their destruction. Giving the working part of the bearing cable the shape of a conical spiral in the found analogues was not found. Therefore, the inventive extractor meets the criterion of "significant differences".
The invention is illustrated by drawings, where in FIG. 1 shows a general view of the extractor; FIG. 2 is a longitudinal section through the distal end; FIG. 3 - cross section of the extractor.
9.The extractor for extracting stones from the ureter contains a supporting cable 1 made of steel wire, placed in a flexible
The extraction of stones is as follows.
The extractor is injected into the ureter. Under the control of X-ray television equipment, they are turned to stone. Moving the
Compared with the prototype of the inventive extractor reduces injuries, as due to the ability to crush large stones increases their patency in the ureter.
