Description
心周期において心房へペーシングパルスが送られる(101)。システムは、心周期において内因性の心室活動を検知する(102)。内因性の活動が検出されない場合(103)、ペーシングパルスが心室へ送られる(104)。
システムは、心房ペーシングパルスが心房を捕捉したかを判断する(164)。システムが、心房ペーシングパルスによって心房が捕捉されなかったと判断した場合、心房の逆行管理が提供される(165)。
心房の組織が不応である間は、逆行性P波は次に予定されている心房ペースが送られるようにする。心房の組織が、予定されている心房ペースに反応不可能であるので、予定されている心房のペースは有効でなくなる。心房の有効不応期(AERP)の終了までに次に予定されている心房ペースの供給を遅延させることによって心房の組織が不応である間、システムは心房ペーシングを送らないようにする。
この静脈は、心臓の右側から直接アクセスできない左心室の表面に達するようにリード用のアクセス経路として使用される。左心室リード206のためのリード配置は、鎖骨下静脈アクセスと、左心室に隣接するLV電極213、217を挿入するための予め形成された案内カテーテルと、を介して達成され得る。
分類の中心点1220は、Mi-1で示されている。分類期間の上限範囲1222は、Mi-1+Ri-1で示され、下限範囲1221は、Mi-1−Ri-1で示される。図6Cは、i番目の拍動と称される、次のペーシングされた拍動の後の心臓応答分類期間を示す。i番目の拍動のピーク振幅時間(Ti)が決定される。Tiが[Mi-1−Ri-1,Mi-1+Ri-1] の範囲内にある場合、i番目の拍動が融合ではなく、捕捉拍動であると考えられ、Tiは分類期間を適合するために使用される。そうでない場合、i番目の拍動は融合と考えられ、この範囲はその拍動からの情報を用いて更新されない。
他の実施において、分類期間は、所定の安定基準が達成されるまで調整され得る。
例えば、図16に示されるように、非捕捉応答テンプレートは、本発明の方法を使用して形成され得る。図16に示されるように、心臓は、内因率より高い率で、捕捉閾値より低いエネルギレベルでペーシングされ得る(2310)。拍動ごとにペーシング率をわずかにあるいはランダムに変えることによってペーシング率のジッタリング(jittering)が融合を回避するために付加的にあるいは代替的に使用される。
Claims (10)
- 医療システムであって、
心臓チャンバへペーシングパルスを送るように構成されているペーシング回路と、
前記ペーシングパルスに関連する心臓信号を検知するように構成されている検知回路と、
前記検知回路に結合されているプロセッサであって、前記ペーシングパルスに対する心臓応答の種類を分類および判断するために心臓信号が検知される前記ペーシングパルス後の期間として定義される心臓応答分類期間を決定するように構成され、少なくとも一つの心臓信号特徴のタイミングに対して前記心臓応答分類期間を一時的に調整し、心臓応答の種類に関連している前記少なくとも一つの心臓信号特徴のタイミング変動性に基づいて、前記心臓応答分類期間の長さを決定するように構成されているプロセッサと、
前記心臓応答分類期間を用いてペーシングに対する心臓応答の種類を判断するように構成されている心臓応答分類回路であって、前記ペーシング回路が、少なくとも部分的に前記心臓応答の種類の判断に基づいて、ペーシング治療を施すように構成されている心臓応答分類回路と、
を含む医療システム。 - 前記心臓信号特徴がピーク振幅を含む、請求項1に記載のシステム。
- 前記心臓応答の種類が捕捉応答を含む、請求項1に記載のシステム。
- 前記プロセッサが前記心臓信号特徴のタイミングの標準偏差を決定するように構成されている、請求項1に記載のシステム。
- 前記プロセッサが、前記心臓信号の各々に対する前記心臓信号特徴のタイミングに対して値を決定し、前記値の平均を取り、前記値の標準偏差を決定し、前記値の平均および標準偏差に基づいて、前記心臓応答分類期間を形成するように構成されている、請求項1に記載のシステム。
- 前記プロセッサは、前記心臓応答分類期間が安定するまで、前記心臓応答分類期間を調整するように更に構成されている、請求項1に記載のシステム。
- ペーシングパルスに対する心臓応答を判断するために前記心臓応答分類期間を使用するように構成されている心臓応答分類プロセッサを更に含む、請求項1に記載のシステム。
- ペーシングパルスに対する心臓応答の種類を分類および判断するためにペーシングパルスの後の心臓信号を検知するために使用される心臓応答分類期間を形成するためのシステムであって、
心臓応答の種類に関連する心臓の信号を検知する手段と、
心臓の信号の一つ以上の心臓応答特徴のタイミングの変動性を判断する手段と、
前記一つ以上の心臓応答特徴の前記タイミングに対して前記心臓応答分類期間を一時的に調節し、前記タイミングの変動性を用いて前記心臓応答分類期間の長さを設定することを含む、前記心臓応答分類期間を形成する手段と、
前記心臓応答分類期間を用いて、心臓応答を判断する手段と、
少なくとも部分的に前記心臓応答の種類の判断に基づいて、ペーシング治療を施すように構成されているペーシング回路と、
を含む分類期間形成システム。 - 前記心臓応答分類期間を形成する手段が、ペーシング治療を施している間に検知された一つ以上の更なる心臓の信号を用いて、前記心臓応答分類期間を変更するように構成されている、請求項8に記載のシステム。
- 前記心臓応答を判断する前記手段が、前記心臓応答分類期間を用いて捕捉を検出するように構成されている、請求項8に記載のシステム。
