Abstract
本发明涉及一种小儿过敏性皮炎专用型临床营养配方,其包括:蛋白质、脂肪、膳食纤维、新资源食品、药食同源成分、葡萄糖酸镁、钙、铁、锌、维生素A、维生素D3、维生素E、维生素B1、维生素B2、维生素B6、维生素B12、维生素C、泛酸、烟酸、叶酸、葡萄籽提取物、海带粉。本发明基于小儿过敏性皮炎患者的营养摄入,通过选用合理的基础营养素,同时添加具有保护肠道功能和营养辅助支持的特殊营养素,以改善小儿过敏性皮炎患者营养结构,对于过敏性皮炎具有较好的治疗效果且其复发性较低,副反应较小,可辅助治疗小儿过敏性皮炎。

技术领域
本发明涉及特殊医学用途配方食品领域,具体涉及一种小儿过敏性皮炎专用型临床营养配方及其制备方法。
背景技术
过敏症是身体的免疫系统对外来物质不适应的反应,其实对身体不算是一种伤害。免疫系统是一种高度复杂的防御机制,可以帮组人体对抗感染。免疫系统会先辨认外来的入侵物质,之后会驱动体内的白血球细胞去攻击外来物质。对于某些人而言,其免疫系统会将一些无毒物质错认为入侵物,同时白血球细胞反应过度,对身体引起的伤害会比入侵物造成的伤害更多。因此,过敏反应本身形成一种疾病。
典型的过敏反应会出现鼻充血、咳嗽、哮喘、发痒、呼吸急促、头痛、疲劳和荨麻疹以及其他的皮疹。会引起过敏反应的物质称为过敏原。几乎任何物质都能引起世界上某些地方的某些人的过敏反应。
近年来小儿过敏性疾病如哮喘、过敏性鼻炎、湿疹、过敏性皮炎等的发病率有逐年增高的趋势,成为危害小儿身体健康的主要疾病之一。20种常见的小儿过敏原为花生、蟑螂、螨虫组合、螃蟹、海洋鱼组合、淡水鱼组合、蛋清、树木、点青霉、艾蒿、猫、牛奶、虾、狗、屋尘、大豆、牛肉、羊肉、普通豚草、啤酒花。发生轻度过敏性皮炎的患儿可能出现局部充血、瘙痒,边缘部位可发现清除的淡血斑;重度过敏性皮炎患儿则会在红斑的基础上发生水疤、大疤、丘疹、渗出、糜烂等,患儿会出现瘙痒难耐及疼痛感,但儿童出现上述症状后,由于表达不清或者家长未注意而导致患儿过度抓挠过敏部位,引起感染。儿童发生过敏性皮炎疾病与患儿自身体质有关,也与外界因素密切相关。
目前用于治疗小儿过敏性皮炎的措施主要以中药制剂、药膏、沐浴液为主,例如中国专利CN105521117A公开一种采用组分川椒的水浸取物治疗过敏性皮炎的药方;中国专利CN108686040A公开一种皮炎用药膏,该药膏包括以下原料:雄黄、白芷、密陀僧、五倍子、土荆皮、冰片、山茶油、薄荷叶、凡士林;中国专利CN107854396A提供一种预防皮炎身体乳,其由下列重量份原料制成:金银花提取液5-8份、白茯苓提取液4-6份、白芨-丹参提取液3-9份、甘油硬脂酸酯0.5-1.8份、牛油果树果油0.8-1.5份、橄榄油4-7.5份、卡波姆0.1-0.4份、水20-40份。但上述各专利并未考虑从机体的饮食结构进行调整改善,目前也没有任何关于小儿过敏性皮炎的肠内营养制剂产品的报道。
发明内容
本发明的目的在于克服现有技术中的缺陷,针对小儿过敏性皮炎患者饮食结构不合理、营养不均匀等不良现状,主要是通过膳食调控,合理搭配碳水化合物、蛋白质、脂肪的供给能量比值,选择多种维生素、矿物质、药食同源和新资源食品,来改善类小儿过敏性皮炎患者营养结构,辅助改善小儿过敏性皮炎患者的机体健康状况。
为实现上述目的,本发明采用如下技术方案:

为了进一步优化上述临床营养配方,本发明所采取的技术措施还包括:
进一步地,所述蛋白质选自水解乳清蛋白粉、浓缩乳清蛋白粉、大豆分离蛋白、全蛋粉、牛初乳粉、全脂奶粉、白蛋白肽、大豆肽、海洋鱼低聚肽、小麦低聚肽、玉米低聚肽粉、α-乳清蛋白粉、鱼胶原蛋白肽粉、氨基酸中的至少一种。更优选为:水解乳清蛋白粉和鱼胶原蛋白肽粉。上述蛋白质均优选优质蛋白,本发明通过补充适量高生物价优质蛋白质,全面均衡地补充人体所需的8种必须氨基酸。
进一步地,所述氨基酸选自L-谷氨酰胺、甘氨酸、赖氨酸、丝氨酸、精氨酸、丙氨酸、蛋氨酸、色氨酸、亮氨酸、异亮氨酸、缬氨酸、组氨酸、酪氨酸、苯丙氨酸、瓜氨酸、鸟氨酸、天冬氨酸、苏氨酸、谷氨酸、胱氨酸中的至少一种。氨基酸是组成蛋白质的基本单位,根据人体合成速度和合成能力,分为必需氨基酸、非必需氨基酸和条件必须氨基酸。谷氨酰胺合成DNA的基质之一,在合成蛋白质扮演重要角色,是唯一一种可直接通过脑血管障壁的氨基酸,为快速分裂的免疫细胞的前导物质,具有增强免疫力的作用。甘氨酸有防治组织损伤,提高抗氧化能力,提高机体免疫力,促进蛋白质合成,治疗代谢紊乱的糖尿病、缺血再灌注损伤、癌症及各种炎性疾病的作用。赖氨酸提高食物蛋白质的吸收和利用,达到均衡营养、促进生长、提高记忆力、帮助机体产生抗体、激素和酶,提高机体免疫力、增加血色素、预防心脑血管疾病、促进骨及软骨组织的生长、治疗骨质疏松等作用。丝氨酸在脂肪和脂肪酸的新陈代谢及肌肉的生长中发挥作用,并且有助于免疫血球素和抗体的产生,因此丝氨酸在免疫系统方面扮演者重要的角色。精氨酸主要是通过NO途径以及调节机体内分泌激素来发挥免疫调节作用,促进免疫球蛋白产生,在机体免疫、肠黏膜功能保护等方面起重要作用。丙氨酸主要是参与组织蛋白的合成、脱氨基生成丙酮酸和谷氨酸、脱羧基生成丙酮酸。在疾病发展过程中能量对免疫系统相对于疾病的反应是非常重要的,因此机体组织从血液中大量吸收丙氨酸来促进糖代谢为机体供能,以此来加强自身免疫。蛋氨酸有助于人体内各种含硫化合物的代谢并且其所含的甲基对有毒物或药物进行甲基化而起到解毒的作用。组氨酸降低胃液酸度,缓和胃肠手术的疼痛,减轻妊娠期呕吐及胃部灼热感,抑制由植物神经紧张而引起的消化道溃烂,对过敏性疾病,如哮喘等也有功效;色氨酸可以预防糙皮病、抑郁症,改善睡眠和调节情绪。蛋白质中的三种常见氨基酸,即亮氨酸、异亮氨酸和缬氨酸被统称支链氨基酸。这类氨基酸通过释放胰岛素和生长激素来促进机体合成代谢,此外还有抗疲劳的作用。亮氨酸能够修复肌肉,控制血糖,并给身体组织提供能量,必需氨基酸苯丙氨酸可以抑制癌瘤生长,又可以降低药物的毒副作用。

进一步地,所述新资源食品选自DHA藻油、花生四烯酸油脂、γ-亚麻酸油脂、牡丹籽油、茶叶籽油、翅果油、御米油、共轭亚油酸、杜仲籽油、低聚甘露糖、小麦低聚肽、壳聚糖、人参(人工种植)、玉米黄质、水解蛋黄粉、圆苞车前子壳粉中的至少一种。更优选为:圆苞车前子壳粉。其中的油脂类物质以微囊粉的形式添加。壳聚糖属高聚阳离子,其活性基团带正电荷,能将细菌吸附粘连在一起,阻止其扩散,吸附细菌后,破坏细菌的细胞膜,致使细胞质内的DNA和RNA流出,从而达到杀灭细菌的效果。阳离子同时还能与细菌表面产生的酸性物质如脂多糖、磷壁质酸、糖醛酸磷壁质、荚膜多糖等相互作用,形成复杂的高分子电解质,从而使细菌的细胞膜功能发生紊乱、死亡。圆苞车前子壳富含半纤维素,这类纤维不能被人体消化,进入肠道可以促进益生菌生长,从而减少有害菌的数量;圆苞车前子壳可吸水膨胀50-100倍,形成果冻状的黏稠物质,促进肠道正常排空;圆苞车前子壳含丰富的膳食纤维,并且主要是水溶性膳食纤维,其不会增加热量摄入。

上述营养配方中,矿物质是人体所必须的重要营养素,很多矿物质是免疫系统的重要组成成分,并且某些矿物质还对哮喘等特应性疾病有不同程度的影响。
硒是抗氧化剂催化酶的辅助因子。它能减少氢过氧化物和其他有机过氧化物。因此在保护细胞免受氧化损伤中发挥关键性的作用。在成人研究中,哮喘患者血清和饮食摄入的硒水平比对照组低。也有研究表明,哮喘患者血清硒水平及谷胱甘肽过氧化物酶活性较低。另外,Shaheen等发现硒摄入可减轻成人哮喘;且在有关硒的多数研究(尤其是在吸烟人群)中,也能得出高水平硒与较好的肺功能相关的结论。
镁通过松弛支气管平滑肌在治疗和预防哮喘方面发挥着重要的作用。在儿童研究中发现,镁补充可提高儿童应用支气管扩张药的效果。
锌是免疫系统许多组分所必须的,包括吞噬细胞和自然杀伤细胞功能,细胞免疫和体液免疫。锌有收敛抑菌,能使粘膜创面形成薄膜,具有保护作用。还有研究显示,持续补充生理剂量的锌约1-2个月可以修复免疫反应,并且可以减少感染事件的发生。Soutar等提出饮食中锌摄入太低会使人患特应性疾病、支气管高反应和变态反应症状的危险性增大。此外,肺组织通过不同的抗氧化机制保护自身免受损伤,而锌则可以通过某些机制扮演抗氧化剂的角色。
维生素D是免疫系统一个选择性调节剂,它对免疫细胞的作用会因暴露时间的不同而有所不同。1,25(OH)2D3是体内维生素D的活性代谢产物,国内外研究证实1,25(OH)2D3除调节体内钙磷代谢外,对免疫系统也有广泛的作用。
维生素E能增加对感染的抵抗力。对维持正常的免疫能力,特别是对T淋巴细胞的功能很重要。
在母乳喂养的婴儿中,母乳中较高浓度的维生素C与其婴儿12个月时特应性皮炎和抗原过敏性疾病患病危险性降低有关。
葡萄籽提取物是从葡萄籽中提取的一种多酚类物质,其多酚含量在95%以上,包含单体酚和聚合多酚,单体酚主要是儿茶素、表儿茶素、没食子酸及儿茶素、表儿茶素的没食子酸酯,聚合多酚由单体酚通过C4~C6或C4~C8键连接形成的不同聚合度的原花青素,聚合度越低,抗氧化性越强。葡萄籽提取物具有抗氧化、自由基捕获、金属离子螯合、蛋白质结合等生物活性,其中最突出的生物活性是抗氧化性。葡萄籽提取物抗氧化作用机制至少有三种:a.通过酚羟基与自由基进行脱氢反应生成稳定的半醌自由基,从而中断链式反应;b.通过葡萄籽提取物还原力作用直接给出电子而清除自由基;c.螯合启动脂质过氧化的金属离子,从而切断自由基的链式反应,发挥抗氧化作用。葡萄籽提取物价格低廉,且具有良好的抗氧化活性,无毒副作用,是天然抗氧化剂中有发展潜力的新型高效抗氧化剂。
本发明的第二个目的是提供一种上述小儿过敏性皮炎专用型临床营养配方的制备方法方法,其包括如下步骤:称取预定量的各原料,其中维生素、复合矿物质、药食同源成分、脂肪和新资源食品预先制备成预混料;将预混料和2%麦芽糊精先混合5~15分钟;将其它原料按照逐级递增的原则加入进行混合,混合10~30分钟,出料,分装。
进一步地,上述各原料均为粒径为50~200μm的粉末形式的原料,配置前及配置完成后均经过杀菌处理。
进一步地,所述营养配方的剂型包括营养粉、营养乳剂、胶囊剂、片剂、丸剂、口服液等。

本发明所述的营养配方作为小儿过敏性皮炎的肠内营养制剂,通过饮食治疗作为缓解患者症状的一种辅助措施。根据我国传统中医药的理论,通过以证论治的食疗基本原则,结合患者体质特征,使用一种或多种药食同源的中药材、新资源食品、可用于食品的益生菌、氨基酸、短肽类、不饱和脂肪酸、水溶性膳食纤维、具有抗氧化功能的多种维生素和多种矿物质,其为全营养或非全营养配方食品,具有预防和缓解小儿过敏性皮炎症状的功效。

应用实施例
本实施例采用实施例1~4制备的营养粉进行治疗小儿过敏性皮炎临床试验研究,其选取100名过敏性皮炎患者,男28例,女22例,平均年龄6岁,已排除器质性病变、外伤等非过敏性病例。随机平均分为五组,分别为对照组、实施例1组、实施例2组、实施例3组和实施例4组。对照组给予氢化可的松软膏,按药品说明书使用,7天为1个疗程。实施例1组、实施例2组、实施例3组和实施例4组分别给予本发明实施例1~4所制得的营养粉,用量为一天2次,1次100mg。
疗效标准:无效-患者症状完全没有改善,且有加重迹象;显效-患者症状有显著改善,但是仍未完全消失;有效-患者症状完全消失。副反应:瘙痒、红肿、继发感染、过敏反应等常见副反应。实验结果如下表所示:

由上可知,与对照组相比,本发明所述的营养配方对于过敏性皮炎的治疗效果与氢化可的松软膏相当,甚至优于氢化可的松软膏,证明其能够有效预防和辅助治疗过敏性皮炎,且其复发性较低,副反应较小。这说明本发明制备的营养配方,可作为小儿过敏性皮炎的日常营养均衡制剂,且其能有效缓解药物治疗引起的消化道的不良反应,辅助治疗小儿过敏性皮炎。
本发明通过选用合理的基础营养素,基于小儿过敏性皮炎患者的营养摄入,对各组分的配比进行适应性调整,同时添加具有保护肠道功能和营养辅助支持的特殊营养素,以实现营养均衡,改善小儿过敏性皮炎患者营养结构,改善其炎症反应及机体健康状况。

8.根据权利要求1所述的一种小儿过敏性皮炎专用型临床营养配方,其特征在于,所述营养配方还包括益生菌1~5份,所述益生菌选自双歧杆菌属、乳杆菌属、链球菌属、嗜酸乳杆菌NCFM、动物双歧杆菌Bb-12、乳双歧杆菌HN019、乳双歧杆菌Bi-07、鼠李糖乳杆菌LGG、鼠李糖乳杆菌HN001、发酵乳杆菌CECT5716、短双歧杆菌M-16V中的至少一种。
9.根据权利要求1所述的一种小儿过敏性皮炎专用型临床营养配方,其特征在于,所述营养配方还包括硒0.003~0.05份,所述硒来源于亚硒酸钠、富硒食用菌粉、富硒酵母中的至少一种。
10.一种如权利要求1~9中任一项所述的小儿过敏性皮炎专用型临床营养配方的制备方法,其特征在于,包括如下步骤:称取预定量的各原料,其中维生素、复合矿物质、药食同源成分、脂肪和新资源食品预先制备成预混料;将预混料和2%麦芽糊精先混合5~15分钟;将其它原料按照逐级递增的原则加入进行混合,混合10~30分钟,出料,分装。
