Description
(A)心臓内チャンバを電気的に隔離する方法であって、光学組立体を肺静脈のところで前記肺静脈の口の近くに導入する段階と、アンカを用いて前記光学組立体を前記肺静脈のところに繋留する段階と、しかる後、レーザ光エネルギを前記光学組立体から前記肺静脈の周方向焼灼領域まで延びる経路で伝える段階とを有することを特徴とする方法。
(1)前記経路に前記アンカが導入されることはないことを特徴とする実施態様(A)記載の方法。
(2)前記レーザ光エネルギを伝える前記段階は、前記レーザ光エネルギを前記焼灼領域と交差する周方向線内へ差し向けることにより行われることを特徴とする実施態様(A)記載の方法。
(3)前記アンカは、バルーンから成り、繋留は、前記バルーンを拡張させて前記肺静脈に係合させることにより行われることを特徴とする実施態様(A)記載の方法。
(4)前記導入段階は、心臓の卵円窩を介して行われ、前記方法は、予備レーザ光エネルギを前記卵円窩に当ててその組織を焼灼し、それにより前記光学組立体がこれを通り易くする段階を更に有していることを特徴とする実施態様(A)記載の方法。
(5)前記レーザ光エネルギを伝える前記段階は、厳密に1回の適用で行われることを特徴とする実施態様(A)記載の方法。
(7)前記パルスの各々の持続時間は、100ミリ秒未満であることを特徴とする実施態様(6)記載の方法。
(8)前記導入段階は、前記光学組立体を血管内カテーテルに取り付け、前記血管内カテーテルの遠位部分を血管を通して心臓内へ至らせることにより行われることを特徴とする実施態様(A)記載の方法。
(9)前記レーザ光エネルギを伝える前記段階は、前記レーザ光エネルギを反射させる段階を更に含むことを特徴とする実施態様(A)記載の方法。
(10)前記レーザ光エネルギを反射させる段階は、鏡を前記アンカの外部であって前記レーザ光エネルギの経路内に配置する段階を含むことを特徴とする実施態様(9)記載の方法。
(12)前記レーザ光エネルギの波長は、約13ミクロンであることを特徴とする実施態様(A)記載の方法。
(B)心臓内チャンバを電気的に隔離する器械であって、肺静脈内へその口の近くで導入されるようになった血管内カテーテルと、前記カテーテルの遠位端部のところに設けられていて、前記肺静脈のところで前記カテーテルの遠位端部を固定するためのアンカと、レーザ光エネルギを前記肺静脈の周方向焼灼領域まで延びる経路で伝える光学組立体とを有することを特徴とする器械。
(13)前記光学組立体は、実質的に前記アンカと非接触関係にあることを特徴とする実施態様(B)記載の器械。
(14)前記経路に前記アンカが導入されることはないことを特徴とする実施態様(B)記載の器械。
(15)前記光学組立体は、前記レーザ光エネルギを光源から伝える光ファイバと、前記光ファイバの出口側フェースのところに設けられたレンズと、前記アンカの外部で前記経路内に設けられていて、前記レーザ光エネルギを前記焼灼領域と交差する周方向線内へ差し向けるレフレクタとから成ることを特徴とする実施態様(B)記載の器械。
(17)前記レフレクタは、放物面鏡であることを特徴とする実施態様(15)記載の器械。
(18)前記レフレクタは、前記アンカの光反射外面を構成していることを特徴とする実施態様(15)記載の器械。
(19)前記アンカは、膨らむと前記肺静脈に係合するようになったバルーンから成ることを特徴とする実施態様(B)記載の器械。
(20)前記バルーンは、2裂片(bilobate)状であることを特徴とする実施態様(19)記載の器械。
(22)前記レーザ光エネルギは、厳密に1回の適用で前記焼灼領域に適用されることを特徴とする実施態様(B)記載の器械。
(23)前記レーザ光エネルギは、一連のパルスの状態で前記焼灼領域に適用されることを特徴とする実施態様(B)記載の器械。

Claims (11)
- 心臓内チャンバを電気的に隔離する器械において、
レーザ光源と、
肺静脈内にその肺静脈の口の近くへ導入される血管内カテーテルと、
前記カテーテルの遠位端部に設けられていて、前記肺静脈で前記カテーテルの遠位端部を固定するためのアンカと、
前記レーザ光源からのレーザ光エネルギを、前記肺静脈の周方向焼灼領域まで延びる経路で伝える光学組立体であって、前記アンカの内面と非接触関係にあり、レーザ光エネルギのための前記経路が前記アンカに導入されることはない、光学組立体と、
を具備し、
前記光学組立体は、
前記レーザ光エネルギを前記レーザ光源から伝える光ファイバと、
前記光ファイバの遠位端部に設けられたレンズと、
前記レンズと前記アンカとの間の前記経路内に設けられていて、前記アンカの外面にまたは前記アンカの外面の外側にあって前記レーザ光エネルギを周方向線内へ差し向けるためのレフレクタと、
を有し、
前記経路は、前記レンズと前記焼灼領域との間における前記アンカの任意の表面を通過しない、
器械。 - 請求項1記載の器械において、
前記レンズは勾配屈折率レンズである、器械。 - 請求項1記載の器械において、
前記レフレクタは放物面鏡である、器械。 - 請求項1記載の器械において、
前記レフレクタは、前記アンカの光反射外面を構成している、器械。 - 請求項1記載の器械において、
前記アンカは、膨らむと前記肺静脈に係合するように構成されたバルーンを含む、器械。 - 請求項5記載の器械において、
前記バルーンは、近位部分と遠位部分とを備えている、器械。 - 請求項5記載の器械において、
膨らんだ状態の前記バルーンの近位部分は、膨らんだ状態の前記バルーンの遠位部分よりも拡張した状態にある、器械。 - 請求項1記載の器械において、
前記レーザ光エネルギの連続する照射を1回だけ行う、器械。 - 請求項1記載の器械において、
前記レーザ光エネルギは、一連のパルスの状態で前記焼灼領域に適用される、器械。 - 請求項9記載の器械において、
