Description

이식가능한 액세스 포트 장치 및 부착 시스템 {Implantable Access Port Device and Attachment System}

발명의 개요

실시예
시스템의 용도
하기 실시예는 위 밴딩 환자에 액세스 포트 어셈블리(10)를 부착하기 위한 본 명세서에서 도시되고 설명된 본 발명의 시스템(20)을 사용하는 한 방식을 설명한다.
일반적으로 도면을 참조하면, 의사는 변형 완화 요소(50)의 록킹 말단(locking end) 너머로 연장하는 약 2 cm의 튜빙을 남기고 변형 완화 요소(50)를 연질 튜빙(soft tubing; 6) 위로 스레딩(thread)한다.
그리고 나서, 포트 하우징(54)의 하우징 측벽(66)을 갖는 플러시(flush)까지 튜빙을 바브(48)에 커플링한다. 그리고 나서, 변형 완화 요소(50)를 눌러 끼우고 커플러(51)에 록킹한다.
의사는 도 7에 나타난 바와 같이 트리거 메커니즘(104)이 완전히 열린 위치인지 확인한다.

의사는 지방이 완전히 제거되어 직근막을 볼 수 있는지 확인한다. 액세스 포트 어셈블리(10)가 커플링된 도구(30)의 운송 헤드(96)를 삽입을 용이하게 하는 각도의 위치에서 절개된 포켓에 넣는다. 액세스 포트 어셈블리(10)를 근막에 대고 평평하게 위치시켜 모든 앵커(74)가 확실하게 근막 및/또는 근육 조직을 붙잡도록 한다. 의사는 운송 헤드(96)의 상부(100)에 손가락 압력을 가하여 액세스 포트 어셈블리(10)가 근막에 대해 평평하게 되도록 확실히 하고 발사를 위해 도구(30)를 움직이지 않게 한다.
의사는 트리거 메커니즘(104)이 완전히 닫혀서, 그에 의해서 앵커(74)가 아래에 있는 근막 내로 전개될 때까지 트리거 메커니즘(104)을 단단히 꽉 쥔다. 이때, 도 8B에 나타낸 바와 같이 활성화 매커니즘(98)이 닫힘 위치로 록킹되고 도 13, 도 15 및 도 17에 나타낸 바와 같이 앵커(74)가 완전히 전개된다.
도구(30)를 액세스 포트 어셈블리(10)로부터 분리하기 위하여, 의사는 운송 헤드(96)를 예를 들어, 수평으로 또는 측면으로 액세스 포트 어셈블리(10)로부터 멀리 슬라이딩시키고, 도구(30)를 절개 부위로부터 들어올린다. 의사는 손가락으로 액세스 포트 어셈블리의 베이스 주위를 훑어서 앵커(74)가 완전히 근막 내에 고정되었는지 확인한다.
액세스 포트 어셈블리(10)가 근막에서 떨어져 있어서 재배치되는 경우, 트리거 해제 버튼을 눌러 도 8B에 도시된 위치로부터 래치 메커니즘을 풀어서 트리거 메커니즘(104)을 해제한다. 트리거가 완전히 열리면 스테인리스 강 앵커가 액세스 포트 어셈블리(10) 내로 다시 완전하게 뒤로 물러날 것이다. 그리고 나서, 상기한 바와 같이 도구(30)를 사용하여 액세스 포트 어셈블리(10)를 상이한, 예를 들어, 더욱 바람직한 위치에 다시 전개시킬 수 있다.

Claims (20)

  1. 액세스 포트를 신체 조직에 부착하기 위한 시스템으로서,
    대체적인 중심축을 가지며 바닥, 측벽 및 니들 침투가능한 격막을 포함하는 액세스 포트와,
    액세스 포트가 신체 조직에 부착될 수 있게 하는 구조를 갖는 부착 메커니즘을 포함하는
    액세스 포트 어셈블리;
    대체적인 세로축을 갖는 핸들과,
    액세스 포트 어셈블리와 맞물리는 구조를 갖는 운송 헤드와,
    운송 헤드가 액세스 포트 어셈블리와 그렇게 맞물릴 때 부착 메커니즘의 전개를 가능하게 하기 위한 활성화 메커니즘을 포함하는
    운송 도구를 포함하며;
    운송 도구는 운송 헤드가 액세스 포트 어셈블리와 그렇게 맞물릴 때 핸들의 세로축이 액세스 포트의 대체적인 중심축으로부터 이격되도록 구성되는 시스템.
  2. 제1항에 있어서, 액세스 포트 바닥은 액세스 포트 측벽과 일체형(integral)인 시스템.
  3. 제2항에 있어서, 액세스 포트 측벽 및 액세스 포트 바닥은 단일의 형태(unitary configuration)인 시스템.
  4. 제3항에 있어서, 액세스 포트 측벽 및 액세스 포트 바닥은 티타늄인 시스템.
  5. 제2항에 있어서, 액세스 포트 어셈블리는 액세스 포트 측벽을 둘러싸는 하우징 측벽을 포함하는 하우징 및 하우징 측벽에 대해 회전가능한 액츄에이터 어셈블리를 추가로 포함하는 시스템.
  6. 제1항에 있어서, 도구의 활성화 메커니즘은 케이블 메커니즘을 포함하는 시스템.

  11. 제1항에 있어서, 부착 메커니즘은 적어도 하나의 와이어 앵커를 포함하는 시스템.
  12. 제11항에 있어서, 와이어 앵커는 다중면 팁을 갖는 구부러진 와이어를 포함하는 시스템.
  13. 제1항에 있어서, 부착 메커니즘은 곡선형의 말단부 및 곡선형의 말단부에 실질적으로 수직인 피봇형의 근위부를 갖는 적어도 하나의 앵커를 포함하는 시스템.
  14. 제1항에 있어서, 부착 메커니즘은 나선형의 말단부 및 나선형의 말단부에 실질적으로 수직인 직선형의 근위부를 갖는 적어도 하나의 앵커를 포함하는 시스템.
  15. 액세스 포트 어셈블리를 신체 조직에 부착하기 위한 운송 도구로서, 액세스 포트 어셈블리는 전개가능한 패스너 및 액세스 포트 및 대체적인 중심축을 포함하며, 도구는
    대체적인 세로축을 갖는 핸들과,
    전개가능한 패스너를 갖는 액세스 포트 어셈블리에 맞물리는 구조를 가지며, 핸들의 말단 단부에 있는 운송 헤드와,
    운송 헤드가 액세스 포트 어셈블리와 그렇게 맞물릴 때 패스너의 전개를 가능하게 하기 위한 활성화 메커니즘을 포함하며;
    운송 도구는 운송 도구가 액세스 포트 어셈블리와 그렇게 맞물릴 때 핸들의 세로축이 액세스 포트의 대체적인 중심축으로부터 이격되도록 구성되는 도구.
  16. 제15항에 있어서, 도구의 활성화 메커니즘은 케이블 메커니즘을 포함하는 도구.
  17. 제16항에 있어서, 케이블 메커니즘은 텅스텐 재료를 포함하는 케이블을 포함하는 도구.
  18. 제16항에 있어서, 케이블 메커니즘은 핸들을 따라 운송 헤드로 연장하는 캐이블을 포함하는 도구.
  19. 제16항에 있어서, 운송 헤드가 액세스 포트 어셈블리와 그렇게 맞물릴 때 케이블 메커니즘의 세로방향 이동이 부착 메커니즘의 회전 운동을 야기하는 도구.
  20. 제18항에 있어서, 케이블은 실질적으로 직선형의 근위부 및 곡선형의 말단부를 포함하는 도구.
