Description
(1) 肺疾患を治療する方法であって、
a.装置を、患者の隣接する肋骨経由、胸骨上、又は胸壁経由、から選択される位置でその長さの少なくとも一部分を通すことによって患者の胸腔に挿入する工程であって、前記装置はエンドエフェクタ及びそれに固定されるエネルギー源を有する、工程と、前記エンドエフェクタを気管支セグメント上にある神経成分の近位に、又は神経成分と接して位置合わせする工程と、前記エネルギー源からのエネルギーを、前記エンドエフェクタを介して前記神経成分に付与する工程と、を含む、方法。
(2) 前記エンドエフェクタは、中に気管支を収容するようにサイズ決めされた少なくとも1つの半円形を有する、実施態様1に記載の方法。
(3) 前記エンドエフェクタは、中央通路を形成するように位置合わせし、かつ中に気管支を収容するようにサイズ決めされた2つの半円形を有する、実施態様1又は2に記載の方法。
(4) 前記エンドエフェクタは、互いに対して移動可能な2つの要素を含み、前記2つの要素は、それらの間において気管支セグメントの外面に接するように構成されてもよい、実施態様1〜3のいずれかに記載の方法。
(5) 前記エンドエフェクタは、前記気管支セグメントの少なくとも一部分の周りに巻き付けられるように変形可能又は柔軟である、実施態様1に記載の方法。
(7) 前記エンドエフェクタは圧縮可能である、実施態様1〜6のいずれかに記載の方法。
(8) 前記装置を患者の体内に、トロカールを介して、又は送達ポートを介して挿入する工程を更に含む、実施態様1〜7のいずれかに記載の方法。
(9) 前記エンドエフェクタは、前記トロカール又は送達ポートを介して挿入される前に成形され、かつ前記トロカール又は送達ポートから出る際に拡張する、実施態様8に記載の方法。
(10) 前記エンドエフェクタは、前記カテーテル又は送達ポートを介して挿入される際に、縮小径に圧縮され、かつ前記カテーテル又は送達ポートから出る際に拡張する、実施態様8に記載の方法。
(12) 前記方法は、ビデオ補助胸腔鏡下手術手技を含む、実施態様1〜11のいずれかに記載の方法。
(13) 前記エンドエフェクタは、少なくとも2つの神経成分に接触する、実施態様1〜12のいずれかに記載の方法。
(14) 前記神経成分は、神経節又は叢を含む、実施態様1〜13のいずれかに記載の方法。
(15) 前記エンドエフェクタは、実質的に前記気管支セグメントを包囲する、実施態様1〜14のいずれかに記載の方法。
(17) 前記エネルギーは、前記エンドエフェクタに配設される複数の電極を介して付与される、実施態様1〜16のいずれかに記載の方法。
(18) 前記エネルギーの付与は、温度上昇をもたらし、かつ前記温度は約65℃又はそれ以上である、実施態様1〜17のいずれかに記載の方法。
(19) 前記神経成分への前記エネルギーの付与は、前記神経成分の切除をもたらす、実施態様1〜18のいずれかに記載の方法。
(20) 肺疾患を治療する装置であって、
a.近位端及び遠位端を有する延長体と、
b.前記遠位端のエンドエフェクタと、
c.前記エンドエフェクタにエネルギーを供給するエネルギー源と、を含み、
前記エンドエフェクタは、患者の胸腔に入り、気管支セグメントの少なくとも一部分上に位置する神経成分に接触し、前記神経成分を治療するために前記気管支セグメントの外面にエネルギーを付与するようにサイズ決めされ、かつ成形されている、装置。
(22) 前記エンドエフェクタは、中央通路を形成するように位置合わせし、かつ中に気管支セグメントを収容するようにサイズ決めされた2つの半円形を有する、実施態様20又は21に記載の装置。
(23) 前記エンドエフェクタは、互いに対して移動可能な2つの要素を含み、前記2つの要素は、それらの間において気管支セグメントの外面に接するように構成されてもよい、実施態様20〜22のいずれかに記載の装置。
(24) 前記エンドエフェクタは、前記気管支セグメントの少なくとも一部分の周りに巻き付けられるように変形可能又は柔軟である、実施態様23に記載の装置。
(25) 前記神経成分は、気管支セグメントの表面に、又は気管支セグメントの表面上に位置する神経節又は叢を含む、実施態様20〜24のいずれかに記載の装置。
(27) 前記エンドエフェクタは圧縮可能である、実施態様20〜26のいずれかに記載の装置。
(28) 前記装置は、カテーテル又は送達ポートを介して患者の体内に挿入されるように圧縮されてもよい、実施態様20〜27のいずれかに記載の装置。
(29) 前記エンドエフェクタは、前記カテーテル又は送達ポートを介して挿入される際に縮小径に圧縮され、かつ前記カテーテル又は送達ポートから出る際に拡張する、実施態様28に記載の装置。
(30) 前記エンドエフェクタは、前記気管支セグメントの外面を実質的に包囲する、実施態様20〜29のいずれかに記載の装置。
(32) 前記エネルギーは、前記エンドエフェクタの複数の電極を介して付与されてもよい、実施態様20〜31のいずれかに記載の装置。
(33) 前記エネルギーの付与は、温度上昇をもたらし、前記温度は約65℃又はそれ以上である、実施態様20〜32のいずれかに記載の装置。
(34) 前記エンドエフェクタは、ニチノールで構成されている、実施態様20に記載の装置。
Claims (13)
- 肺疾患を治療する装置であって、
a.近位端及び遠位端を有する延長体と、
b.前記遠位端のエンドエフェクタと、
c.前記エンドエフェクタにエネルギーを供給するエネルギー源と、を含み、
前記エンドエフェクタは、患者の胸腔に入り、気管支セグメントの少なくとも一部分上に位置する神経成分に接触し、前記神経成分を治療するために前記気管支セグメントの外面にエネルギーを付与するようにサイズ決めされ、かつ成形されており、
前記エンドエフェクタは、前記延長体の前記遠位端から遠位方向に延在する第1接触要素および前記延長体の前記遠位端から遠位方向に延在する第2接触要素を含み、前記第1接触要素は、前記第1接触要素の内面である第1接触面を含み、前記第2接触要素は、前記第2接触要素の内面である第2接触面を含み、前記第1接触面および前記第2接触面は、前記気管支セグメントの前記外面と接触し、前記エンドエフェクタは、前記延長体に対して前記延長体の長手方向軸の周りに回転することができる、装置。 - 前記第1接触要素および前記第2接触要素は、半円形である、請求項1に記載の装置。
- 前記第1接触面および前記第2接触面の少なくとも1つは、内部に流体が導入されている嚢状接触面である、請求項1または2に記載の装置。
- 前記神経成分は、前記気管支セグメントの表面に、又は前記気管支セグメントの前記表面上に位置する神経節又は叢を含む、請求項1〜3のいずれか一項に記載の装置。
- 前記エンドエフェクタは、前記気管支セグメントの表面に沿うように前記エンドエフェクタの表面上に材料を含む、請求項1〜4のいずれか一項に記載の装置。
- 前記エンドエフェクタは圧縮可能である、請求項1〜5のいずれか一項に記載の装置。
- 前記装置は、カテーテル又は送達ポートを介して患者の体内に挿入されるように圧縮される、請求項1〜6のいずれか一項に記載の装置。
- 前記エンドエフェクタは、前記カテーテル又は前記送達ポートを介して挿入される際に縮小径に圧縮され、かつ前記カテーテル又は前記送達ポートから出る際に拡張する、請求項7に記載の装置。
- 前記エンドエフェクタは、前記気管支セグメントの前記外面を実質的に包囲する、請求項1〜8のいずれか一項に記載の装置。
- 前記エンドエフェクタは、加熱素子又は電極、ガス又は液体などの加熱流体、低エネルギー超音波又は高強度集束超音波を含む超音波エネルギー、高調波エネルギー、直流又は焼灼にさらすこと、電磁エネルギー、高周波エネルギー、マイクロ波、プラズマエネルギー、赤外線、パルスレーザ、フラクショナルレーザ、又は高エネルギーレーザ露出を含むレーザ治療などの非電離光エネルギー、アルファ、ベータ、ガンマ、X線、プロトン、ニュートロン、又はイオン放射線を含む他の放射エネルギー、の形態でエネルギーを付与する、請求項1〜9のいずれか一項に記載の装置。
- 前記エネルギーは、前記エンドエフェクタの複数の電極を介して付与される、請求項1〜10のいずれか一項に記載の装置。
- 前記エネルギーの付与は、温度上昇をもたらし、前記温度は約65℃又はそれ以上である、請求項1〜11のいずれか一項に記載の装置。
- 前記エンドエフェクタは、ニチノールで構成されている、請求項1に記載の装置。
