Abstract

The nozzle has a dynamic system of distal anchoring, which prevents a detaching of the nozzle from a passage when the system is opened. The anchoring frees the nozzle to enter and come out from tissues, when the system folds up. An external compression system compresses a tissue wall crossed between the compressing system and the unfolded dynamic distal anchoring, when a lid of the system is screwed by an external screw.

Description

La présente invention concerne une canule arthroscopique qui sert commeThe present invention relates to an arthroscopic cannula which serves as

Elle permettrait de réduire le  protection of the access path of the instruments used during arthroscopic surgery. It would reduce the

Arthroscopy is a rapidly expanding field of surgery. The surgeons' learning curve is increasing and the techniques are becoming more and more efficient.

To support this progress, the equipment must be modified accordingly.

Currently the arthroscopic cannula used fulfills the function of keeping the introductory site open and free for the surgical material to be introduced without letting air or water escape from the operating site. Some have an option for anchoring the fabrics they pass through. To prevent the escape of air or water outside, different valve systems are designed, ranging from the latex membrane to the mechanical valves. Several problems in the current arthroscopic cannula consume the attention of the surgeon and the operating time which are important factors in this surgical practice: 1- the surgeon is always careful not to get the cannula out of its conduit, which occupies a part of his attention. Similarly by removing the material from the cannula, often the latter is entrained outside the tissue, forcing the surgeon to introduce it again or else to reposition it, which also consumes time.

2- if the cannula leaves its place, the liquid or the gas can escape into the tissues, which produces an infiltration of the tissues and a reduction in the operating space to be viewed. This edema, in some cases like arthroscopy of the shoulder, will make the rest of the operation very annoying even very difficult.

3-the site of introduction of the cannula is a tapered and bleeding place which also impairs vision in a sometimes very narrow place and requires the surgeon to consume very precious time in cleaning this site.

4-the cannula is of non-modifiable length which often hinders instruments which are not long enough and limits their use.

5- the mobility of the instruments which pass through the cannula is partly inversely proportional to its length, often the cannula limits their amplitude.

The cannula according to the invention overcomes these problems mentioned above. It has the following characteristics: 1- A cannulaA (1), which exists in different diameters according to the choice of the surgeon. It is graduated (1G) in such a way as to allow the surgeon to assess the thickness of the tissues crossed to reach the operating field. This graduation makes it possible to choose a certain cannula B (3) so as to obtain a length suitable for the thickness of the tissues crossed. The proximal part of the cannulaA can be in the form of a funnel (1P) to allow the manipulator to introduce the cannulaB-articulator assembly (10) more easily into the cannulaA.

A shutter (2) to allow the cannulaA to easily pass through the tissue after the skin incision made by the surgeon according to the drawing (6).

3- A cannula B (3) with a diameter slightly smaller than that of cannula A, made in several lengths to adapt to the thickness of the tissues crossed. This length is estimated by the graduations on the cannula A according to the drawing (6 N). The cannulaB is articulated with an articulator (4) to be introduced through the cannulaA. The articulation between cannula B and articulator can be through an internal thread on the proximal part of the cannula B (3 I) and an external thread on the distal part of the articulator (4 F), or in other forms of articulations .

The distal part of the cannula B must include a dynamic distal anchoring system (3 P) which can be in the form of rectangular, triangular, claw or other shapes, so as to allow compression of the tissues crossed. The distal anchoring system must be designed so as to bend elastically in the cannulaA, during the introduction of the cannulaB-articulator assembly, and to unfold at the end of the gesture (10). The extraction of cannula 20 follows the reverse path (16-17-18-19-20).

4- An articulator (4) of slightly smaller diameter than that of the cannula B with a distal thread (4 F), or other form of articulation, which allows, according to the drawings (8-9-10-11-12-13 ), keep the cannula B in place while waiting for the next gesture.

De même, il doit avoir la capacité de laisser la canule B et l'articulateur passer à travers un toit ouvrant (5 C).  5- A cap (5), of diameter slightly larger than that of the cannulaB, which is articulated with the cannulaB. This cap can contain an elastic membrane with holes to facilitate the introduction of surgical instruments without leaking water or air (5 E), or other valve system. Likewise, it must have the capacity to allow the cannula B and the articulator to pass through a sunroof (5 C).

A second model of this cannula is designed and comprises: 1- a cannulaC (21) composed of a proximal part (21A), made to receive a cover (23), of a threaded body in its external part (21D) and an anchoring system (21E-24). The threaded body allows a cylinder (21C) to screw or slide over the entire length of the thread which produces a compression effect. The distal part of the cannula C is formed by an anchoring system controlled proximally by an external cylinder (21 B). The anchoring system is formed by several pins (24) which are bent proximally and distally in a sagittal plane so as to form claws. The pins are incorporated into the cannula. The rotation of the pins made proximally using a cylinder (21 B) allows the claws to move away from the cannula thus forming the distal anchoring system of the cannula (25) (26).

2- a cover (23) which is articulated with the proximal part of the cannulaC. This cover contains in its central part an elastic membrane (23 E) or other valve system.

3- A obturator (22) The solutions provided by the present invention are as follows: 1- the distal anchoring system which includes fins (3 P) or claws (24 A) fixes the cannula inside the tissues which allows it to be left in place during the introduction and extraction of surgical material.

2- the compression system generated by screwing the cap (13-1415) or the cylinder 15 (21B) increases the pressure in the tissues crossed which has a hemostatic effect and reduces the infiltration of tissues by water or l 'air. Likewise, the compression of the tissues will increase the operating space by a few millimeters, which is indeed considerable since we often estimate the working space in certain regions like the shoulder by a few millimeters itself.

3- distal anchoring and compression add stability to the cannula which reduces recurrent fraying at the distal end.

4- the modularity of the cannula and the different sizes of the cannulaB, allow to gain length for the instruments and amplitude in the field of work.

The installation in place of the cannula B is carried out according to the following steps: 25 - Skin incision.

The screwing of the cap is done in reverse of the articulator which allows to tighten well and to obtain the required compression.

-The ablation of the cannula or its movement to another level is done by the reverse path according to the drawings (16-17-18-19-20).
