Description
Izum se odnosi na sredstvo za njegu zuba sa obnavljajućim djelovanjem površine zuba kao i na postupak za obnavljanje površine zuba. Sredstva za njegu zuba služe u prvom redu za čišćenje površine zuba od ostataka hrane, promjena boja i bakterijskih zubnih naslaga koje čvrsto prianjaju.
Dalje se pokušava specijaliziranim dodacima, npr. pomoću fluor spojeva ili antimikrobijalnih materija spriječiti oboljenje zuba kao karijes ili paradontoza.
Kao jedan od prvih stadija zubnog karijesa zapažene su lezije u zubnoj caklini i otvorenim kanalićima dentilne (tzv. Tomes Pits), koje nastaju procesima razgradnje pod utjecajem bakterije koje stvaraju kiselinu.
Ova kvarenja supstance dentina mogu se zapaziti na npr. osjetljivošću zubnog vrata prema temperaturnim promjenama.
Dok se pomoću dodataka desenzibilizirajućih aktivnih tvari suzbijaju jedino bolni simptomi, već se pokušalo pomoću dodataka koji smanjuju razgradljivost apatiteta, da se spriječi stvaranje takvih lezija površine zuba.
U novije vrijeme činjeni su već također i prijedlozi da se prisutna kvarenja smanje pomoću remineralizirajućih sredstava za njegu zuba.
Tako je predloženo od strane Chow i Brown-a (u J.Dent. Res., 54 (1975) 65 - 70), da se za remineraliziranje dentina stavi dikalcij fosfat dihidrat. Iz US 4,097,588 je bila poznata vodica za usta sa remineralizirajućim djelovanjem, koja je bila zasićena sa CaHP04 . 2H20.
Do sada poznati pokušaji da se na ovaj način obnovi površina zuba vodili su pak do nekontroliranog rasta hidroksiapatit kristala na površini zuba, koji je daljnjem napadu na površinu zuba pružao malo otpora.
Stoga je cilj ovog izuma, da razvije zubnu pastu, koja uzrokuje kontroliranu remineralizaciju, naročito u brazdama ogrebotina i kanalima dentina i ova u velikoj mjeri izravnava, tako da se stvara zatvorena, glatka površina zuba.
Otkriveno je da se ovaj cilj u velikoj mjeri postiže pomoću kombinacija sredstava za poliranje prema izumu.
Predmet izuma je zubna pasta za obnavljanje površine zuba sa sadržajem sredstava za poliranje, fluor spojeva, sredstava za održavanje vlažnosti, veziva i vode, koja kao sredstvo za poliranje sadrži kombinaciju silicijeve kiseline i dikalcij fosfat dihidrata (Brushit) u odnosu mase od 10:1 do 1:1.
Zapaženo je da se pravilnom obradom zuba pomoću paste za zube prema izumu postiže zatvaranje kanala dentina uz izravnavanje u velikoj mjeri površine zuba.
Kao silicij kiselinska sredstva za poliranje pogodne su sve galsilicijeve kiseline, hidrogalsilicijeve kiseline i taložne silicijeve kiseline koje su poznate kao tijela za čišćenja.
Galsilicijeve kiseline se izrađuju reakcijom natrij silikatnih otopina sa jakim, vodenim mineralnim kiselinama uz stvaranje hidrosola, stvaranjem u hidro-gel, ispiranjem i sušenjem.
Ako se sušenje obavlja pod blagim uvjetima na sadržaj vode od 15 do 35 mas. -%, tada se dobivaju takozvane hidrogelsilicijeve kiseline, kako su one poznate npr. iz US 4,153,680. Sušenjem na sadržaje vode ispod 15 mas.% obavlja se ireverzibilno smežuravanje prethodno rastresite strukture hidrogala u gustu strukturu tzv. kserogela.
Takve kserogel silicijeve kiseline su npr. opisane u Us 3,538,230. Druga, podobna grupa silicij kiselinskih sredstava za poliranje koja se pretpostavlja su taložene silicijeve kiseline.
Ove se dobivaju taloženjem silicijeve kiseline iz razblaženih otopina alkalnog silikata dodavanjem jakih kiselina pod uvjetima pri kojima ne može na-stupiti agregacija u sol i gel.
Pogodni postupci za izradu taloženih silicijevih kiselina su npr. opisani u DE-OS 25 22 486 i u DE-OS 31 14 493.
Pogodno se pretpostavlja prema DE-OS 31 14 493 izrađena taložna silicijeva kiselina sa BET površinom od 15 - 110 m2/g, veličinom čestica od 0,5-20 u, pri čemu treba biti najmanje 80 mas. - % primarnih čestica ispod 5 u, i viskozitetom u 30%tnoj disperziji glicerin-voda (1:1) od 30 - 60 Pa.s (20 °C) u količini od 10 -20 mas. - % zubne paste.
Pogodne taložne silicijeve kiseline koje se pretpostavljaju osim toga imaju zaobljene kutove i rubove i mogu se dobiti pod trgovačkom oznakom Side-nt R12 (DS) (DEGUSSA).
Da bi se dobio dovoljno visok udio silicijeve kiseline sa srednjom veličinom čestice od manje od 5 u i naročito za udio od najmanje 3 mas. - %, rač-nato na cjelokupnu zubnu pastu, silicijeve kiseline sa primarnom veličinom čestice od 1 - 3 u, stavlja se pogodno pored navedene taložne silicijeve kiseline još jedna finijeg tipa sa BET površinom od 150-250.
Pogodan tip silicijeve kiseline je npr. Sipernat (R)22LS (DEGUSSA), koji se u zubnoj pasti stavlja u količini od 1 - 5 mas. - %.
Pokazalo se kao naročito pogodno za tvorbu glatke površine zuba, ako zubna pasta sadrži prema izumu male količine svježe taložene, tj. in situ pri izradi paste izrađena silicijeva kiselina.
Ova se postiže npr. time, što se pri izradi zubne paste npr. dodavanjem limunske kiseline održava pH vrijednost od 3 - 5 i potom se dodatkom malih količina vodene otopine natrij silikata pH vrijednost povisi na 7-7,5.
Na ovaj način in situ stvorena silicijeva kiselina je izvanredno finočestičasta i iznosi manje od 0,1 mas. - % zubne paste.
Pored silicij kiselinskih sredstava za poliranje mogu se u manjem obujmu sadržavati i druge poznate tvari za poliranje, npr. aluminij oksid u obliku slabo kalcinirane gline sa sadržajem γ- i α-aluminij oksida.
Takav aluminij oksid može se dobiti pod trgovačkom oznakom Poliertoner-de PLO feinst (Giulini- Chemie). Odnos se takvih tvari u datom slučaju dodatnih komponenata sredstava za poliranje u silicij kiselinskom sredstvu za poliranje ne bi ipak trebao biti viši od (1-15) : 100.
Druga, obvezna komponenta sredstva za poliranje je dikalcij fosfat dihidrat (CaHPO4 . 2H2O) koji se u prirodi nalazi kao brshit. Dikalcij fosfat dihidrat se odavno primjenjuje kao sredstvo za poliranje u zubnim pastama i može isto tako da se dobije u pogodnim veličinama zrna u trgovini od 1-50 u. Količine stavljanja koja se pretpostavlja iznosi 1 - 5 mas. - % zubne paste.
Zubna pasta prema izumu po mogućnosti ne bi trebale sadržavati daljnje komponente za poliranje koje grade kalcij ione kao na pr. kredu ili druge kalcij fosfate.
Zapaženo je naprotiv da magnezijevi ioni imaju povoljan utjecaj na kontroliran rast hidroksiapatita koji raste u zubnim lezijama, potpomažu ubrzano otvrdnjavanje zubne cakline, i otuda stvaraju daljnji doprinos za rješenje zadatka prema izumu.
Daljnji predmet izuma je otuda zubna pasta prema izumu, koja sadrži u vodi topljivu magnezijevu sol u količini koja odgovara 0,1 - 0,5 mas. - % Mg.
Kao soli magnezija pogodne su sve soli; koje su u vodi na 20° C bar do 0,5 mas. - % (računato kao Mg) topljive, npr. MgSO4, MgCl2 i magnezij-monofluorfosfat (MgPO3F).
Dalje je utvrđeno, da fluorfosfatni ioni imaju povoljan utjecaj na kontrolirani rast kristala hidroksiapatita. Otuda se pretpostavljaju zubne pa-ste prema izumu, koje kao fluor spojevi sadrže monofluorfosfat u količini koja odgovara 0,2 - 2,0 mas. - % (PO3F). Za ovo su pogodne npr. alkalne soli, npr. u trgovini uobičajeni natrij monofluorfosfat.
Sadržaj magnezij iona i fluorfosfat iona koji se pretpostavlja, može se posebno prigodno postići tako, što se kao fluor spoj sadrži magnezij monofluorfosfat.
Kao nosač za zubne paste prema izumu, koji omogućuje podešavanje pogodne konzistencije za doziranje iz tube, posuda za raspodjelu ili savitljivih boca na osnovi kombinacije sredstava za poliranje prema izumu, pogodna je kombinacija od sredstava za održavanje vlažnosti mogu se staviti na pr. glicerin, sorbit, ksilit, propilen glikol, polietilen glikol, polietilen glikoli, naročito takva sa srednjim molekularnim masama od 200 - 800.
Kao regulatori konzistencije (odn. veziva) služe prirodni i/ili sintetski polimeri topljivi u vodi kao alginati, karagenati, tragant, škrob i škrobni eteri, celulozni eteri kao npr. karboksimetil celuloza (Na so), hidroksietil celuloza, metilhidroksipropil celuloza, guar, guma akcija, agar-agar, ksantan guma, brašno rogača, pektini, u vodi topljivi karboksivinil polimeri (npr. Carbopol (R) tipovi), polivinil alkohol, polivinilpirolidon, polietilen glikoli, naročito takvi sa molekularnim masama, od 1 500 - 1 000 000.
Daljnje tvari, koje su pogodne za kontrolu viskoziteta, su npr. slojeviti silikati kao npr. monmorilonitne gline, koloidne silicijeve kiseline za zgušnjavanje kao npr. aerogel-silicijeve kiseline, pirogene silicijeve kiseline ili najfinije samljevene taložene silicijeve kiseline.
Za izradu zubnih pasta sa kombinacijom sredstava za poliranje prema izu-mu naročito dobar pogodan nosač sadrži
15 - 25 mas. -% glicerina
10 - 20 mas. -% sorbita
1 - 5 mas. -% polietilen glikola MM : 400
1 - 5 mas. -% silicijeve kiseline za zgušnjavanje
0,1 - 1 mas. -% ksantan gume i 35-45 mas. -% vode.
Zubne paste mogu u njihovom viskozitetu isto tako nisko biti podešene, da se mogu dozirati kao tekuće sredstvo za čišćenje zuba sa viskozitetom od 2 000 - 10 000 m.Pa.s (25°C) iz savitljive posude od plastične mase na zubnu četkicu, tamo prodrijeti između čekinja, ali da ne otkaplju sa četkice za zube.
Za ovu svrhu pogodno je kao vezivo kombinacija koja se pretpostavlja od 0,1 - 1 mas. -% ksantan gume i 0,01 - 5 mas. -% dodatka koji stabilizira viskozitet iz grupe
- hidroksipropil supstituiranih hidro koloida ili
- kopolimera polietilenglikol/polipropilenglikola sa srednjom molekulamom masom od 1 000 do 5 000.
Daljnji uobičajeni dodaci zubnim pastama, koji se ukupno mogu iznositi čak do 5 mas. -% sastava zubne paste, su npr. tenzidi za potpomaganje djelovanja čišćenja i u danom slučaju za razvijanje pjene pri četkanju zuba i za stabiliziranje disperzije komponenata sredstava za poliranje u nosaču.
Pogodni tenzidi su npr. linearni natrij alkil sulfati sa 12 do 18 C-atoma u alkil grupi.
Ove tvari imaju dodatno djelovanje koje koči enzim bakterijske izmjene tvari zubne naslage. Daljnji pogodni tenzidi su alkalne soli, pretpostavljaju se natrijeve soli alkilpoliglikoletersulfata sa 12 do 16 C-atoma u linearnoj alkil grupi i 2 - 6 glikoleterskih grupa u molekuli, linearnog alkan (C12-C18)-sulfonata, monoalkil (C12-C18) estera sulfojanitame kiseline, sulfatiranih monoglicerida masnih kiselina, sulfatiranih alkanolamida masnih kiselina, alkil (C12-C16)-estera sulfooctene kiseline, acilsarkozina, aciltaurida i acil-izetionata sa uvijek 8-18 C-atoma u acil grupi.
Isto tako su pogodni amfotemi, amfolitički i neionski tenzidi, npr. okse-tilati mono- i digliceridi masnih kiselina, sorbitam estera masnih kiselina i alkil (oligo)-glukozidi.
Daljnji uobičajeni dodaci zubnih pasta su
- konzervansi i antimikrobijalne tvari, kao npr. metil, etil ili propil esteri p-hidroksibenzove kiseline, natrij sorbat, natrij benzoat, bromklorofen, ester fenil-solicilne kiseline, bigvanidi, timol itd.
- sredstva za zaslađivanje kao npr. saharin natrij, natrij ciklamat, sukroza, laktoza, maltoza, fruktoza,
- aroma kao npr. ulja od nane, ulje obične nane, ulje eukaliptusa, ulje anisa, ulje divlje mirodije, ulje kima, mentil acetat, cimetni aldehid, anetol, vanilin, timol kao i smjesa ovih i drugih prirodnih i sintetskih aroma,
- pigmenti kao npr. titan dioksid ,
- boje
- puferne supstance kao npr. primarni, sekundarni ili tercijarni alkalni fosfati ili limunska kiselina/natrij citrat,
- tvari koje liječe rane i koje koče upalu kao npr. alantoin, karbamid kao i azulen, aktivne tvari kamilice, derivati acetil salicilnne kiseline.
Sljedeći Primjeri trebaju predmet izuma objasniti pobliže.
Primjeri
Izrađene su zubne paste prema izumu (Primjer 1, 2 i 3) i jedna usporedna zubna pasta (Primjer V) (uspor. Tabelu 1).

Dokaz djelovanja
Sa zubnom pastom prema izumu (Primjer I) i usporednom zubnom pastom (Primjer V, bez dikalcij fosfat dihidrata i bez magnezij sulfata) izvedeni su sljedeći oglede:
Iz ljudskog zuba pilom su isječeni 4x5mm veliki komadi (tzv. slabs) iz krune (za oglede sa caklinom) 1 iz područja korijena (za oglede sa dentinom) i površina se izravna pomoću papira za mokro brušenje i glatko ispolira sa polir pastom.
Slabs cakline se dodatno nagrizaju pomoću 0,1 molarne vodene mliječne kiseline, koja je sadržavala 500 mg/1 hidroksiapatita, pri pH = 4,6 i T = 37 °C preko 6 sati.
Tako pripremljeni uzorci se potom dva puta dnevno čiste 5 minuta sa oglednom zubnom pastom (koja je razblažena 1:1).
U preostalom vremenu se uzorci na 37°C drže u CaHPO4 . 2H2O suspenziji, koja svježe izrađivala dva puta dnevno ( ujutro i uvečer ) taloženjem (CaCl2 + Na2HPO4 --- CaHPO4 + 2NaCl). Ova obrada zuba se uvijek izvodila 20 dana dugo.
Prije obrade (tj. poslije poliranja slabs) i poslije završetka obrade obavljena su snimanja površine uzoraka rasterelektromikroskopsi (REM).
Na Slikama 1 - 6 su ovi dani u srazmjeru 5000 : 1.

Sl. 6 Ljudski dentin, obrađen pomoću zubne paste Primjera 1. Jasno se zapaža u velikoj mjeri zatvaranje kanala dentina uslijed izdvojenog hidroksiapatita.
Claims (5)
1. Zubna pasta za obnavljanje zubne površine sa sadržajem sredstava za poliranje, fluornih spojeva, sredstava za održavanje vlažnosti, veziva i vode, naznačena time, što kao sredstvo za poliranje sadrži kombinaciju silicijevih kiselina i dikalcij fosfat dihidrata (brushit) u odnosu mase 10 : 1 do 1: 1.
2. Zubna pasta prema zahtjevu 1, naznačena time, što se sadrži magnezijeva sol topljiva u vodi u količini koja odgovara 0,1 - 0,5 mas. -% magnezija.
3. Zubna pasta prema zahtjevu 1 ili 2, naznačena time, što se kao fluorni spoj sadrži monofluorfosfat u količini koja odgovara 0,2 - 2,0 mas. - % ( PO3F-).
4. Zubna pasta prema zahtjevu 1 do 2, naznačena time, što se kao fluorni spoj sadrži magnezij-monofluorfosfat.
5. Zubna pasta prema zahtjevu 1 do 4, naznačena time, što se kao sredstvo za poliranje sadrži taložena silicijeva kiselina sa srednjom veličinom dijelova od 0,5 - 30 mikrona ( u) u količini od 10 - 20 mas. - % i dikalcij fosfat dihidrat (brushit) u količini od 1 - 5 mas. - %.
