Abstract

골융합 임플란트의 단부 부재{END MEMBER FOR A BONE FUSION IMPLANT}
뼈의 결함을 치료 또는 취급하는 것은 정형외과에서 가장 주목하고 있는 문제 중의 하나이다. 뼈의 결함은 다양한 임상 상태에서 발생한다. 뼈에 결함이 있는 임의의 길이의 뼈를 자연 해부학적 길이 및 형상으로 회복하는 것은 불확실하다. 더욱이, 척추골을 제거해야 하는 경우에는, 척추의 자연적인 길이 및 곡률을 회복하고 뼈의 융합을 촉진하기 위하여 척추골 스페이서(spacer)를 삽입해야 한다. 척추의 해부학적 구조와 안정성을 회복하고 뼈의 융합을 촉진하는 것은 코펙토미(corpectomy), 즉 하나 또는 몇 개의 척추골의 한 섹션이 제거되는 수술 절차에서는 더욱 어렵게 된다.
뼈의 결함에 사용되기 위한 다양한 임플란트가 개발되어 왔다. 소위 "메쉬 임플란트(mesh implants)"가 특히 효과적인 것으로 나타났다. 이와 같은 임플란트의 하나가 미국의 펜실바니아주 파올리 소재의 신세스(Synthes)(USA)로부터 입수 가능한 신메쉬(SynMeshTM)(등록상표명)이다. 신메쉬는 복수개의 균일하게 이격된균일한 크기의 구멍이 표면 상에 분포되어 있는 티타늄 메쉬 실린더이다. 뼈에 결함이 발생하는 다양한 임상 상태를 반영하기 위해, 신메쉬는 다양한 길이와 직경으로 입수 가능하다. 뼈의 결함을 치료하기 위한 상용 가능한 동종 이식체(allograft)가 최근에 입수 가능해졌다.
메쉬 및 동종 이식체의 임상 결과가 일반적으로 긍정적이나, 하나의 가능한 임상적 합병증은 임플란트의 침하이다. 임플란트의 단부가 접촉하는 뼈 속으로 과도하게 침하하는 것은 뼈가 극히 다공성인 경우에 특히 문제가 된다. 역학적 및 생리학적 시점에서 볼 때, 임플란트와 뼈 사이의 접촉 면적을 증가시키는 것이 유리하다. 비더만(Biedermann) 등의 미국 특허 제5,702,451호는 이러한 문제를 다룬 시도로서 단부 링을 개시한다. 그러나, 개시된 단부 링은 보유, 삽입 및 신연(distraction) 도구를 위한 어떠한 설비도 구비하지 않고 있다. 따라서, 이식이 어려울 수 있다. 더욱이, 이식 후에는 뼈와 개시된 단부 링 사이에 과도한 움직임이 있을 수 있다. 이는 뼈의 융합을 지연시키거나, 아마도 방해할 것이다.
이상의 논의가 나타내는 바와 같이, 골융합 임플란트와 사용되는 개량된 단부 부재가 요구된다.
본 발명은 뼈, 특히 척추골의 융합을 위한 장치에 관한 것으로, 특히 독립항인 청구항 제1항, 제37항 및 제38항의 전제부에 기재된 바와 같은 뼈 또는 특히 척추골을 융합하기 위한 임플란트의 단부 부재에 관한 것이다.

본 발명은 이러한 문제를 뼈, 특히 척추골의 융합을 위한 장치, 특히 청구항 제1항, 청구항 제37항 및 청구항 제38항의 특징부에 의해 특징 지워지는 뼈, 특히 척추골을 융합하기 위한 임플란트를 위한 단부 부재에 의해 해결한다.
본 발명은 뼈의 융합 부분에서 골융합 임플란트와 사용되는 단부 부재에 관한 것이다. 단부 부재는 제1 부분과, 임플란트의 보어 내로 삽입되는 크기로 된 제2 부분과, 제1 부분과 제2 부분 사이의 견부를 갖는다. 제2 부분이 보어 내로 삽입되면, 견부는 임플란트의 모서리 상에 놓여진다. 제1 부분의 상부면은 뼈의 크기 및 형상과 일치하며 수술 도구를 수용하기 위한 하나의 채널 또는 복수개의 채널을 갖는다. 복수개의 채널이 존재하는 경우, 모든 채널들은 동일한 방향으로 연장되거나(예를 들어, 채널은 전측방향(anterolateral direction)으로 연장된다), 채널들은 상이한 방향으로 연장될 수 있다(예를 들어, 제1 채널은 전후 방향으로 연장되고 제2 채널은 측방향으로 연장된다).
임플란트가 금속으로 제작되면, 단부 부재는 부식되지 않기 위해 동일한 재료로 제작된다. 단부 부재는 또한 동종 이식 뼈로 제작될 수 있다. 단부 부재는 뼈의 자연 곡률을 회복하거나 비직교 골절단(non-orthogonal osteotomy)을 교정하기 위해 쐐기 형태 또는 구형 외형을 가질 수 있다. 쐐기형 외형을 생성하기 위한 하나의 방법은 제1 부분의 제1 단부를 제2 단부보다 두껍게 하는 것이다.
상부면은 타원형, 직사각형 또는 원형의 임의의 적절한 단면으로 제작될 수 있으며, 환형 또는 중실(solid)일 수 있다. 상부면은 평탄한 평면이거나 뼈의 형태를 모방한 곡률을 가질 수 있다. 상부면에는 뼈와 맞물리기 위한 치부(teeth) 또는 표면 조직(texture)이 구비될 수 있다. 치부는 피라밋 또는 톱니 형태를 가질 수 있다.
단부 부재를 임플란트에 고정시키기 위해, 제2 부분은 체결구(fastener)를 수용하기 위한 구멍을 가질 수 있다. 이와는 다르게, 제2 부분의 외부면은 임플란트의 구멍 내에 체결되는 크기로 된 돌기부를 가질 수 있다. 예시적인 실시예에 있어서, 제2 부분은 단부 부재를 임플란트에 고정시키기 위해 복수개의 탭을 포함한다. 이들 탭은 제2 부분이 임플란트 내로 삽입될 시에 탭이 내측으로 휘고 나서 다시 외측으로 휘어져서 단부 부재를 임플란트에 고정시키는 억지 끼움을 형성하도록 탄성적일 수 있다. 또 다른 실시예에 있어서, 단부 부재는 제1 부분에 힌지식으로 연결되거나 피벗식으로 연결된 돌기와, 제2 부분 내의 나사 보어와, 이 나사 보어 내에 삽입 가능한 고정 나사를 포함한다. 고정 나사를 보어 내에 나사 체결하면 돌기를 외측으로 이동시켜서 단부 부재가 임플란트에 고정되게 된다.
또 다른 실시예에 있어서, 본 발명에 따른 단부 부재는 뼈와 크기 및 형태가 일치하는 상부면과, 상부면으로부터 연장되며 임플란트의 단부를 수용하는 형태와 크기로 된 슬리브를 포함한다. 단부 부재는 슬리브에 의해 임플란트의 단부를 수용하기 용이하도록 된 개방 위치와 단부 부재를 임플란트에 고정시키는 폐쇄 위치 사이에서 피벗 가능한 제1 및 제2 부분을 포함한다. 로킹 요소는 단부 부재를 폐쇄 위치에서 고정시킨다.
또 다른 실시예에 있어서, 단부 부재는 경사면을 갖는 프롱(prong)을 갖는 외측 링과 이 외측 링에 대하여 회전 가능한 내측 링을 포함한다. 내측 링은 회전 시에 프롱의 경사면과 접촉함으로써 프롱을 반경 방향 외측으로 추진하는 돌기를 갖는다.
본 발명의 바람직한 특징들이 첨부 도면에 개시되었으며, 도면에서 동일한 참조 부호는 여러 개의 도면에 걸쳐 동일한 요소를 가리킨다.
도 1은 본 발명에 따른 단부 부재(10)의 제1 실시예를 도시한다. 단부 부재(10)는 제1 부분(12)과 제2 부분(14)을 갖는다. 제1 부분(12)이 제2 부분(14)보다 크므로, 견부(16)는 제1 부분과 제2 부분 사이의 교차부에 형성된다. 제1 부분(12)의 상부면(18)에는 보유, 삽입 및/또는 신연(distraction) 도구와 같은 수술 기구를 수용하기 위한 제1 채널(20)이 구비된다. 상부면(18)은 둥근 형상으로 도 1에 도시되었다. 그러나, 다른 실시예들에서 자명한 바와 같이, 상부면(18)은 임의의 적절한 형상을 가질 수 있다. 바람직하게는, 상부면(18)은 상부면이 접촉하게 되는 뼈의 형상과 일치하는 형상을 갖는다.
제2 부분(14)은 또한 둥근 형상으로 도시되었다. 상부면(18)의 경우에서와 같이, 제2 부분(14)은 임의의 적절한 형상을 가질 수 있다. 바람직하게는, 제2 부분(14)의 형상은 단부 부재와 사용되는 골융합 임플란트의 형상과 일치한다. 도 2에 잘 도시된 바와 같이, 제2 부분(14)은 견부(16)가 골융합 임플란트(22) 상에 놓이도록 골융합 임플란트(22)의 내측 보어 내에 삽입되는 크기와 형태를 갖는다. 골융합 임플란트(22)는 예를 들어 메쉬 임플란트, 동종 이식 임플란트 또는 임의의 금속 또는 비금속 임플란트를 포함한 여러 가지 다양한 임플란트 형태일 수 있다. 임플란트(22)가 금속 재료로 제작되면, 단부 부재(10)는 바람직하게는 혼합 금속 부식(갈바닉 부식)을 피하기 위해 동일한 금속 재료 또는 비금속 재료로 제작된다. 단부 부재(10)는 또한 망상 조직 뼈, 외피 뼈, 망상 조직 및 외피 뼈의 혼합 또는 망상 조직 및 외피 뼈의 합성으로부터의 동종 이식 뼈로 제작될 수 있다. 단부 부재(10) 및 골융합 임플란트(22)의 이식 후에, 생리학적 압축력이 단부 부재(10)를골융합 임플란트(22)에 대한 제위치에 보유하게 된다. 그러나, 추가의 안정성을 제공하고 수술 전 및 수술 중 취급을 돕기 위해, 제2 부분(14)은 골융합 임플란트(22)에 고정될 수 있다. 예를 들어, 제2 부분(14)은 골융합 임플란트(22) 내에 억지 끼워지거나 스냅식으로 끼워질 수 있다. 부가적으로, 골융합 임플란트(22)가 통상적으로 변형 가능하기 때문에, 골융합 임플란트(22)가 단부 부재(10)에 찌그러질 수 있다. 제2 부분(14)에는 또한 핀 또는 나사와 같은 체결구를 수용하기 위한 구멍(24)이 구비될 수 있다. 단부 부재(10)는 구멍(24)이 골융합 임플란트(22) 상의 구멍(26)들 중 하나와 정렬되도록 골융합 임플란트(22) 내로 삽입된다. 구멍(24)은 제2 부분(14)을 통해 부분적으로 또는 완전히 연장될 수 있으며 나사부가 형성될 수 있다. 도 6에 도시된 바와 같이, 제2 부분(14)은 다르게는 단부 부재를 골융합 임플란트에 기계적으로 고정시키기 위하여 구멍(26) 내에 체결되는 크기로 된 돌기(28)를 가질 수 있다. 단부 부재를 골융합 임플란트에 고정시키기 위한 다른 예시적 기구가 이하에 보다 상세하게 설명되었다.
다시 도 1과 도 2를 참조하면, 상부면(18)의 표면적은 골융합 임플란트(22)의 상부면과 하부면의 표면적보다 크다. 이와 같이 큰 표면적 때문에, 상부면(18)과 주변 뼈 사이의 접촉 면적이 임플란트(22)의 상부면과 하부면에서보다 크게 된다. 접촉 면적의 증가는 임플란트(22)의 침하 방지를 돕는다. 접촉 면적의 증가는 단부 부재(10)와 주변 뼈 사이에 하중이 더 많이 분배되는 것과 같은 다른 이점도 있다. 상부면(18)은 환형 공간(30)이 있는 링으로 도시되었다. 새로운 뼈는 환형 공간(30) 내에 형성되어 융합을 증진시킬 수 있다. 환형 공간은 뼈 조각 또는 임의의 다른 골유도성(osteoinductive) 또는 골전도성(osteoconductive) 재료로 충전되어 뼈의 형성을 증진시킬 수 있다.
이와는 다르게, 도 7에 도시된 바와 같이, 상부면(18)은 상부면(18)과 주변 뼈 사이의 접촉 면적을 최대화하기 위해 중실 표면일 수 있다. 고른 상부면(18)은 침하가 특히 염려되는 임상 상태에서 바람직하다.
도 3은 본 발명에 따른 단부 부재(110)의 제2 실시예를 도시한다. 일반적으로, 대부분의 단부 부재(110)(및 이하에 설명된 실시예들)의 구조는 단부 부재(10)의 구조와 유사하거나 동일하고, 따라서 동일한 참조 번호가 동일한 구성 요소에 사용되며 이들 동일한 구성 요소에 관한 논의는 불필요할 것으로 믿어진다. 단부 부재(110)는 타원형 또는 직사각형이며 주변의 뼈(그리고 결과적으로 골융합 임플란트)가 대체로 타원 또는 직사각형인 상태에 사용될 것이다. 단부 부재가 단부 부재(110)와 같이 비대칭형인 경우, 제1 채널(20)은 임의의 배향으로 제공될 수 있다. 예를 들어, 제1 채널(20)은 단부 부재(110)의 장축을 따라 연장되는 것으로 도시되었으나, 주변 뼈에 대하여 상이하게 배향되도록 임의의 방향으로 연장될 수 있다. 제1 채널(20)의 배치의 다양성은 제1 채널(20)이 사용되고 있는 특정의 수술 방법에 가장 적합하게 위치될 수 있음을 의미한다. 구체적으로, 단부 부재(110)가 단부 부재의 단축이 전후 방향인 상태로 이식되는 경우에, 제1 채널(20)은 횡방향 수술 방법에 이상적이다. 만약 전방 또는 후방 접근이 예견된다면, 제1 채널(20)은 전후 방향으로 연장되어야 할 것이다.
도 4a는 제1 채널(20)이 단부 부재(210)의 장축에 대하여 임의의 각도로 연장되는 단부 부재(210)를 도시한다. 제1 채널(20)의 이 방향은 척추 수술에 사용될 수 있는 전측방향 수술 접근에 바람직하다. 도 4b는 제1 및 제2 채널(20, 21) 모두가 단부 부재(212)의 장축에 대하여 임의의 각도로 연장되는 단부 부재(212)를 도시한다. 바람직하게는, 제1 및 제2 채널(20, 21)이 단부 부재(212)의 장축에 대하여 대칭적으로 위치한다. 단부 부재(212)에 제1 및 제2 채널(20, 21) 모두를 제공하는 것은 좌측 또는 우측이 전측방향 수술 접근에 사용되더라도 상관없이 이식 중에 제1 및 제2 채널(20, 21) 중 적어도 하나가 접근되는 것을 허용한다. 더욱이, 채널(20, 21)의 배열은 하나의 단부 부재가 골융합 임플란트의 각 단부에 사용되도록 하며, 여전히 상부 단부 부재 상의 채널들은 하부 단부 부재 상의 채널들과 정렬된다. 도 4c는 제1 및 제2 채널(20, 21)을 역시 포함하는 단부 부재(214)를 도시한다. 제1 채널(20)은 단부 부재(214)의 장축에 평행하게 연장되며 제2 채널(21)은 단부 부재(214)의 장축을 가로질러 연장된다. 제1 및 제2 채널(20, 21)을 단부 부재(214) 상에 포함함으로써 단부 부재(214)의 이식 방향 또는 수술 접근에 무관하게 제1 및 제2 채널(20, 21)의 적어도 하나에는 접근할 수 있게 된다.
도 4a, 도 4b 및 도 4c에 도시된 각각의 단부 부재들에 있어서, 상부면(18)은 단부 부재와 주변의 뼈 사이에 기계적인 맞물림을 제공하는 복수개의 치부(32)를 갖는다. 치부(32)는 뼈 속으로 침입함으로써 기계적인 맞물림을 제공한다. 치부(32)에 의해 제공되는 초기의 기계적인 안정성은 단부 부재의 수술 후 제거 또는 탈거의 위험을 최소화한다. 치부(32)는 피라밋형, 톱니형 등과 같은 임의의 적절한 형태를 가질 수 있다. 이와는 다르게, 상부면(18)은 단부 부재와 주변의 뼈 사이에 기계적인 맞물림을 제공하도록 표면 조직을 가질 수 있다.
도 5에 도시된 바와 같이, 단부 부재(310)는 쐐기 외형을 가질 수 있다. 쐐기 외형은 척추 또는 임의의 뼈의 자연 곡률을 회복하기 위해 척추 적용에 유용하다. 쐐기 외형은 또한 뼈를 제거하기 위해 병리학, 임상 상태에 의해 절단 지시된 또는 잘못 이루어진 비직교 골절단을 보상하는 데 유용하다. 단부 부재가 통상적으로 골융합 임플란트의 각 단부 상에 사용되므로, 상이한 쐐기 외형을 갖는 단부 부재가 함께 사용될 수 있음에 유의해야 한다. 이러한 쐐기 형상을 달성하는 하나의 방법은 상부면(18)이 각도(α)에 의해 형성되는 경사를 갖도록 제1 단부(34)로부터 제2 단부(36)까지 제1 부분(12)의 두께를 점진적으로 감소시키는 것이다. 도 5에 있어서, 상부면(18)은 평탄한 평면으로 도시되었다. 그러나, 상부면(18)은 곡면일 수 있으며, 여전히 쐐기형 외형을 보유할 수 있다. 곡선이 척추골 단부판의 해부학적 형태의 거울상으로 제작될 수 있으므로, 만곡된 상부면(18)은 척추골 단부판의 윤곽선과 일치하게 된다.
앞서 논의한 바와 같이, 도 6은 골융합 임플란트(22)(도2)의 구멍(26)들에 끼워지는 크기의 돌기(28)들이 구비된 단부 부재(410)를 도시한다. 역시 논의되었던 도 7은 침하 발생율이 높은 경우에 사용하는 것이 바람직한 중실 상부면(18)을 갖는 단부 부재(510)를 도시한다.
도 8은 단부 부재(610)를 골융합 임플란트(22)에 고정시키기 위한 복수개의 탭(40)을 포함하는 제2 부분(14)을 갖는 단부 부재(610)를 도시한다. 일 실시예에있어서, 탭(40)들은 단부 부재(610)의 제2 부분(14)이 골융합 임플란트 내로 가압된 때, 탭(40)들이 내측으로 휘어지고 나서 단부 부재(610)를 골융합 임플란트(22)에 고정시키도록 외측으로 다시 복귀할 수 있도록 탄성적이다. 골융합 임플란트(22)가 탭(40)을 수용하기 위해 외측으로 휘어지도록 어느 정도의 탄성을 가지므로, 탭(40)은 탄성일 필요는 없다. 탭(40)의 개수, 크기 및 배열은 변경될 수 있다. 도 9는 전체적인 형상이 변경(원형에서 타원 또는 직사각형)된 것을 제외하고는 단부 부재(610)와 유사한 단부 부재(710)를 도시한다.

이상 본 발명이 다양하게 설명되었으나, 다양한 특징이 단독으로 또는 조합되어 사용될 수 있음을 이해해야 한다.
그러므로, 본 발명은 본 명세서에 기재된 구체적인 바람직한 실시예들로만 한정되지 않는다.
또한, 본 발명의 범주 내에서의 변형 및 수정이 본 발명이 속하는 분야의 숙련자에 의해 이루어질 수 있음을 이해하여야 한다. 따라서, 본 명세서의 기재로부터 당해 분야의 숙련자에 의해 용이하게 달성될 수 있으며 본 발명의 범위 내에 있는 모든 적당한 수정은 본 발명의 또 다른 실시예로서 포함된다. 본 발명의 범위는 따라서 첨부된 청구의 범위에 기재된 바와 같이 한정된다.

Claims (38)

  1. A) 뼈, 특히 척추골의 단부판과 접촉하는 상부면(18, 812)과 종축(27A)을 갖춘 제1 부분(12)과, B) 종축(27)과 동축이며 임플란트의 단부에 장착되는 형태와 크기로 된 제2 부분(14)과, C) 제1 및 제2 부분(12;14)을 연결하며, 제2 부분(14)이 임플란트에 장착된 때 임플란트의 모서리 상에 위치되는 크기로 된 견부(16)를 포함하는 뼈, 특히 척추골을 융합하기 위한 임플란트와 사용되는 단부 부재에 있어서,
    D) 상부면(18)은 수술 도구를 수용하기 위한 제1 채널(20)을 갖는 것을 특징으로 하는 단부 부재.
  2. 제1항에 있어서, 제2 부분(14)은 임플란트의 보어 내에 삽입되는 형태와 크기로 된 것을 특징으로 하는 단부 부재.
  3. 제1항에 있어서, 제2 부분(14)은 임플란트의 단부를 수용하는 형태와 크기를 갖는 슬리브(814)를 포함하는 것을 특징으로 하는 단부 부재.
  4. 제1항 내지 제3항 중 어느 한 항에 있어서, 상부면(18;812)은 수술 도구를 수용하기 위한 제2 채널(21)을 갖는 것을 특징으로 하는 단부 부재.
  5. 제1항 내지 제3항 중 어느 한 항에 있어서, 채널(20)은 기본적으로 종축(27)에 직각으로 제1 부분(12)을 관통하는 것을 특징으로 하는 단부 부재.
  6. 제4항 또는 제5항에 있어서, 제2 채널(21)은 기본적으로 종축(27)에 직각으로 제1 부분(12)을 관통하는 것을 특징으로 하는 단부 부재.
  7. 제1항 내지 제6항 중 어느 한 항에 있어서, 제1 채널(20)은 상부면(18;812)을 향해 개방된 것을 특징으로 하는 단부 부재.
  8. 제4항 내지 제7항 중 어느 한 항에 있어서, 제2 채널(21)은 상부면(18;812)을 향해 개방된 것을 특징으로 하는 단부 부재.
  9. 제1항 내지 제8항 중 어느 한 항에 있어서, 제1 채널(20)은 종축(27)에 대하여 대칭 형태인 것을 특징으로 하는 단부 부재.
  10. 제4항 내지 제9항 중 어느 한 항에 있어서, 제2 채널(21)은 종축(27)에 대하여 대칭 형태인 것을 특징으로 하는 단부 부재.
  11. 제1항 내지 제10항 중 어느 한 항에 있어서, 제1 채널(20)은 제1 부분(12)의 단면의 20％ 내지 80％를 차지하는 것을 특징으로 하는 단부 부재.
  12. 제4항 내지 제10항 중 어느 한 항에 있어서, 제2 채널(21)은 제1 부분(12)의 단면의 20％ 내지 50％를 차지하는 것을 특징으로 하는 단부 부재.
  13. 제4항 내지 제12항 중 어느 한 항에 있어서, 제1 채널(20)은 제2 채널(21)에 대하여 임의의 각도로 연장하는 것을 특징으로 하는 단부 부재.
  14. 제4항 내지 제12항 중 어느 한 항에 있어서, 제1 채널(20)은 제2 채널(21)에 평행하게 연장하는 것을 특징으로 하는 단부 부재.
  15. 제1항 내지 제14항 중 어느 한 항에 있어서, 상부면(18;812)은 뼈와 맞물리기 위한 복수개의 치부(32)를 추가로 포함하는 것을 특징으로 하는 단부 부재.
  16. 제15항에 있어서, 치부(32)는 피라밋 형상을 갖는 것을 특징으로 하는 단부 부재.
  17. 제1항 내지 제16항 중 어느 한 항에 있어서, 상부면(18;812)은 중실 표면인 것을 특징으로 하는 단부 부재.
  18. 제1항 내지 제17항 중 어느 한 항에 있어서, 상부면(18;812)은 환형인 것을특징으로 하는 단부 부재.
  19. 제1항 내지 제17항 중 어느 한 항에 있어서, 상부면(18;812)은 원형인 것을 특징으로 하는 단부 부재.
  20. 제1항 내지 제17항 중 어느 한 항에 있어서, 상부면(18;812)은 타원형인 것을 특징으로 하는 단부 부재.
  21. 제1항 내지 제17항 중 어느 한 항에 있어서, 상부면(18;812)은 직사각형인 것을 특징으로 하는 단부 부재.
  22. 제1항 내지 제21항 중 어느 한 항에 있어서, 상부면(18;812)은 평탄한 평면인 것을 특징으로 하는 단부 부재.
  23. 제1항 내지 제22항 중 어느 한 항에 있어서, 단부 부재는 쐐기형 외형을 갖는 것을 특징으로 하는 단부 부재.
  24. 제23항에 있어서, 제1 부분(12)은 제1 단부(34)와 제2 단부(36)를 가지며, 제1 단부의 높이는 제2 단부의 높이보다 커서 쐐기형 외형을 제공하는 것을 특징으로 하는 단부 부재.
  25. 제1항 내지 제24항 중 어느 한 항에 있어서, 단부 부재는 임플란트에 체결 가능한 것을 특징으로 하는 단부 부재.
  26. 제25항에 있어서, 단부 부재는 임플란트와 해제 가능하게 연결 가능한 것을 특징으로 하는 단부 부재.
  27. 제26항에 있어서, 제2 부분(14)은 단부 부재를 임플란트에 고정시키기 위해 체결구를 수용하기 위한 구멍(24)을 포함하는 것을 특징으로 하는 단부 부재.
  28. 제1항 또는 제2항에 있어서, 제2 부분(14)의 외부면은 단부 부재를 임플란트에 고정시키기 위해 구멍에 끼워지는 크기의 적어도 하나의 돌기(28)를 갖는 것을 특징으로 하는 단부 부재.
  29. 제1항 또는 제2항에 있어서, 제2 부분(14)은 단부 부재를 임플란트에 고정시키기 위한 복수개의 탭(40)을 포함하는 것을 특징으로 하는 단부 부재.
  30. 제29항에 있어서, 각각의 복수개의 탭(40)은 탄성적이어서, 단부 부재를 임플란트에 고정하도록 제2 부분(14)을 임플란트에 삽입할 때 내측으로 휘어지고 나서 다시 외측으로 휘어지는 것을 특징으로 하는 단부 부재.
  31. 제1항 내지 제30항 중 어느 한 항에 있어서, 단부 부재는 임플란트와 동일한 재료로 제작되는 것을 특징으로 하는 단부 부재.
  32. 제1항 내지 제30항 중 어느 한 항에 있어서, 단부 부재는 동종 이식 뼈로 제작되는 것을 특징으로 하는 단부 부재.
  33. 제1항 또는 제2항에 있어서, 제1 부분(12)에 피벗식으로 연결되는 돌출부(952)와, 제2 부분(14) 내의 나사 보어(956)와, 이 나사 보어 내에 나사식으로 삽입 가능한 고정 나사(954)를 추가로 포함하고, 나사 보어(956) 내에 고정 나사(954)를 삽입하는 것은 돌출부(952)를 외측으로 이동시켜서 단부 부재가 임플란트에 고정되게 하는 것을 특징으로 하는 단부 부재.
  34. 제3항 내지 제33항 중 어느 한 항에 있어서, 제1 및 제2 부분(12;14) 각각은 슬리브(814)에 의해 임플란트의 단부를 수용하기 용이한 개방 위치와, 단부 부재를 임플란트에 고정시키는 폐쇄 위치 사이에서 피벗 가능한 2개의 섹션(816;818)과, 단부 부재를 폐쇄 위치에서 고정시키는 로킹 기구를 포함하는 것을 특징으로 하는 단부 부재.
  35. 제34항에 있어서, 로킹 기구는 상호 협동하는 제1 톱니형 엣지(822)와 제2톱니형 엣지(824)를 포함하는 것을 특징으로 하는 단부 부재.
  36. 제1항 내지 제32항 중 어느 한 항에 있어서, 제2 부분(14;910)은 경사면을 갖춘 프롱(920)을 갖는 외측 링(914)과 외측 링(914)에 대하여 회전 가능하며 적어도 하나의 돌기부(918)를 포함하는 내측 링(912)을 포함하고, 상기 돌기부(918)는 회전 시에 프롱(920)의 경사면과 접촉함으로써 프롱(920)을 반경 방향 외측으로 가압하는 것을 특징으로 하는 단부 부재.
  37. A) 뼈 또는 특히 척추골의 단부판과 크기 및 형상이 일치하는 상부면(812)과, B) 상부면(812)으로부터 연장되며 임플란트의 단부를 수용하는 형태와 크기로 된 슬리브(814)를 포함하는 뼈를 융합하기 위한 임플란트와 사용되는 단부 부재에 있어서,
    C) 단부 부재는 슬리브(814)에 의해 임플란트의 단부를 수용하기 용이한 개방 위치와, 단부 부재를 임플란트에 고정시키기 위한 폐쇄 위치 사이에서 피벗 가능한 제1 및 제2 섹션(816;818)과, 단부 부재를 폐쇄 위치에서 고정시키는 로킹 기구를 추가로 포함하는 것을 특징으로 하는 단부 부재.
  38. 뼈를 융합하기 위한 임플란트와 사용되는 단부 부재에 있어서,
    A) 경사면을 갖춘 프롱(920)을 갖는 외측 링(914)과,
    B) 외측 링(914)에 대하여 회전 가능하며 적어도 하나의 돌기부(918)를 포함하는 내측 링(912)을 포함하고,
    상기 돌기부(918)는 회전 시에 프롱(920)의 경사면과 접촉함으로써 프롱(920)을 반경 방향 외측으로 가압하는 것을 특징으로 하는 단부 부재.
