発明の技術分野
本発明は、股関節全体の置換処置に使用される寛骨臼カップ・プロテーゼに関し、特に、機能を改良した寛骨臼カップおよび関連部材に関する。
従来技術の説明
人間の股関節は、ボールとソケットのジョイントのように機械的な働きをし、大腿骨の球状の頭部が骨盤のソケット型の寛骨臼内に位置されている。股関節全体の置換では、大腿骨の頭部および寛骨臼の表面は、プロテーゼ装置と置換される。股関節全体の置換処置における重大な問題の1つは、移植時およびプロテーゼの全寿命を通しての両方でプロテーゼ装置と患者の骨との間の強力な接着をどの様にして獲得するかである。プロテーゼ装置を患者の骨に固定することの問題は、特に寛骨臼カップ・プロテーゼに関してである。多くの通常の寛骨臼カップは、締まり嵌め、機械的な接合装置および、または骨のセメントのような接着材料のいずれかによって準備された寛骨臼内に固定される半球状のカップである。
寛骨臼内への寛骨臼カップ・プロテーゼの接着に骨のセメントを使用することは、非常に迅速な接着を行えるが、時間が経つと現れる様々な欠点を有する。荷重の応力は、プロテーゼの寿命にわたって寛骨臼カップに繰り返し印加される。骨のセメントが寛骨臼にカップを固定するために使用される場合、骨のセメントは繰り返し荷重によって疲労し破壊する可能性がある。幾つかの例において、骨のセメントの完全性の劣化により、カップはゆるみ、置換を必要とすることになり得る。さらに、患者の寛骨臼内に骨のセメントを適用することにおいて、通常、骨のセメントを固定する点に供給するために固定穴が周りの骨に開けられる。骨のセメントが破壊し、カップが置換されることを要する場合、古い骨のセメントは骨における固定穴から除去されなければならない。このような工程は困難であり、時間がかかり、寛骨臼の周りの健康な骨の組織に危害を加える可能性がある。さらに、通常の骨のセメントは、患者の寛骨臼に入れられてしまった後で硬化される。セメントが寛骨臼内に入れられ硬化される時に化学的発散物が骨のセメントから出る。このような発散物は、何人かの患者に有害な反応を生じさせる可能性があり、股関節置換処置を受ける患者の被る危険を増す。
セメント固定技術の欠点を認識して、即時の安定のために寛骨臼にカップを接合する機械的固定手段、および長期安定な固定のために生物学的に骨と接着される様々な表面処理を使用する従来技術の寛骨臼カップが開発されてきた。寛骨臼カップを機械的に固定する簡単な技術は、捩子即ちネジまたはその他の機械的留め具によって寛骨臼内にカップを付着することである。しかしながら、寛骨臼の周りの骨の性質、および動脈の位置のようなその他の制限因子により、捩子はある限られた領域にのみ適用されることができる。さらに、捩子はカップの補足的な固定および安定化を与えることができるが、カップの形状が準備された寛骨臼の形状に完全に適合していないカップをしっかりと固定するために使用されることができる。
寛骨臼カップを機械的に固定する別の方法は、カップの外側に配置された捩子の使用によるものである。このような実施例においては、カップは、寛骨臼の骨に捩じ込まれ、捩子の歯が骨に食い込み、係合する。寛骨臼カップを患者に移植するこのような方法は、米国特許第4,662,891号明細書、“FIXATION ELEMENTS FOR ARTIFICIAL JOINTS”に例示されている。従来技術の捩子山の付いた外側表面を有する多くのカップは、先の鋭い比較的大きな捩子山を使用している。このような従来技術のカップの欠点は、カップの外側表面と骨との間に生物学的な繋がりがないので、カップに印加される荷重の応力は大部分が捩子に伝達されることである。通常、捩子の外周は鋭くなっているので、大きな応力の集中点が捩子の周辺部における骨に生じる。このような荷重の応力は、骨によって耐えられる応力量を越え、骨の中に有害な反応を生じる可能性があり、幾つかの例においては、カップの移動、ゆるみ、痛み、およびまたは接合の転位による機能不全を生じる。
寛骨臼カップを移植するさらに別の方法は、当初の安定化の手段として締まり嵌めの使用を含む。締まり嵌めを使って移植されるある従来技術の装置は、半球状の外側表面を有するカップを使用する。寛骨臼は所定の大きさまで球状に抉られ、寸法の大きい目の球状のカップが締まり嵌めを提供するために強制的に挿入される。寛骨臼の球状の抉りは、単純さおよび患者から患者へさらに正確に再現される能力のため、他の形状より好まれる。図1aおよび1bを参照すると、従来技術の半球状の寛骨臼カップ110が、準備された寛骨臼112と共に示されている。半球状の寛骨臼カップ110は、既知の曲率半径を有する。寛骨臼112は、カップ110の曲率半径より僅かに小さい曲率半径まで球状に抉られる。したがって、カップ110は、締まり嵌めによって寛骨臼112に移植されることができる。図1bにおいて、カップ110が寛骨臼112に押し込まれるとき、寛骨臼112は変形されることが分かる。変形された寛骨臼表面によってカップ110の周辺縁部に印加される力のみが、摩擦によって寛骨臼112内にカップ110を保持するように作用する水平方向の保持分力を有し、締まり嵌めを提供する。図1bに見られるように、寛骨臼の変形は、カップの表面領域と寛骨臼の表面領域との間にギャップ113を生ずる。ギャップ113によって取り囲まれているカップ110のこの部分には力は印加されない。しかしながら、カップ110はこの時は最頂端部でギャップの下方の寛骨臼の表面にもう1度係合し、そこではそれによって、カップに伝えられる垂直方向の反作用力は、カップが完全に着座されることを妨げ得る、押し戻す効果を生じようとする。
締まり嵌め移植によって生じるカップと寛骨臼との間の締まりの領域を制御する、従来技術の別の寛骨臼カップが開発されている。図2aおよび2bを参照すると、2つの曲率半径を有する外側表面を有するカップ114が示されている。カップ114の縁部に近接する領域115は、カップ114の頂端部116のものより大きな曲率半径を有する。通常、カップ114は、カップ114の頂端部116にほぼ等しい曲率半径まで抉られている寛骨臼117に挿入される。カップ114が寛骨臼117内に嵌め込まれる時、カップ114の縁部に近接する領域115は寛骨臼117を変位させ、締まり嵌めを生ずる。カップ114による寛骨臼117の変位により、寛骨臼117は最初の球状に抉られた形状から変形される。したがって、抉られた寛骨臼の底部は、カップ114の頂端部116によって接触される時にはもはや球状ではない。図2bに見られるように、カップ114の球状に湾曲した頂端部116は寛骨臼117の非球状の底部に完全には適合しない。したがって、カップと骨との間の境界面に沿って隙間118が存在し得る。記載されたような2つの半径を持つ従来技術のカップは、Figgie氏らによる米国特許第4,892,549号明細書、“DUAL-RADIUS ACETABULAR CUP COMPONENT”に例示されている。
図3aおよび3bにおいては、寛骨臼カップ120が球状に湾曲した頂端部121および円筒形の縁部122を有する、別の従来技術の実施例が示されている。縁部122と頂端部121との間の形状における相違は明白であり、段のある外側表面を有するカップ120を提供する。カップ120を受容するため、寛骨臼124は、寛骨臼124がカップ120の段のある外側表面に適切に適応できるように、2つの大きさおよび形状の異なるリーマによって抉られなければならない。必要とされる寛骨臼124の段のある2つの抉り作業は、カップ120を移植するときに要求される複雑さおよび労力を増す。カップ120が抉られた寛骨臼124内に嵌め合わされ時、カップ120の縁部122は、カップ縁部122と寛骨臼124との間に締まり嵌めを生じるように寛骨臼124を転位する。寛骨臼124は、本来、球状になるように抉られる。しかしながら、カップ120の縁部122によって生じる寛骨臼124の転位により、寛骨臼124は最初の球状の形から変形される。したがって、カップ120の球状にされた頂端部121は、寛骨臼124ときっちりとは当らない。隙間125のようなものが、カップ120の頂端部121を横切る様々な位置にカップと骨との境界面に沿って生ずる。上記されたようにカップ形状を持つ従来技術のカップは、Averill氏らによる米国特許第4,704,127号明細書、“DUAL-GEOMETRY ACETABULAR CUP COMPONENT AND METHOD OF IMPLANT”に例示されている。
さらに、ある従来技術の寛骨臼カップが2つの部分の装置として使用者に提供されており、それは内部ベアリング挿入物が医者によって外部シェルに嵌め合わされることも述べられている。2つの部分の装置の使用は、1連の大きさの外部シェルを提供し、続く組立てのために大きさの異なる大腿部の頭部に適応する内部ベアリング挿入物のアレイを提供することを可能にする。さらに、2つの部分の構成は、大腿部の頭部が、シェルの材料に対するものよりも大腿部の頭部への少ない摩擦を提供する材料と接触することを可能にする。このような2つの部分のカップ・プロテーゼは、Goymann氏らによる米国特許第4,795,470号、“TWO-PART COCKET FOR HIP-JOINT PROSTHESIS”によって例示されている。
骨盤置換を受けた患者が手足の運動を実行する時、大腿部のプロテーゼの頭部が変化する方向にカップに力を伝達することと、大腿部のプロテーゼの頸部ネックが移植されたカップの縁部に時折接触し得ることとは分かっている。このような変化する力およびまたは接触の結果として、寛骨臼に相関してカップを移動しようとする力が様々な具合に生じる。上記された2つの部分のカップにおいて、これらと同様の複合力が内部ベアリングに作用し、外部シェルに相関して内部ベアリング挿入物を揺動し、回転し、移動しようとする。十分には固定されていないベアリング挿入物は、ベアリング挿入物と外部シェルとの間で微細運動を生じる。このような微細運動により、ベアリング挿入物の磨滅が生じ、挿入物とシェルとの境界面から完全さがさらに減ずる。シェルからの挿入物の分離を防ぐため、外部シェル内にベアリング挿入物を保持する様々な方法が開発されている。例えば、ある従来技術の装置は、ベアリング挿入物の外側表面に形成される表面をざらざらにされた突出物を有する。表面をざらざらにされた突出物は、外部シェルに相関的なベアリング挿入物の全体的な頂端の整合を確実にするために、外部シェルを通って形成される穴内に嵌め合わされるが、同様の別の商業化された保持方法は、シェルと挿入物との間の微細運動を防がない。このような従来技術のプロテーゼ装置は、Rhenter氏らによる米国特許第4,878,916号明細書、“PROSTHETIC CUP”に例示されている。
シェル部品内にベアリング挿入物を保持する別の一般的な従来技術の方法は、Forte氏による米国特許第5,092,897号明細書、“IMPLANTABLE ACETABULAR PROSTHETIC HIP JOINT WITH UNIVERSAL ADJUSTABILITY”に例示されたように、捩子または他の機械的留め具を使用することによるものである。この構造は寛骨臼内の寛骨臼カップの大きな運動に対する安全性を提供するが、通常、捩子頭とシェルとの境界面、または挿入物とシェルとの境界面に生じる微細運動は防がない。
シェル部品内にベアリング挿入物を保持する別の従来技術の方法は、スナップ嵌め合せ構造を使用することによるものである。このような機構は、シェルから挿入物の分離に対する可能性を様々な程度に対して扱っているが、微細運動および結果的に生ずる摩滅破片の有害な影響は扱ってはいない。
2つの部分の寛骨臼カップは、挿入物の大きさの選択および外部シェル挿入の便宜のための外科手術の時に組立てられることを考えている。外科手術中のこの手による組立は、製造者が部品の長期間の互換性を保証する必要があり、この互換性は全ての組立でなくとも大抵の組立の際に2つの部品間に隙間を生ずる寸法的な公差を導く。これらの設計された隙間は、股関節によって予測可能に経験される複合的なおよび周期的な荷重に晒されるときの挿入物とシェルとの間に存在する免れがたい微細運動の可能性を生ずる。さらに、部品間のこれらの隙間は、体液で満たされ、マイクロ運動から生ずる磨滅破片と混合する空間を構成する。2つの部品にわたって伝達される荷重は周期的であり、方向が様々であるので、ピストンの動きのように空間が開閉して、体液および磨滅破片の混合物が現れる。穴が捩子穴または他の穴の形状でシェルに存在する場合、破片の混じった流体は、シェルと、シェルと骨との間の固定が目論まれている寛骨臼との間の境界面に現れ得る。骨への磨滅破片の射出は、磨滅破片が、カップの緩み、および耐久性のある再構成の可能性が減少する再手術の必要性をもたらし得る骨のリーシスを生ずると考えられるという事実によって好ましくないことは明瞭である。
上述には、締まり嵌めによって移植されることができ、カップの嵌め合いおよび安定性に関する上述された従来の問題の多くを避けることに効果的である改良された寛骨臼カップが記載されている。微細運動とシェル部品とベアリング挿入物との間の隙間の両方をほぼなくす方法で予め組立られている改良されたシェル部品およびベアリング挿入物がさらに記載されている。
発明の概要
本発明は、内部ベアリング挿入物を受容する空洞を含む外部シェル部品から構成される寛骨臼カップ・プロテーゼ、および患者の寛骨臼内に寛骨臼カップを移植する方法である。
寛骨臼カップのシェル部品は、患者の寛骨臼内に移植される外側表面を有する。外部表面は複数の部分から成り、全ての表面の部分は1つ以上の楕円体の湾曲に従っている。患者の寛骨臼内にシェル部品を移植するため、寛骨臼は、シェル部品の外側表面によって取り囲まれる容積より僅かに小さい容積を画定するように球状に抉られる。シェル部品が球状に抉られた寛骨臼に挿入されるとき、寛骨臼はほぼ1つの楕円体と同じ形に変形する。したがって、シェル部品が患者の寛骨臼に挿入されるとき、寛骨臼の形状は、寛骨臼の壁がその形状に沿って全ての外側表面にほぼ接触し同時に締まる力が周囲の縁部部分にのみ生じるように、シェル部品の外形に適応するように変形される。
環状の固定リブおよび固定溝は、縁部に隣接した領域におけるシェル部品の外側表面上に形成される。固定リブは、シェル部品が患者に移植されるときに、寛骨臼を形成し、取り囲む骨と入れ代る。固定リブは、骨と機械的に係合して寛骨臼にシェル部品を接合する。固定溝は、通常上方に収斂している側壁を有し、従ってダブテール型即ちほぞ穴型の断面プロフィルを有する。シェル部品が寛骨臼に移植されると、骨は固定溝中へ成長する。固定溝のダブテール型により、中に成長した骨およびシェル部品が寸法的に互いに嵌まり込むようになる。シェル部品が寛骨臼に接合し、締まり嵌めおよび固定リブの存在によって生じられる機械的な力は、寛骨臼内の設定位置にシェル部品を保持し、プロテーゼのシェル部品を転置するために即時および長期で作用する様々な力に耐える。長期にわたって、固定リブ間に配置される溝は、骨がその間に成長することを可能にすることによって寛骨臼に相関的なシェル部品の転移を阻止する。
ベアリング挿入物は、締まり嵌めによってカップのシェル部品の受容空洞内に保持される。ベアリング挿入物の形状は、シェル部品内に形成される空洞の形状に対応する。締まり嵌めの生成は、当接しているシェル部品とベアリング挿入物の様々な表面を横切る、シェル部品とベアリング挿入物との間の隙間をほぼ除去する。ベアリング挿入物およびシェル部品は、ベアリング挿入物とシェル部品との間にくさび作用の状態を生ずる向い合って傾斜した表面を含む。くさび作用の状態は、シェル部品内にベアリング挿入物を着座させるように作用し、ベアリング挿入物のピストン的動きの可能性およびシェル部品からのベアリング挿入物の移動を防ぐ。

本発明を実行するための最良の実施態様
図4においては、本発明の寛骨臼カップ・プロテーゼのシェル部品の1つの好ましい実施例の部分断面図が示されている。シェル部品10の外側表面は輪郭付けをされており、頂端部12、縁部14および小さなベース部16から構成されている。頂端部12の表面湾曲は、ほぼ楕円曲線E1に従う断面の表面湾曲を有する楕円体である。楕円曲線E1の短軸は、紙面上で中央の軸線18に沿って決定され、一方楕円曲線E1の長軸は中央の軸線18に垂直に決定される。シェル部品10の好ましい実施例において、楕円曲線E1の長軸は、シェル部品10が移植される寛骨臼の大きさに依存して、楕円曲線E1の短軸より0.5乃至4.0ミリメートル大きい。
引き続き図4を参照すると、複数の固定リブ20はシェル部品10の縁部14に沿って配置されていることが認められる。固定リブ20は、平行に配列され、シェル部品10の中央軸18を取り巻いて同軸に位置される。さらに、遷移リブ21はシェル部品10の縁部14の上部に配置される。遷移リブ21も、固定リブ20と平行に配列され、シェル部品10の中央軸18を取り巻いて同軸に位置される。
図5aにさらに明瞭に示されるように、各固定リブ20は、第1および第2の上方に拡がる各側壁表面22および24を含むほぼダブテール型の断面形状を具備し得る。図示された実施例において、各固定リブ20の第1の表面22は、予め定められた傾斜角度Aだけ水平より上に傾斜される。この様にして各固定リブ20の第1の表面22は、他の全ての固定リブ20の対応する第1の表面に平行である。各固定リブ20の各第1の表面22の傾斜角度Aは0°乃至90°の如何なる角度ででもあり得るが、好ましい実施例における傾斜角度Aは約30°である。
各固定リブ20の第2の表面24は、予め定められた下方への傾きの角度Bだけ水平より下に傾斜する下降表面24を有する。それ故、各第2の表面24は他の全ての固定リブ20の対応する第2の表面に平行である。各第2の表面24の下方への傾きの角度Bは0°乃至90°の如何なる角度ででもあり得るが、好ましい実施例における下方への傾きの角度Bは約5°である。
遷移リブ21は各固定リブ20の第2の表面24に平行である下降表面25を有し、同じ下方への傾きの角度Bを共有する。遷移リブ21は、遷移点Tでシェル部品10の頂端部12の楕円曲面と交わる線形の上部表面27を有する。線形の上部表面27は、傾斜角度Cだけ遷移点Tを通過する水平面より下に傾斜する。下方への傾きの角度Cは如何なる鋭角ででもあり得るが、好ましい実施例における下方への傾きの角度Cは約45°である。
遷移リブ21および固定リブ20は全て平行な平面に配置されるので、溝34は、遷移リブ21と次の隣接する固定リブ20との間、および各固定リブ20間に存在する。図示された実施例において、各溝34は遷移リブ21の下降表面25または固定リブ20の第2の表面24によって第1の側部を規定される。各溝34の反対側の側部は、隣接する固定リブ20の第1の表面22によって規定される。結果として、各溝34は、ダブテール型の断面の輪郭を有する各溝34を提供するように作用する上方に広がる側壁によって規定される。
図5bにさらに明瞭に示されるように、固定リブ20は、各リブ20の外側表面23が楕円曲線E1を越えて同心に配置されるように楕円曲線E1をまたぐことが好ましい。同様に、各溝34のベース表面37は楕円曲線E1より内側で同心に配置される。勿論、代りに、リブ外側表面23および溝ベース表面37は、楕円曲線E1によって規定される軸と異なる長軸および短軸を有する第2の楕円曲線(図示されていない)の上および下に延在するようにそれぞれ輪郭を有することができる。
製造の平易さのため、各溝34のベース37は平らで、ほぼ楕円曲線E1に近似させられることができることは容易に理解される。後に説明されるように、固定リング20の形状および溝34のダブテール形状は、骨が溝34の中に成長することを可能にし、中に成長した骨がシェル部品10に機械的に係合することを可能にする。
再び図4を参照すると、シェル部品10のベース部16は、面取りされた縁部52まで最下部の固定リブ20の第2の表面24の下に延在する楕円上にほぼ垂直な表面50を含むことが分かる。面取りされた縁部52は、シェル部品10の水平なベース表面54にほぼ垂直な表面50を接合する。

It is contemplated that the two-part acetabular cup will be assembled at the time of surgery for the convenience of insert size selection and external shell insertion. This manual assembly during surgery requires the manufacturer to ensure long-term compatibility of the parts, and this compatibility will leave a gap between the two parts during most, if not all, assembly. Guide the resulting dimensional tolerances. Summary of the Invention
The present invention is an acetabular cup prosthesis comprised of an outer shell component that includes a cavity that receives an internal bearing insert, and a method of implanting an acetabular cup within a patient's acetabulum.
The shell component of the acetabular cup has an outer surface that is implanted within the patient's acetabulum. The outer surface is composed of a plurality of parts, all of which are in accordance with the curvature of one or more ellipsoids. In order to implant the shell component within the patient's acetabulum, the acetabulum is spheroidized to define a volume that is slightly smaller than the volume surrounded by the outer surface of the shell component. When the shell part is inserted into a spherically-shaped acetabulum, the acetabulum deforms to approximately the same shape as one ellipsoid. Thus, when the shell part is inserted into the patient's acetabulum, the shape of the acetabulum is such that the force of the acetabular wall substantially touching all outer surfaces along its shape and simultaneously tightening is It is deformed to accommodate the outer shape of the shell part so that it only occurs in the part.
Annular fixation ribs and fixation grooves are formed on the outer surface of the shell part in the region adjacent to the edge. The fixation rib forms an acetabulum and replaces the surrounding bone when the shell part is implanted in the patient. The fixation rib mechanically engages the bone to join the shell component to the acetabulum. The fixed groove usually has an upwardly converging side wall and thus has a dovetail or mortise cross-sectional profile. When the shell part is implanted in the acetabulum, the bone grows into the fixation groove. The dovetail mold of the fixed groove allows the bone and shell parts grown therein to fit together dimensionally. The mechanical force generated by the shell part joining to the acetabulum and the presence of an interference fit and fixation ribs keeps the shell part in place in the acetabulum and immediately replaces the prosthetic shell part. The bearing insert is retained in the receiving cavity of the shell part of the cup by an interference fit. The generation of an interference fit substantially eliminates the gap between the shell part and the bearing insert that crosses the various surfaces of the abutting shell part and the bearing insert.
