Пример №1. Пациент №1 поступил с жалобами на опоясывающие боли в верхних отделах живота. Амилаза в крови составляла 1150 Ед/л. Результаты ультразвукового исследования (УЗИ): поджелудочная железа увеличена, головка 35 мм, тело 32 мм, хвост 21 мм, резко неоднородная, контур размыт, парапанкреатическая клетчатка отечна, жидкости не определяются. МСКТ картина соответствует острому отечному панкреатиту. Пациенту начата терапия согласно рекомендациям российского общества хирургов, при этом в двенадцатиперстную кишку пациента был установлен катетер 100 согласно настоящему изобретению на 24 часа, и применен метод активной аспирации. Через 24 часа уровень амилазы в крови снизился до 230 Ед/л, а болевой синдром был купирован.
Пример №2. Пациент №2 поступил с клинической картиной механической желтухи. В результате обследования был выявлен повышенный уровень лейкоцитов до 14,1*109, при этом общий билирубин составлял 145 мкМоль/л, а прямой билирубин составлял 113,2 мкМоль/л. Амилаза в крови составляла 57 Ед/л, аланинаминотрансфераза (АЛТ) составляла 391 Ед/л, а аспартатаминотрансфераза (ACT) составляла 90 Ед/л. Результаты УЗИ: в желчном пузыре множественные конкременты до 1,3 см в диаметре, а желчные протоки расширены до 11 мм. Результаты магнитно-резонансной томографии (МРТ): в дистальном отделе холедоха имеется тень, соответствующая конкременту размером 6 мм. Результаты эндоскопической ретроградной холангиопанкреатографии (ЭРХПГ): выявлен единичный конкремент размером 8 мм, и в дальнейшем был эндоскопически удален. Для профилактики острого панкреатита в двенадцатиперстную кишку пациента был установлен катетер 100 согласно настоящему изобретению на 24 часа, и применен метод активной аспирации. В послеоперационном периоде уровень амилазы в крови не повышался более 87 Ед/л.
Пример №3. Пациент №3 поступил в плановом порядке для обследования и лечения. На выполненном ранее МСКТ было выявлено образование тела поджелудочной железы. При проведении эндоскопического УЗИ был выявлено образование тела, имеющего размер 2 см на 3 см, при этом выполнение тонкоигольной биопсии оказалось технически невозможным. Для селективного забора панкреатического сока в двенадцатиперстную кишку пациента был установлен катетер 100 согласно настоящему изобретению. В результате цитологического и молекулярно-генетического исследований в полученном панкреатическом соке были обнаружены клетки аденокарциномы, а также мутации в драйверных генах, экзосомы и протеиновые маркеры.
Пример №4. Пациент №4 поступил с клинической картиной механической желтухи. Результаты МРТ: выявлен блок на уровне конфлюенса долевых правого и левого печеночных протоков, что соответствует опухоли Клацкина типа ПА. В двенадцатиперстную кишку пациента был установлен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению, и применен метод активной аспирации. В результате цитологического и молекулярно-генетического исследований в полученной желчи были обнаружены атипичные клетки, а также маркеры злокачественного новообразования.
Пример №5. Пациентке №5 был поставлен диагноз хронический панкреатит. Несмотря на проводимую гастроэнтерологом терапию, в том числе прием пациенткой назначенных ферментативных препаратов, у пациентки отмечались снижение массы тела, расстройства дефекации и диареи. Для оценки функционального состояния поджелудочной железы в двенадцатиперстную кишку пациента был установлен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению на 24 часа, и применен метод активной аспирации. В результате биохимического исследования полученного панкреатического сока были выявлены снижение уровня альфа амилазы и липазы в крови, а также снижение объема суточной экскреции панкреатического сока и желчи. Пример №6. Пациент №5 поступил в клинический инфекционный стационар города с клиникой желтухи. При физикальном осмотре печень была увеличена, при этом результаты биохимического анализа крови были следующие: общий билирубин 329 мкмоль/л, прямой билирубин 141 мкмоль/л, АЛТ 1040 Ед/л, ACT 804 Ед/л. Серологический анализ крови на антигены HCV, HBsAg отрицательный. Больному был установлен катетер для создания изолированной зоны в полом органе, при этом в результате активной аспирации было получено 100 мл желчи, а в результате ПЦР была выявлена высокая копийность вируса HCV.
Пример №7. Пациент №7 проходил лечение в инфекционной клинике по поводу уточненного вирусного гепатита. После проведенного курса интерферонов повторный ПЦР анализ крови не выявил активного вирусного процесса. Больному был установлен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению, при этом в результате активной аспирации было получено 200 мл желчи, а ПЦР анализ полученной желчи указал на продолжающийся вирусный процесс, что потребовало смены схемы и длительности терапии.
Пример №8. У пациента №8 был выявлен кишечный свищ и перитонит. В экстренном порядке выполнена операция, санация и дренирования брюшной полости. Для изолирования дефекта стенки кишечника, предотвращения контакта с кишечным содержимым, улучшения процесса заживления последнего, а также для обеспечения дополнительной функции лечения перитонита за счет разобщения брюшной полости и патологического субстрата в область травмы стенки кишечника был установлен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению таким образом, чтобы балоны располагались выше и ниже дефекта. После раздувания баллонов была создана изолированная зона, а с помощью подключения дополнительного канала к отсосу была обеспечена санация содержимого из области кишечного свища и заживление дефекта, а также обеспечена реализация введения энтерального питания больному.
Пример №9. Пациент №9 поступил в стационар в экстренном порядке с опоясывающими болями в животе. По результатам обследования пациенту был поставлен диагноз по классификации Атланта острый панкреатит, средней степени тяжести. На момент поступления уровень амилазы составлял 1320 Ед/л. Пациенту был поставлен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению, при этом функциональный канал был подключен к насосу контролируемого отрицательного давления при одновременном проведении энтерального питания и введения лекарственных средств. В результате в течение 24 часов динамика биохимических параметров стала положительной в соответствии со шкалой BIS АР, и больной выздоровел через 7 дней.
Пример №10. Пациент №10 поступил в стационар в экстренном порядке с болями в правом подреберье, гектической формой лихорадки и периодическими пожелтениями кожных покровов. Из анамнеза известно, что за 3 месяца до текущей госпитализации у пациента была выполнена эндоскопическая манипуляция на желчных протоках с целью удаления из них конкремента. УЗИ, МСКТ не показали изменений печени, протоков или наличия абсцессов. С целью диагностики холангита больному был установлен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению с последующим обеспечением забора 50,0 мл желчи, которая была отправлена на бактериологическое исследование. В результате был выявлен обильный рост Klebsiella spp., с определением чувствительности ее к антибактериальным препаратам. После подбора патогномоничной антибактериальной терапии симптомы холангита прошли, и пациент был выписан на амбулаторное лечение.
Пример №11. Пациенту №11 была проведена операция резекции сигмовидной кишки по причине объемного образования. В момент мобилизации кишечника было выявлено, что образование прорастает все слои кишечника, при этом имелась регионарная лимфаденопатия с вовлечением забрюшинной клетчатки. В момент мобилизации кишки с опухолью была установлена связь с мочеточником левой почки, который был травмирован. В операционную были приглашены урологи, которые выполнили ушивание дефекта мочеточника. Для профилактики несостоятельности ретроградно по проводнику был заведен уменьшенный катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению, один из баллонов которого был раздут в лоханке левой почки, а другой баллон (нижний баллон) был раздут в мочевом пузыре, при этом функциональный канал был подключен к отсосу контролируемого отрицательного давления. На четырнадцатые (14) сутки катетер 100 был удален, при этом экскреторная функция левой почки не была нарушена. Больной был выписан на амбулаторное лечение.Example No. 11. Пример №12. Пациент №12 поступил с клиникой легочного кровотечения. При бронхоскопии была выявлена распадающаяся кровоточащая опухоль правого главного бронха. С целью гемостаза и сохранения проходимости бронха, а также для предотвращения ателектаза легкого в правый бронх был введен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению. Катетер 100 был установлен таким образом, чтобы опухоль оказалась между баллонами, после раздувания которых кровь перестала поступать в другие отделы бронха. Для остановки кровотечения через функциональный канал катетера 100 был введен гемостатический препарат. В результате за три (3) дня наблюдения рецидива кровотечения не было. В дальнейшем катетер 100 был удален. Больной через десять (10) дней был выписан на амбулаторное лечение.
Пример №13. Пациент №13 поступил в экстренном порядке с клиникой медиастенита, который был подтвержден инструментально. В результате обследования было выявлено, что причиной стал дефект пищевода из-за некроза, вызванного куском мяса. Для лечения в пищевод установили катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению. Дефект был изолирован при раздувании баллонов катетера 100, а функциональный канал был подключен к отсосу для создания контролируемого низкого давления. В результате через 1,5 месяца пациент был выписан на амбулаторное лечение.
Пример №14. Пациент №14 поступил в экстренном порядке с головными болями и общемозговой симптоматикой. При МСКТ с внутривенным контрастированием у пациента была выявлена аневризма в бассейне правой среднемозговой артерии с признаками экстревазации. Больной был взят в ангиографическую операционную, при этом через бедренный доступ ему был введен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению. Под контролем ренетгеноскопии было осуществлено позиционирование катетера 100 таким образом, чтобы его баллоны изолировали область аневризмы, при этом функциональный канал находился напротив аневризмы. Через функциональный канал был введен пломбирующий раствор, который заполнил полость аневризмы. После необходимой экспозиции благодаря катетеру 100 был сохранен периферический кровоток, при этом в дальнейшем катетер 100 был удален. Пациент был выписан из стационара на 21 сутки.
Пример №15. Пациент №15 поступил в плановом порядке для хирургического лечения рака поджелудочной железы. При обследовании было установлено, что опухоль головки прорастает нижнюю полую вену. Для уменьшения кровопотери в вену был введен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению, а баллоны были раздуты выше и ниже опухоли головки поджелудочной железы, что обеспечило шунтирование крови и ее возврат к сердцу. Во время резекции железы участок нижней полой вены был резицирован с аутопластикой подкожной веной нижней конечности. Кровопотеря составила 500 мл. Таким образом, были созданы оптимальные как временные, так и физиологические условия как для пациента, так и для хирургов.
Пример №16. Пациент №16 поступил для хирургического лечения рака сигмовидной кишки. В послелоперационном периоде у больного развилась несостоятельность коло-ректоанастомоза на 1/3 окружности. Для лечения в кишку ретроградно через прямую кишку был заведен катетер 100 для создания изолированной зоны в полом органе согласно настоящему изобретению таким образом, чтобы один из баллонов катетера 100 был расположен дефекта проксимальнее, а другой баллон катетера 100 был расположен дистальнее дефекта. После раздувания баллонов катетера 100 зона дефекта была изолирована от других отделов толстого кишечника, и создана зона пониженного давления. За счет основного канала было выполнено очищение проксимальных отделов кишечника водой. В результате через 3 недели дефект закрылся.Example No. 16. После удаления катетера 100 аспират желчи был отправлен на генетическое и цитологическое исследования, в результате которых не было обнаружено каких-либо патологических маркеров и клеток. Таким образом, вышеописанный катетер 100 позволяет осуществить или выполнить функциональное исследование состояния любого полого органа, а также провести надлежащее лечение. Кроме того, исследование и/или лабораторный анализ биологической жидкости, собранной с использованием катетера 100, позволяет с высокой точностью дифференциально диагностировать воспалительные образования, доброкачественные образования и злокачественные образования, а также инфекционные болезни. За счет эвакуации специфической жидкости катетер 100 позволяет осуществлять лечение воспалительных заболеваний, кровотечений и дефектов стенок полых органов. Создание изолированной зоны с функцией шунтирования во время хирургической манипуляции позволяет осуществлять контроль гемостаза, а также выполнять хирургические манипуляции, в частности резекции участка сосудов с последующей их пластикой. Следует также отметить, что катетер 100 согласно настоящему изобретению или вышеописанная система для создания изолированной зоны в полом органе млекопитающего, в состав которой может входить катетер 100, обеспечивает возможность получения большого объема биологического материала в нормофизиологическом состоянии без дополнительной стимуляции экскреторной функции полого органа, в частности только благодаря созданию в изолированной зоне полого органа, в просвет которого вставлен катетер 100, отрицательного давления, соответствующего отрицательному физиологическому давлению, которое может быть создано, например, за счет перистальтики самого кишечника.Thus, the above-described
