Abstract
Description
The invention relates to a medullary boring head according to the preamble of patent claim 1.
In osteosynthetic nailing of fractured long bones, the medullary cavity must be drilled out before the intramedullary nail is implanted. For this purpose, a drill mandrel is first passed through the medullary segment of the individual bone fragments, over which a hollow, flexible drill shaft with a fixed drill head is then advanced. The drilling shaft is driven by a motor, mostly by means of a pneumatic drilling machine. Then a flexible shaft with interchangeable medullary boring heads with different outer diameters is used for the final drilling of the medullary canal. The marrow boring heads can be attached to or detached from the flexible shaft in a rotationally fixed manner by means of a simple coupling.
Since the medullary boring heads according to the prior art - apart from the continuous channel for the drilling mandrel, which is itself closed by this - are solid, a considerable metaphyseal and radial pressure is created during the boring process generated within the medullary canal of the distal fracture fragment. Peak values for pressure up to 1500 mm Hg can occur.
The enormous increase in pressure necessitates an intrusion of marrow fat into the transcortical vessels, which has a disadvantageous effect on fracture healing, because the function of the transcortical vessels is affected not only by the drilling process itself but also by the laying with marrow fat. In addition, there is pressure-induced fat flushing into the laxative venous system, which can lead to fat embolism.
The temperature rise during the drilling process - with peak values of up to 50 ° C - is also undesirable.
Finally, the drill head is often jammed in the medullary canal, which must then be knocked out with suitable instruments.
Hier will die Erfindung Abhilfe schaffen. The invention seeks to remedy this. The invention has for its object to provide a medullary boring head which allows the hydraulic pressure to be dissipated in the distal fracture fragment during the drilling of the medullary canal.
The invention solves the problem with a medullary boring head, which has the features of claim 1. Further advantageous embodiments of the invention are characterized in the dependent claims.
The advantages achieved by the invention are essentially to be seen in the fact that, thanks to the medullary boring head according to the invention, the pressure which occurs during the boring process in front of the boring head can be discharged through the openings provided for this purpose through the hollow boring head.
The invention is explained in more detail below with the aid of a representation of a preferred exemplary embodiment.
