Description

(式中、RはC(O)-(CH2)n-X、nは0、1、または2、Xは炭素数3~9を有する環式炭化水素であり、任意に1つ以上の不飽和結合を含む)。
(a)バルーンの表面に第一コーティングと第二コーティングとを有するバルーンカテーテルを提供する工程と、
(b)個人の頭蓋骨の硬膜下腔にアクセスする工程と、
(c)前記バルーンカテーテルを硬膜下腔に挿入する工程と、
(d)前記第二コーティングの組織への放出を可能にするため、前記バルーンを有効時間膨張させる工程とを有するものである。
本発明の大環状トリエン免疫抑制化合物は複数の実施形態を有するものであり、以下の表1の種のうち少なくとも1つを有するものとして記載できる。
GliaSite(登録)バルーンからの直接の薬物送達は、薬物送達ウェーハの使用を潜在的に排除することになるであろう。薬物は、従来のGliaSite(登録)装置に関連して使用される別のバルーンカテーテルから送達することも可能であろう。原理実証試験として、カルムスチンは、GliaSite(登録)バルーンのバルーンカテーテルシミュレーションから送達された。
穿頭孔手術とは、血餅を除去するために硬膜下腔にアクセスする場合に、あるいは排液のためにカテーテルを挿入する場合に、頭蓋骨に孔を開ける手技のことである。通常の孔のサイズは直径14mmであり、穿孔は特殊な電気ドリルまたはハンドドリルを用いて行われる。本発明は、従来の穿頭孔手術または直径がはるかに小さい頭蓋骨孔を用いた穿頭孔手術との関連で、薬物の局所送達が可能なバルーン技術を提供するものであるので、治療を必要とする明確な硬膜下部位へ正確な薬物量を容易に送達することが可能となる。手術完了後、小さな孔は、頭皮が閉じるに従ってそのまま治癒する。
Claims (8)
- バルーンカテーテルを有する治療薬の頭蓋内送達用医療装置であって、
前記バルーンは、血清蛋白質または血液蛋白質を含み、前記治療薬を含まない第一コーティングと、前記治療薬を含む第二コーティングとを有し、前記第二コーティングは前記第一コーティングの上に適用され、前記第一コーティングは単一層である、
医療装置。 - 前記第一コーティングは、ヒトのフィブリノーゲン、グロブリンまたはアルブミンを含む、請求項1に記載の医療装置。
- 前記治療薬は、カルムスチン、テモゾロミド、ロムスチン、プロカルバジン、ビンクリスチン、またはラパマイシン(シロリムス)、エベロリムス、ゾタロリムス、ビオリムス、ノボリムス、ミオリムス、テムシロリムス、およびそれらに関連した誘導体から成る群から選択される大環状トリエン免疫抑制化合物、または以下の構造を有するラパマイシン40エステル類似体
(式中、RはC(O)-(CH2)n-X、nは0、1、または2、Xは炭素数3~9を有する環式炭化水素であり、任意に1つ以上の不飽和結合を含む)である、請求項1または2に記載の医療装置。 - Rは、以下の構造のうち1つである、請求項3に記載の医療装置:
- 前記治療薬は、カルムスチン、テモゾロミド、プロカルバジン、およびビンクリスチンから選択される少なくとも1つである、請求項1または2に記載の医療装置。
- 前記第二コーティングは、さらに、リポ酸脂肪族アルコール、脂肪族アルデヒド、脂肪酸、およびそれらの組み合わせから成る群から選択される非ポリマー、非イオン性の直鎖炭化水素または界面活性剤を含む、請求項1~5のいずれか1項に記載の医療装置。
- 前記第二コーティングは、複数の層からなり、その最外層は、前記治療薬と、前記非ポリマー、非イオン性の直鎖炭化水素または界面活性剤とを含む、請求項6に記載の医療装置。
- 前記非ポリマー、非イオン性の直鎖炭化水素または界面活性剤は、式:CxHyO(式中、xは少なくとも18で、最も多くて35であり、yは少なくとも38で最も多くて72である)によって表される脂肪族アルコールである、請求項6または7に記載の医療装置。
