Definitions
- the invention relates to an imaging device for generating digital image data of a body region of a patient.
- the invention further relates to a therapy system with such an imaging device.
- three-dimensional image data of the patient's body are usually used to determine the area of the body to be treated, e.g. To locate a tumor as accurately as possible in advance of the actual therapy.
- These image data are usually acquired in an imaging step preceding each treatment session, in particular to optionally alter the spatial location of this body region in relation to previous treatments, e.g. to recognize a change in size or displacement of a tumor in the body tissue.
- conventional medical imaging methods in particular computed tomography (CT), magnetic resonance tomography (MR), positron emission tomography (PET) or single photon emission computed tomography (SPECT) are used.
- CT computed tomography
- MR magnetic resonance tomography
- PET positron emission tomography
- SPECT single photon emission computed tomography
- radiation therapy encompasses radiation therapy in the narrower sense in which high-energy electromagnetic radiation is applied to tumor tissue, as well as particle therapy methods in which tumor tissue is bombarded with particle radiation (for example accelerated protons, carbanions, etc.).
- the machine settings of the irradiation device - eg irradiation direction, position, field size, etc. - are usually set relative to a given patient isocenter in the patient's body which forms the origin of a patient coordinate system. This point can for example, in the middle of a tumor volume visible on the three-dimensional image data.
- the invention has for its object to provide an imaging device and a therapy system that simplifies the implementation of a therapy, in particular radiotherapy.
- the imaging device comprises a table for supporting a patient (hereinafter referred to as an imaging table) and an image acquisition unit for generating digital image data that images a region of the interior of the patient.
- the image acquisition unit does not scale the coordinates of the recorded image data (only) with respect to a patient coordinate system, but (at least) with respect to a spatial coordinate system with respect to which the imaging table is fixedly mounted.
- the coordinates of the image data are spatially scaled in relation to a table coordinate system that is stationary relative to a patient support, ie the "tabletop" of the imaging table, is defined.
- scaling is used literally in the sense of "provided with a (spatial) scale".
- the image data set is thus assigned a location or location information that assigns each pixel of the image data set a point within the room or table coordinate system, and thus a room point.
- the scaling thus relates to the coordinates of the pixels within the image data set.
- the scaling does not involve a change in size, i. the number of pixels, the image data set.
- spatial coordinate system comprises any coordinate system in which the imaging table is mounted in a fixed position, that is, in a constant spatial relationship with at least one attachment point of the imaging table.
- the spatial coordinate system is also defined in terms of location with respect to the surrounding imaging space, and the imaging table is mounted correspondingly stationary relative to the surrounding imaging space.
- a mobility of the imaging table relative to the surrounding imaging space may also be provided, in which case the spatial coordinate system is moved with the imaging table.
- the spatial coordinate system and the table coordinate system form substantially equivalent or redundant reference systems for describing the location of a spatial point in the imaging space, one or both of which may be explicitly defined within the scope of the invention.
- the table coordinate system and the spatial coordinate system are - at least as long as the (optional removable) patient support is connected to the imaging table - in a defined spatial relationship and are interconvertible.
- the image data generated during a measurement have an absolute reference to a spatial region, which in turn is in a defined spatial relationship with the imaging device.
- recordings can be reproduced identically or substantially identically in a particularly simple and precise manner.
- the position of a body region to be treated can thereby be characterized independently of markings on or in the body of the patient and the invasive introduction of implanted markings (so-called BBs) in the vicinity of the tumor and the associated risks for the patient avoided.
- BBs implanted markings
- the imaging device is designed as an X-ray system, in particular computed tomography.
- the imaging device may also be based on another imaging technique, in particular MR, PET or SPECT.
- the digital image data contain three-dimensional image information, ie form a 3D image data record or volume data record. The invention is optionally applied to two-dimensional image data.
- the patient support of the imaging table is intended for immobilized and substantially reproducible recording of the patient.
- the patient support is designed in particular in the form of a couch which is used to immobilize the patient e.g. equipped with a customized to the patient foamed plastic shell or other adjustable patient fixation organs.
- the patient support may also be designed in the manner of a chair, so that the patient is stored in a sitting position.
- the patient support is preferably reversibly removable from a fixed mounted tripod of the imaging table. This has the advantage that the patient is in immobilized state Further treatment steps can be supplied and thus allows both a time-saving treatment process as well as a particularly precise treatment planning, especially since the posture of the patient after the localization of the patient isocenter no longer needs to be changed. This embodiment also has the advantage that a customized patient support device can be easily coupled to the imaging device for each patient.
- the patient support can be fastened on the stand only in a defined position.
- the stand is in particular adjustable, so that the patient support can be offset in particular in height, but optionally also horizontally relative to the spatial coordinate system.
- the tripod is this either manually adjustable or designed as a robot arm.
- the image recording unit of the imaging device is preferably displaceable relative to the imaging table, and thus relative to the spatial coordinate system, movable, in particular on rails or the like in a defined direction of movement.
- the position of the image acquisition unit in the spatial coordinate system is detected and used as input for scaling the coordinates of the image data using stored information about the geometry of the image acquisition unit, in particular during image acquisition.
- the therapy system comprises, in addition to the imaging device described above, a therapy device, in particular an irradiation unit for radiation treatment of the patient with high-energy electromagnetic radiation or accelerated particles includes.
- the therapy device is associated with a further spatial coordinate system in which a table for supporting the patient (hereinafter referred to as therapy table) is arranged stationary. Additionally or alternatively, the therapy device is assigned a further table coordinate system, which is arranged stationarily with respect to a patient support of the therapy table.
- the imaging device and the therapy device are in this case matched by appropriate calibration or adjustment to one another such that on the basis of the scaling, i. the location information of the coordinates of the image data recorded by the imaging device and a stored conversion rule a corresponding spatial area in the space or table coordinate system of the therapy device can be determined.
- the conversion rule is used to transmit the volume of space of the imaging space depicted in the image data to a corresponding volume of space of the therapy room.
- it can be determined from the scaled image data at which spatial point in the environment of the therapy device a body region of the patient, in particular a tumor, imaged in the image data is arranged, when the patient is stored on the therapy table.
- the scaled image data can be used directly for therapy planning.
- the tables of the imaging and therapy device are of a similar construction and set and arranged the same with respect to the respective spatial coordinate system.
- the conversion rule is reduced to a factor of 1, i.
- the position information of the image data identically reflects the position of the imaged body region of the patient stored in the therapy device in the associated room or table coordinate system.
- the tables of the imaging or therapy device constructed or set up differently or with respect to the respective Space coordinate system arranged differently, so the mediating between the two space coordinate systems conversion rule of 1 is different and based on the local conditions in each case to determine. If, for example, the table height of the therapy table is twice the table height of the imaging tables, then the scaling of the coordinates of the image data is corrected by the conversion rule with respect to the height by a factor of 2, etc.
- the therapy table has a removable patient support and a mechanical interface for docking a patient support, the patient support and interface being configured to be compatible with the corresponding parts of the imaging table.
- the similar design of the two tables allows an exchange of patient support between the imaging device and the therapy device, and thus in particular a transport of the patient in an immobilized state.
- FIG. 1 shows an imaging device 1.
- the imaging device 1 comprises a computer tomograph 2 (CT for short) as the image acquisition unit.
- the CT 2 includes a CT gantry 3 with a channel 4, in which a patient 5 during a computer tomographic examination is inserted.
- the CT gantry 2 is arranged and fixed on a rail system 6 along a displacement direction 7 displaceable on the floor 8 of an imaging space.
- the displacement direction 7 is horizontal and thus aligned in particular parallel to an isocentric axis 9 of the CT gantry 3, along which or parallel to which, as a rule, the patient 5 is aligned with respect to its body longitudinal axis.
- the patient 5 may be deviating but also inclined to the isocentric axis 9 stored.
- the imaging device 1 further comprises a table 10 for supporting and positioning the patient 5 in the examination position.
- the table 10 includes a stand 11, which is designed as a manually adjustable tripod or as a robot arm and is fixed with a stand base 12 stationary on the floor 8.
- a mechanical interface 13 At an end of the stand 11 spaced from the stand base 12, there is arranged a mechanical interface 13 by means of which a patient support 14 is detachably mounted on the stand 11 in a defined position.
- the patient support 14 is designed in the manner of a patient bed.
- the patient support 14 also has fixing organs 15, which immobilize the patient 5 on the patient support 14 in the intended examination position.
- the fixatives include e.g. an individually adapted to the patient 5 shell or mold made of foamed plastic, in which the patient's body or individual body parts thereof are embedded for the investigation.
- the CT gantry 3 moves over the splint system 6 along the displacement direction 7, the channel 4 being guided over the patient 5 arranged on the table 10, so that the patient 5 is moved relative to the CT gantry 3 by the patient Channel 4 is moved.
- a spatial coordinate system 16 is defined, which is arranged stationary in the imaging space, and in which the table 10 correspondingly is fixedly mounted with his tripod base 12.
- a table coordinate system 17 with respect to the patient support 14 is defined. Between the space coordinate system 16 and the table coordinate system 17 is - as long as the patient support 14 is in turn coupled via the interface 13 to the stand 11 - a defined spatial relationship, which depends only on the tripod setting.
- a third coordinate system hereinafter referred to as patient coordinate system 18, is conventionally defined in relation to the patient's body.
- the patient coordinate system 18 in the context of the present invention represents a purely auxiliary quantity, which is specified only concretely and for the purpose of clarification.
- the origin of the patient coordinate system 18 is usually placed in the center of a body region of the patient 5 to be treated, which contains, for example, one or more tumors.
- the patient coordinate system 18 particularly characterizes the position of these tumors within the patient's body.
- patient isocenter the origin
- the coordinates of the image data recorded by means of the imaging device 1 are not scaled (or at least not exclusively) with respect to the patient coordinate system 18, but with respect to the spatial coordinate system 16.
- the aim is to determine the absolute position of the patient's isocenter in the spatial coordinates and to transfer these into a therapy room.
- the traversing position of the CT gantry 3 on the rail system 6 is detected by the computer tomograph 2 during image acquisition.
- the computer tomograph calculates the position of the CT gantry therefrom 3 with respect to the spatial coordinate system 16.
- the position of the recorded 3D image data with respect to the spatial coordinate system 16 is determined therefrom . Based on this position information, the coordinates of the image data are scaled, ie, the determined position information is stored together with the 3D image data, so that an association of the image data to the recorded volume of space, based on the spatial coordinate system 16, is possible.
- the thus scaled image data thus contain the information about the absolute position of the patient isocenter with respect to the space coordinate system 16, and thus with respect to the imaging space. If the tripod setting is changed, the associated displacement of the patient isocenter in the room is taken into account by a stored correction function depending on the tripod adjustment. Similarly, when the patient support 14 with the patient 5 immobilized thereon is removed from the tripod 11 of the imaging device 1 and mounted on the tripod of another imaging or therapy room, the absolute location of the patient isocenter in the new imaging or therapy room is backed up by a stored conversion policy (which, for example, reflects a difference in size of the two stands). In particular, however, the positional information of the 3D image data can be taken over identically if both tripods resemble each other in their geometric structure, the tripod setting and their position with respect to a respectively assigned spatial coordinate system.
- the coordinates of the 3D image data are scaled with respect to the table coordinate system 17.
- the spatial relationship between the spatial coordinate system 16 and the table coordinate system 17 determined.
- the attitude information of the captured 3D image data is mathematically transformed from the space coordinate system 16 to the table coordinate system 17.
- the position of the patient isocenter is defined exclusively with respect to the patient support 14. This has the advantage that this position information is still directly usable even when the patient support 14 is removed from the stand 11.
- FIG. 2 shows a therapy system 19 for radiotherapy.
- the therapy system 19 comprises (at least) an imaging space in which the imaging device 1 described above is arranged, and (at least) a therapy room with a therapy device 20 which comprises an irradiation unit (not shown in more detail).
- the therapy device 20 is assigned a further spatial coordinate system 21, the origin of which is laid in particular in the beam path of a therapy beam (not shown in greater detail).
- the origin of the spatial coordinate system is placed in particular in the isocenter of the therapy beam.
- the therapy device 20 further comprises a table 22 with a stand 23, which is constructed identically to the stand 11 of the table 10 of the imaging device 1.
- the stand 23 is preferably mounted with its stand base 24 in the same position to the spatial coordinate system 21 as the stand 11 of the imaging device 1 with respect to the spatial coordinate system 16.
- the tripods 11 and 23 can also be arranged differently with respect to the respective spatial coordinate system 16 and 21, respectively. In this case, the different table arrangement is taken into account by a stored, empirically specified correction rule.
- the tables 10 and 22 are mechanically similar. In particular, they are also equipped with similar patient supports 14 or can exchange the same patient support 14.
- the patient 5 is first examined in the imaging device 1.
- the position and extent of the tumors, metastases, etc. to be treated with respect to the spatial coordinate system 16 and / or the table coordinate system 17 are ascertained for planning the subsequent irradiation by recording the image data.
- the patient support 14 is removed with the patient 5 immobilized thereon from the stand 11, brought to the therapy device 20 and coupled there to the stand 23 of the table 22.
- the absolute position of the patient isocenter in the therapy room be determined by a stored conversion rule. If the tables 10 and 22 are of the same construction, arranged and adjusted, the patient 5 lies in the same position relative to the spatial coordinate system 16 or 21 of the respective imaging or therapy chamber, in particular in the imaging device 1 or in the therapy device 20.
- the patient 5 can be moved into the irradiation position by adjusting the stand 23, wherein the displacement of the patient eye space in the spatial coordinate system 21 is taken into account during the tripod adjustment by a stored correction function.
- the irradiation position is in particular determined such that the patient isocenter in the Origin of the space coordinate system 21, and thus lies in the beam path of the therapy beam, in particular in its isocenter.
- the 3D image data recorded during the examination by the imaging device 1 and scaled to the spatial coordinate system 16 can therefore be used directly for therapy planning.
- the positioning of markers in or on the patient 5 is not necessary due to the absolute spatial reference of the 3D image data.

Abstract
Description
The invention relates to an imaging device for generating digital image data of a body region of a patient. The invention further relates to a therapy system with such an imaging device.
For therapy planning, in particular in the context of radiotherapy, three-dimensional image data of the patient's body are usually used to determine the area of the body to be treated, e.g. To locate a tumor as accurately as possible in advance of the actual therapy. These image data are usually acquired in an imaging step preceding each treatment session, in particular to optionally alter the spatial location of this body region in relation to previous treatments, e.g. to recognize a change in size or displacement of a tumor in the body tissue. To generate the three-dimensional image data set, conventional medical imaging methods, in particular computed tomography (CT), magnetic resonance tomography (MR), positron emission tomography (PET) or single photon emission computed tomography (SPECT) are used. Within the scope of the following remarks, the term radiation therapy encompasses radiation therapy in the narrower sense in which high-energy electromagnetic radiation is applied to tumor tissue, as well as particle therapy methods in which tumor tissue is bombarded with particle radiation (for example accelerated protons, carbanions, etc.).
The machine settings of the irradiation device - eg irradiation direction, position, field size, etc. - are usually set relative to a given patient isocenter in the patient's body which forms the origin of a patient coordinate system. This point can for example, in the middle of a tumor volume visible on the three-dimensional image data.
It is now customary to project lines on the skin of the patient with the aid of electronically controlled laser beams, which mark the position of this point. These lines are drawn on the skin using color markers and must remain visible until the next examination date or on the date of the irradiation. If necessary, the patient must be ordered to trace the lines.
The invention has for its object to provide an imaging device and a therapy system that simplifies the implementation of a therapy, in particular radiotherapy.
This object is achieved according to the invention with respect to an imaging device, as it is used for a therapy planning by the features of
The imaging device comprises a table for supporting a patient (hereinafter referred to as an imaging table) and an image acquisition unit for generating digital image data that images a region of the interior of the patient. According to the invention, it is provided here that the image acquisition unit does not scale the coordinates of the recorded image data (only) with respect to a patient coordinate system, but (at least) with respect to a spatial coordinate system with respect to which the imaging table is fixedly mounted. Additionally or alternatively, it is provided according to the invention that the coordinates of the image data are spatially scaled in relation to a table coordinate system that is stationary relative to a patient support, ie the "tabletop" of the imaging table, is defined.
The term "scaling" is used literally in the sense of "provided with a (spatial) scale". In the course of the "scaling" the image data set is thus assigned a location or location information that assigns each pixel of the image data set a point within the room or table coordinate system, and thus a room point. The scaling thus relates to the coordinates of the pixels within the image data set. The scaling, on the other hand, does not involve a change in size, i. the number of pixels, the image data set.
The term "spatial coordinate system" according to the above definition comprises any coordinate system in which the imaging table is mounted in a fixed position, that is, in a constant spatial relationship with at least one attachment point of the imaging table. Preferably, the spatial coordinate system is also defined in terms of location with respect to the surrounding imaging space, and the imaging table is mounted correspondingly stationary relative to the surrounding imaging space. However, in the sense of a kinematic reversal, a mobility of the imaging table relative to the surrounding imaging space may also be provided, in which case the spatial coordinate system is moved with the imaging table.
The spatial coordinate system and the table coordinate system form substantially equivalent or redundant reference systems for describing the location of a spatial point in the imaging space, one or both of which may be explicitly defined within the scope of the invention. The table coordinate system and the spatial coordinate system are - at least as long as the (optional removable) patient support is connected to the imaging table - in a defined spatial relationship and are interconvertible.
By scaling the coordinates of the image data with respect to the spatial coordinate system or the table coordinate system, it is achieved that the image data generated during a measurement have an absolute reference to a spatial region, which in turn is in a defined spatial relationship with the imaging device. As a result of this defined spatial relationship, recordings can be reproduced identically or substantially identically in a particularly simple and precise manner. In particular, the position of a body region to be treated can thereby be characterized independently of markings on or in the body of the patient and the invasive introduction of implanted markings (so-called BBs) in the vicinity of the tumor and the associated risks for the patient avoided.
Preferably, the imaging device is designed as an X-ray system, in particular computed tomography. Alternatively, the imaging device may also be based on another imaging technique, in particular MR, PET or SPECT. In particular, the digital image data contain three-dimensional image information, ie form a 3D image data record or volume data record. The invention is optionally applied to two-dimensional image data.
The patient support of the imaging table is intended for immobilized and substantially reproducible recording of the patient. The patient support is designed in particular in the form of a couch which is used to immobilize the patient e.g. equipped with a customized to the patient foamed plastic shell or other adjustable patient fixation organs. Alternatively, the patient support may also be designed in the manner of a chair, so that the patient is stored in a sitting position.
The patient support is preferably reversibly removable from a fixed mounted tripod of the imaging table.
In order to create reproducible conditions with regard to the placement of the patient support on the stand, it is preferably provided that the patient support can be fastened on the stand only in a defined position.
The stand is in particular adjustable, so that the patient support can be offset in particular in height, but optionally also horizontally relative to the spatial coordinate system. The tripod is this either manually adjustable or designed as a robot arm.
The image recording unit of the imaging device is preferably displaceable relative to the imaging table, and thus relative to the spatial coordinate system, movable, in particular on rails or the like in a defined direction of movement. By calibrating a motion control of the image acquisition unit, by sensors or otherwise, the position of the image acquisition unit in the spatial coordinate system is detected and used as input for scaling the coordinates of the image data using stored information about the geometry of the image acquisition unit, in particular during image acquisition.
The therapy system according to the invention comprises, in addition to the imaging device described above, a therapy device, in particular an irradiation unit for radiation treatment of the patient with high-energy electromagnetic radiation or accelerated particles includes. The therapy device is associated with a further spatial coordinate system in which a table for supporting the patient (hereinafter referred to as therapy table) is arranged stationary. Additionally or alternatively, the therapy device is assigned a further table coordinate system, which is arranged stationarily with respect to a patient support of the therapy table.
The imaging device and the therapy device are in this case matched by appropriate calibration or adjustment to one another such that on the basis of the scaling, i. the location information of the coordinates of the image data recorded by the imaging device and a stored conversion rule a corresponding spatial area in the space or table coordinate system of the therapy device can be determined. In other words, the conversion rule is used to transmit the volume of space of the imaging space depicted in the image data to a corresponding volume of space of the therapy room. Thus, with the help of the conversion rule, it can be determined from the scaled image data at which spatial point in the environment of the therapy device a body region of the patient, in particular a tumor, imaged in the image data is arranged, when the patient is stored on the therapy table. The scaled image data can be used directly for therapy planning.
Tischkoordinatensystem wieder. In a particularly preferred embodiment of the invention, the tables of the imaging and therapy device are of a similar construction and set and arranged the same with respect to the respective spatial coordinate system. In this embodiment, the conversion rule is reduced to a factor of 1, i. The position information of the image data identically reflects the position of the imaged body region of the patient stored in the therapy device in the associated room or table coordinate system.
Are the tables of the imaging or therapy device constructed or set up differently or with respect to the respective Space coordinate system arranged differently, so the mediating between the two space coordinate systems conversion rule of 1 is different and based on the local conditions in each case to determine. If, for example, the table height of the therapy table is twice the table height of the imaging tables, then the scaling of the coordinates of the image data is corrected by the conversion rule with respect to the height by a factor of 2, etc.
In particular, the therapy table has a removable patient support and a mechanical interface for docking a patient support, the patient support and interface being configured to be compatible with the corresponding parts of the imaging table. The similar design of the two tables allows an exchange of patient support between the imaging device and the therapy device, and thus in particular a transport of the patient in an immobilized state.

The
In a CT examination, the
In the
A third coordinate system, hereinafter referred to as patient coordinate
In the context of the invention, the coordinates of the image data recorded by means of the
For this purpose, the traversing position of the
The thus scaled image data thus contain the information about the absolute position of the patient isocenter with respect to the space coordinate
Additionally or alternatively, the coordinates of the 3D image data are scaled with respect to the table coordinate
By scaling the coordinates of the 3D image data in units of the table coordinate
The
The tables 10 and 22 are mechanically similar. In particular, they are also equipped with similar patient supports 14 or can exchange the same
In the course of a radiation therapy session, the
In a subsequent step, the
Therapieraumes.By knowing the geometric structure of the tables 10 and 22, their arrangement in the respective spatial coordinate
If the position of the patient isocenter in the spatial coordinate
The 3D image data recorded during the examination by the
