DESCRIPTION Un laryngoscope ou vidéo-laryngoscope est un instrument utilisé dans le cadre de la procédure d'intubation au cours d'une anesthésie générale et visant à faciliter la mise en place de la sonde d'intubation par laquelle s'effectue la ventilation du patient. Le laryngoscope ou vidéo-laryngoscope comprend en général deux parties assemblées mécaniquement: un manche réutilisable contenant les dispositifs d'éclairage et une lame invasive réutilisable ou à usage unique ; dans le cas d'une lame réutilisable celle-ci doit être impérativement décontaminée entre deux interventions. Le manche et le dispositif d'éclairage ou de capture d'images sont en général décontaminés partiellement afin de préserver leur durée de vie. La sonde est guidée lors de son introduction par une forme en coin de la partie de la lame libre de contact avec la langue ou bien par un canal ouvert latéralement, dont les parois sont munies de dispositifs qui maintiennent la sonde vers la partie distale de la lame. Des systèmes mécaniques introduits dans la sonde permettent de modifier la courbure de la sonde ou à l'image des endoscopes insérés dans la sonde de guider l'introduction entre les cordes vocales ; Leur usage nécessite le retrait après positionnement de la sonde, montrant par la même l'utilité de l'invention présente qui en supprime avantageusement l'utilisation. Les lames de laryngoscopes qui utilisent la présente invention ont, contrairement aux solutions existantes, l'encoignure ou le canal destiné à guider la sonde trachéale ouvert du côté de la face de la lame en contact avec la langue. L'utilisation de l'instrument est alors sensiblement améliorée notamment grâce au canal qui permet le pré positionnement ou le guidage de la sonde sur la lame ; il suffit alors de pousser la sonde, alors que la vision des cordes vocales est réalisée avec l'instrument, pour mettre celle-ci en place sans traumatisme. La lame peut ensuite être extraite de la bouche sans affecter le positionnement de la sonde. Le canal (fig. 4, N°3) dans lequel la sonde est pré- installée (fig. 5 N°8) avant l'utilisation de la lame ou bien glissée une fois les cordes vocales découvertes est tout ou partiellement ouvert sur sa surface côté langue (fig.3 et fig.4) afin d'utiliser la langue elle même comme élément de guidage et de maintien de la sonde dans le canal. Les bords supérieurs de la paroi externe verticale du canal peuvent être alignés avec la face interne de la lame (fig. 2 N°2). Selon la profondeur du canal, la sonde pré positionnée, en coopération avec les bordures du canal sert de surface complémentaire d'appui sur la langue. La paroi inférieure du canal peut être alignée et dans le plan de la face externe de la lame afin de rendre la lame lisse et facile à introduire dans la bouche (fig.1, N°1). Les dimensions et formes en coupe transversale du canal peuvent varier en fonction des sondes utilisées (fig.5 N°8). Afin de maintenir la sonde dans le cas d'une utilisation pré-positionnée et d'améliorer le guidage dans les autre cas, l'une ou les parois latérales internes du canal peuvent être légèrement creusées au niveau du contact avec la sonde ou munies d'un relief longitudinal réduisant l'espace entre les parois au dessus du diamètre de la sonde (fig. 5 N° 8). La paroi inférieure du canal qui épouse extérieurement la courbure de la lame peut être dans sa partie interne au canal d'une épaisseur variable sur sa longueur (fig.4 N° 5, 6) afin d'accommoder au mieux le guidage de la sonde et notamment de modifier la courbure de la sonde afin d'obtenir une visée parfaite entre les cordes vocales lors de son introduction.The laryngoscope blades that use the present invention have, unlike existing solutions, the corner or the channel for guiding the tracheal probe open on the side of the face of the blade in contact with the tongue.
