Description

Ureter rapid liquid-gas expansion pipeline kit and use method thereof
Technical Field
The invention relates to the technical field of ureter soft lens operation treatment, in particular to a ureter rapid liquid-gas expansion pipeline kit and a using method thereof.
Background
Compared with a ureter hard lens, the ureter soft lens has a bendable function, can treat ureteral calculus which goes up to a kidney along a ureter in a calculus crushing process, and ureter upper segment calculus, and is more and more subjected to ureter soft lens operation treatment. The percutaneous nephrolithotomy is the absolute mainstream of nephrolithotomy in kidney stones with indications of surgical lithotomy for a long time before the development of ureter soft lens technology and technology. Compared with the percutaneous nephroscope lithotripsy, the ureter soft-lens lithotripsy does not need renal puncture, so the risk of renal hemorrhage caused by puncture expansion and pulling the lens body during lithotripsy is avoided. With the promotion of the technology of the ureter soft lens and the popularization of the technology, more and more kidney stones and ureter upper segment stones which are not loaded too much and have operation indication are treated by the ureter soft lens in recent years. The percutaneous nephrolithotomy combined with the ureter soft lens is also a research hotspot. For kidney stones with high hardness and unsuccessful extracorporeal shock wave lithotripsy, ureteroscopy became the first choice.
Although the ureter soft lens lithotripsy has a plurality of advantages compared with ureter hard lens lithotripsy and percutaneous nephroscope lithotripsy, the ureter soft lens lithotripsy also has limitations, wherein important points are that: the inner diameter of the ureter lumen is relatively small, limiting the outer diameter of the soft lens and the diameter of the operating (water flow) channel. The consequence is: 1) in the first stage of the ureter soft lens lithotripsy, the ureter soft lens sheath cannot be successfully placed into a relatively narrow ureter cavity, and the ureter soft lens sheath cannot be placed into the ureter soft lens sheath, so that the soft lens lithotripsy operation cannot be completed directly. Even if the units and experts with abundant operation experience break the ureter soft lens at the first stage, the failure rate of the tube placement is about 10 percent or even higher. 2) The narrow inner diameter of the ureter improves the process requirement of the flexible ureteroscope, so that the flexible ureteroscope is high in price and easy to damage, the cost of the technology is increased as a direct result, and the development of the technology is limited. 3) The flexible ureteroscope operation channel is narrow and small, and the passageway is occupied by rubble optic fibre in the rubble process, and various consequences that the rivers play unsmooth and lead to: kidney damage caused by increased pressure, retrograde infection, accidental injury caused by unclear visual field, reduced lithotripsy efficiency, prolonged operation time, limited total operation time and the like. The method is adopted at present, but the operation needs 2 times of operations, and the interval time between the two operations is about one week or even longer. The ureter internal bracket is carried, which has obvious influence on the life quality of patients: discomfort in the waist, hematuria, frequent urination, lumbago during urination, etc. Therefore, a plurality of units and experts with rich operation experience of the ureter soft lens refuse to place the internal support before the ureter soft lens lithotripsy. In any case, the effect of placing the ureteral stent in advance to expand the ureter is relatively limited. Whether a bracket is placed in a ureter for expansion before the ureter soft lens lithotripsy operation or not is an ongoing topic in the urinary academia in recent years.
In fact, ureters are flexible, water-expandable muscular tubes with a greater passive expansion capacity, which can be understood from the anatomy of the ureter. Compared with the mechanical force expansion of the ureter, water and gas expand the ureter with softer and more uniform force. As a urologist, expansion of the ureter by water, increase of the internal diameter is very easy to understand: when ureteral calculus obstructs ureter, the part above obstruction plane of kidney and ureter is expanded by urine to different extent, even the diameter of ureter inner diameter exceeds 20 mm. Moreover, this expansion is mostly reversible, without significant impairment of the ureteral anatomy and function: after the acute ureteral obstruction is relieved for a period of time, B-ultrasonic examination or radiography examination is carried out again, the ureter which is expanded originally is not expanded any more, the renal effusion disappears, and the renal function is recovered to be normal. However, we can understand from the following experience: at the initial stage of relieving the acute ureteral obstruction, the ureter still keeps an expanded state: patients with ureteral calculus and renal colic receive ureteroscope calculus, only one spacious ureter is found, no ureteral calculus is found, and the calculus is discharged. When the calculus is discharged, a spacious ureter can still be obtained.
The Panyu central hospital Liqiang physician filed an invention patent application of 'a double-balloon catheter kit for ureter expansion' (application number: 201810563731.5) to the national patent office on 6/4/2018, which is currently in the stage of substantial examination. This patent discloses the use of fluid or gas pressure to dilate the ureter with a mild and controlled measurable amount of force, with no trauma and bleeding. After the ureter is expanded by the tool and the method, a thicker soft lens sheath can be conveniently and safely placed, so that the operation success rate of the ureter soft lens is increased, the process requirement and the price of the ureter soft lens are reduced, the performance of the ureter soft lens is improved, the diameter of a water flow channel is increased, and the safety and the effect of the ureter soft lens are quickly improved.
However, the device is a double-balloon four-channel device, the structure is complicated, the design and manufacturing process of the device can be increased, and the problem of low success rate of inserting the external member into the ureter caused by over-thick diameter can also occur. Therefore, it is desirable to provide a ureteral quick liquid-gas expansion conduit set and a use method thereof to solve the above problems.

When the first balloon 15, the second balloon 16, the third balloon 27 and the fourth balloon 28 are expanded in the ureter, the annular bulge 40 can be completely attached to the inner wall of the ureter to serve as a sealing ring, so that the sealing performance of the ureter is improved, and liquid or air is prevented from permeating into the ureter in the process of injecting air or liquid into the blocked ureter.
Compared with the prior art, the invention pre-expands the ureter through the first ureteral catheter, because the first ureteral catheter removes the renal pelvis drainage channel, which is not suitable for a longer time remaining in the ureters, otherwise problems arise with increased pressure in the working side kidneys, so that the first ureteral catheter is only ready for a short expansion, the ureter is primarily expanded, after the diameter of the ureter is increased, a second ureter catheter can be conveniently placed into the ureter for long-time expansion, the expansion time is several hours or ten hours or more, the expansion effect and safety of the ureter are greatly improved, the placement success rate of the second ureter catheter is improved, the operation success rate of a flexible ureteroscope is increased, the technological requirement and price of the flexible ureteroscope are reduced, the performance of the flexible ureteroscope is improved, the diameter of a water flow channel is increased, and the safety and the effect of the flexible ureteroscope are quickly improved.
The safety problem of the invention with the use of liquid gas to expand the ureters is briefly described as follows:
urologists are all aware of: when a calculus is blocked at the lower section of a ureter, urine generated by a kidney cannot flow out, the urine with certain pressure is gathered in the ureter and a kidney collecting system, and at the moment, the whole ureter, a renal pelvis and the kidney collecting system (a place for collecting and discharging the urine in the kidney) are expanded, so that the most obvious upper section of the ureter is expanded, but the lower section is also well expanded, but the upper section is well expanded, interrupted and slightly expanded or not expanded; or the bulging expansion of a certain ureter section and a certain ureter point;
the pressure of liquid used for dilating the ureter can be accurately controlled, and can be fixed at the intrarenal pressure when the kidney cannot discharge urine when the ureter is obstructed by stones, or can be set at about the pressure; rather than a rapid, uncontrolled total water injection, and the amount of pressure in the ureter that is measured when the water fills the ureter, and the diameter of the ureter at various locations.
The above-mentioned embodiments only express several embodiments of the present invention, and the description thereof is more specific and detailed, but not construed as limiting the scope of the invention. It should be noted that, for a person skilled in the art, several variations and modifications can be made without departing from the inventive concept, which falls within the scope of the present invention.
