Изобретение относится к медицине и применяется для профилактики стенозов трахеи, вызванных давлением на ее слизистую оболочку манжет эндотрахеальных (трахеотомических или интубационных) трубок и аспирационных осложнений при проведении длительной искусственной вентиляции легких (ИВЛ).The invention relates to medicine and is used to prevent tracheal stenosis caused by pressure on its mucous membrane of the cuffs of the endotracheal (tracheotomy or endotracheal) tubes and aspiration complications during prolonged mechanical ventilation (IVL).
Известно, что основной причиной рубцового стеноза трахеи является длительная ИВЛ. По мнению ряда авторов, частота осложнений длительной ИВЛ через трахеостомическую или интубационную трубку достигает 80%. Из всех осложнений на долю рубцового стеноза трахеи приходится до 25% случаев. Доля постинтубационных стенозов трахеи в структуре заболеваемости составляет от 14 до 45%, посттрахеостомических - от 51 до 73%. Главным травмирующим фактором считается превышение давления в манжете интубационной трубки по отношению к капиллярному давлению слизистой трахеи. Соблюдение режимов рекомендованного давления в манжете интубационной трубки в пределах 25-30 мм рт.ст. (3333,05-3999,66 Па) не всегда позволяет избежать повреждений слизистой оболочки трахеи. При массивной кровопотере, снижении артериального давления центрального генеза наблюдается уменьшение капиллярного давления в слизистой оболочке. В результате возникшей разницы между давлением в манжете интубационной трубки и капиллярным давлением возникает ишемия слизистой оболочки с последующим некрозом, метаплазией эпителия и развитием продуктивного воспаления в месте прилегания манжеты к слизистой трахеи. По команде с пульта управления 6 включается компрессор 9 и регулятор давления 10 настроенный на заданную величину давления P1. Пневмораспределители 13, 16, 24, 21 находятся в закрытом положении. После достижения давления в ресивере 12, которая дополнительно контролируется манометром 11, компрессор 12 отключается, открываются пневмораспределитель 13 и сжатый воздух из ресивера 12 через пневмораспределитель 13 поступает через отвод к трубке для изменения давления воздуха в манжетах 15 в трубку для изменения давления воздуха в манжетах 4 и в манжету 2, в которой создается давление уровня Р1. Давление в манжете 2 контролируется по манометру 14. При снижении давления в ресивере 12 ниже заданного промежутка P1 предается сигнал на пульт управления 6 и по команде с пульта управления 6 автоматически включается компрессор 9 до достижения в ресивере 12 давления P1.
