本願は、2015年8月14日に出願され、「屈折性眼内埋植システムのための装置及び方法(Apparatus and Mehtods for Refractive Intraocular Implant System)」と題された米国仮出願連続番号第62/205,226号の、米国特許法第119条(e)の定めによる優先権の利益を主張する。当該仮出願の内容は、その全体が引用されることにより、依拠されてここに組み込まれる。
この開示の分野は一般に、眼内取り付けシステムのための装置及び方法に関し、より特定的には、眼内埋植物を虹彩に添着するための埋植物設計、外科用方法、ツール、及び係止具に関する。
患者及び外科医は、長い間、眼の異常を補償する眼鏡の代替物に関心を寄せてきた。これらの代替物には、例えば、コンタクトレンズ、放射状角膜切開術、LASIK即ちレーザ視力矯正手術などが含まれる。しかしながら、このような代替物には、それぞれの欠点及び不備がない訳ではない。例えば、LASIK手術は、矯正力に限界があり、患者の角膜を脆弱にする恐れがあり、他の合併症(例えば、視力変動、ハロー、グレア、ドライアイなど)を誘発する恐れがある。
ここに開示される実施形態は、眼内レンズ(IOL)、並びに、関連するアセンブリ、及び、埋植と眼内取り付けとの方法を含む。いくつかの態様において、IOLを瞳孔に対して相対的に虹彩に添着するために、螺旋形状のコイル係止具、並びに、関連付けられるアプリケータツール及び外科用挿入方法が提供される。1つの実施形態において、IOLアセンブリは、乱視、老眼、及び/又は、近視若しくは遠視を矯正するために、IOLを虹彩に添着する螺旋形状のコイル係止具を含む。IOLは、視力の異常を矯正するために、患者の本来の水晶体が除去されるか又は定位置に残された状態で、埋植することが可能である。IOLは、(例えば、水晶体と共に使用するために)有水晶体埋植物として、又は、(例えば、水晶体なしで使用するために)無水晶体埋植物として、使用され得る。IOLは、乱視及び/又は近視若しくは遠視を矯正する光学的効果(例えば、第1の光学的効果)を有する光学部品を有する。IOLは、(例えば、2.0ジオプトリまでにおける、水晶体と組み合わさって作用する有水晶体IOLとして)老眼に対処するか、又は、(例えば、4.0ジオプトリまでの、)無水晶体IOLとして老眼に対処する、別の光学的効果(例えば、第2の光学的効果)も有し得る。光学部品は、その周縁から延在し、且つ、瞳孔に対して相対的に虹彩へのIOLの添着を容易にするように構成される、1つ以上のハプティックも有する。ハプティックは、角膜から適切な距離を維持しながら、虹彩問題接触を最小にするようにドーム状に湾曲させることができる。この点に関し、光学部品に係合してIOLを虹彩に添着するように構成される、螺旋形状のコイル係止具が提供される。螺旋形状のコイル係止具は、頭部と、当該頭部の底面から延在し、且つ、当該頭部の反対側の尖端を含む螺旋ワイヤとを含む。IOLが瞳孔に対して相対的に一旦位置付けられると、螺旋形状のコイル係止具は、各ハプティックの遠端に適用されて、虹彩の前面に斜角で貫入するように構成され、それにより、瞳孔に対して相対的にIOLを添着する。螺旋形状のコイル係止具は、虹彩への回転可能な貫入によって虹彩に添着されるように構成され、よって、螺旋形状のコイル係止具は、逆回転させることにより虹彩から取り外すことができる。螺旋形状のコイル係止具は、体積が小さく、表面積が大きく、貫入の断面積が小さく、斜角で貫入する。よって、螺旋形状のコイル係止具は、適用が容易であり、取り外しが容易であり、組織損傷を最小にし、安定性を最大にし、貫入力を最小にする。
Claims (22)
- 眼内に挿入及び添着されるように構成される眼内レンズアセンブリであって、
予め選択された光学的効果を生じるための光学部品を含む眼内レンズであって、前記光学部品が外周縁を有する、眼内レンズと、
前記光学部品の前記外周縁から延在するハプティックであって、前記ハプティックが、近端、遠端、及び、それらの間の立ち上がり区域を含み、前記光学部品の前記外周縁における前記近端が、前記遠端とは異なる高さに存在する、ハプティックと、
尖端を有する端部を有するワイヤを有する螺旋形状のコイル係止具であって、前記螺旋形状のコイル係止具が、前記ハプティックの前記遠端を通じた挿入と、虹彩の前側への回転可能な貫入とにより、前記ハプティックの一部を前記螺旋形状のコイル係止具の上部と前記虹彩の前記前側との間で加圧することにより、前記ハプティックを前記眼に添着するように構成される、螺旋形状のコイル係止具と、
を備える、眼内レンズアセンブリ。 - 前記ハプティックは、前記眼内レンズの一部である、請求項1の眼内レンズアセンブリ。
- 前記眼内レンズは、長さにおいて4mmよりも小さな切開を通り抜けるように構成される、請求項1の眼内レンズアセンブリ。
- 前記虹彩に取り付き、且つ、前記虹彩の括約筋及び散大筋への干渉を回避するように構成される、請求項1の眼内レンズアセンブリ。
- 前記螺旋形状のコイル係止具は、色素上皮に貫入しないように、ある深さまで前記虹彩に貫入するように構成される、請求項1の眼内レンズアセンブリ。
- 前記予め選択された光学的効果は、近視、遠視、乱視、又はそれらの組み合わせを矯正するように構成される、請求項1の眼内レンズアセンブリ。
- 前記予め選択された光学的効果は、近視、遠視、乱視、又はそれらの組み合わせのうち少なくとも1つを矯正するように構成される第1の光学的効果を含み、老眼を矯正するように構成される第2の光学的効果を採り入れる、請求項1の眼内レンズアセンブリ。
- 前記光学部品は、前記眼の本来の水晶体と共に作用するように構成される、請求項7の眼内レンズアセンブリ。
- 前記光学部品は、前記眼の本来の水晶体なしで作用するように構成される、請求項7の眼内レンズアセンブリ。
- 前記光学部品は、屈折性特徴、回折性特徴、多焦点特徴、二焦点特徴、焦点域拡大又は視域拡大の特徴、及び非球面化特徴から成る群から構成される光学的特徴を含む、請求項1の眼内レンズアセンブリ。
- 前記ハプティックは、複数のハプティックであり、
前記螺旋形状のコイル係止具は、複数の螺旋形状のコイル係止具であり、
前記複数のハプティックの各々は、前記複数の螺旋形状のコイル係止具のうちの1つにより前記虹彩に添着される、請求項1の眼内レンズアセンブリ。 - 前記遠端は、前記螺旋形状のコイル係止具をその中に受けるように構成される空隙を含む、請求項1の眼内レンズアセンブリ。
- 前記遠端は、底壁を含み、前記螺旋形状のコイル係止具は、前記底壁を通って挿入されて前記ハプティックを前記眼に添着するように構成される、請求項1の眼内レンズアセンブリ。
- 前記ハプティックの前記近端は、前記光学部品の前記外周縁に接続される複数の近端を含む、請求項1の眼内レンズアセンブリ。
- 前記ハプティックは、前記光学部品に一体的に接続される、請求項1の眼内レンズアセンブリ。
- 前記ワイヤは、中央円筒開口を規定する螺旋ピッチを有する螺旋を形成する、請求項1の眼内レンズアセンブリ。
- 前記螺旋形状のコイル係止具は、第1の方向に回転されると前記虹彩の前記前側へ回転可能に貫入し、前記第1の方向とは反対の第2の方向に回転されると前記虹彩の前記前側から回転可能に取り外されるように構成される、請求項1の眼内レンズアセンブリ。
- 前記螺旋形状のコイル係止具の前記上部は頭部を有し、頭部幅は、前記ワイヤの螺旋直径よりも広い、請求項1の眼内レンズアセンブリ。
- 前記螺旋形状のコイル係止具の前記上部は頭部を有し、頭部幅は、前記ワイヤの螺旋直径と略同じサイズである、請求項1の眼内レンズアセンブリ。
- 前記眼内レンズアセンブリは、前記螺旋形状のコイル係止具が前記虹彩内へ充分に貫入して前記ハプティックを前記眼に添着すると、前記螺旋形状のコイル係止具が前記ハプティックの上面に対して相対的に窪むように構成される、請求項1の眼内レンズアセンブリ。
- 前記ハプティックの前記遠端内に位置付けられるグロメットをさらに備え、前記グロメットは、その中に前記螺旋形状のコイル係止具を受け、前記螺旋形状のコイル係止具が前記グロメットに対して相対的に内部並進することを可能にするように構成される、請求項1の眼内レンズアセンブリ。
