背景技术
当前的新冠肺炎大流行凸显了呼吸系统受损患者对机械式换气系统的需求。呼吸治疗装置可用于在受试者的呼吸周期中在适当时间以治疗压力向患者供应清洁的可呼吸气体(通常是空气,有或没有补充氧气)。治疗压力辅助可以以与患者的呼吸同步的方式实施,以便在患者的正常呼吸的吸气周期期间允许更大的压力并且在呼气期间允许更低的压力。也可以实施治疗压力辅助以覆盖患者的正常呼吸的吸气周期。
呼吸护理系统通常包括气体或空气流发生器或压缩气体或空气源、空气过滤器、鼻腔、口腔或全脸面罩、将气流发生器连接到面罩的空气输送导管、各种传感器和基于微处理器的控制器。可选地,代替面罩,气管切开管也可以用作患者接口。气流发生器可以包括伺服控制电机和形成吹风机的叶轮。在一些情况下,可以实施用于吹风机电机的制动器以更快地降低吹风机的速度,从而克服电机和叶轮的惯性。制动可以允许吹风机不管惯性及时地更快速实现较低的压力条件,用于与患者的呼气同步。在一些情况下,气流发生器还可以包括能够配置为将产生的空气排放到大气中的阀,作为改变输送给患者的压力的装置,其作为电机速度控制的替代方案。除其它外,传感器测量电机速度、质量流速和出口压力,例如使用压力传感器等。该装置可选地可以包括在空气输送回路的路径中的加湿器和/或加热器元件。控制器可以包括具有或不具有集成的数据检索和显示功能的数据存储容量。
呼吸护理系统可用于治疗许多疾病,例如由于肺、神经肌肉或肌肉骨骼疾病以及呼吸控制疾病引起的呼吸功能不全或衰竭。它们也可用于与睡眠呼吸紊乱(SDB)(包括轻度阻塞性睡眠呼吸暂停(OSA))、过敏诱导的上呼吸道阻塞或上呼吸道早期病毒感染有关的疾病。
当前的新冠肺炎大流行已经使当前呼吸护理系统的供应紧张。医院被迫共享呼吸护理系统,即两名患者之间的呼吸机。医院也采取了将传统用于阻塞性睡眠呼吸暂停的装置调整为传统呼吸机的糟糕代替。
此外,目前的呼吸机是复杂且昂贵的设备,需要不断的监督和调整,而且容易出现故障。
发明内容
本公开提供了一种简单的低成本呼吸机,其克服了现有技术呼吸机的上述和其他缺点。
更具体地,本公开提供了一种呼吸机,该呼吸机在成本、尺寸减小、重量减小、功率减小、噪声减小和可靠性方面比当前的呼吸机具有显著优势。本公开的该呼吸机的一个关键是独特的空气或气流阀,其具有结合到阀中的空气或气体储存器或收集器。将空气或气体储存器或收集器结合到该阀中简化了系统的构造和成本,同时提供了改进的响应时间,从而提供了更好的患者支持。传统的呼吸机采用比例电磁阀(PSOL阀)或基于涡轮机的设计,其中核心流量/压力调节部件是一种高成本的多部件产品(订单1500美元-2000美元)。此外,在实践中,传统PSOL阀的柱塞的导柱上的静摩擦可能损害阀的灵敏度,这反过来可能导致迟滞效应。为了克服传统呼吸机的上述和其他缺点,本发明采用了一种新颖的低成本空气或气体阀,其具有结合到该阀中的一体式空气或气体储存器或收集器,并且该阀基本上由五个主要元件和基本上一个移动部件组成。

在本发明的另一个实施方式中,换气系统包括用于调节空气或气体的加热器和/或加湿器。
阀可以响应于患者的正常呼吸周期而打开和关闭,或者阀可以打开和关闭来引入空气或气体流以覆盖患者的正常呼吸周期。

具体实施方式
在以下详细描述中,术语“空气”和“气体”以及术语“呼吸器”和“呼吸机”分别可互换使用。
本发明的呼吸治疗装置基于患者的自然潮式呼吸周期或基于程序化的呼吸周期以间歇时间间隔向患者提供补充空气或氧气。
参考图1,呼吸机系统10包括连接到加压气体源14的换气控制器12。加压气体源可以是加压空气或空气/氧气罐,如图所示的压缩机或空气泵,或加压空气管路。下面将详细描述的换气控制器12允许加压气体通过在16处的气体供应管路流到患者,该气体供应管路固定到患者接口,例如患者22佩戴的鼻罩或全面罩18。或者,患者接口18可以包括插管或气管切开插管。完成该系统的是感测和测量来自患者的吸气和/或呼气气流的二氧化碳描记监测器24,以及命令输入和监测器26。二氧化碳描记监测器24以及命令输入和监视器26是常规的,不需要进一步描述以理解本发明。

现在参考图3-5,气体或气流控制阀28包括阀壳体40,其包含气体或空气流控制阀28的有源元件。气体供应入口42显示在负X轴面上,而气体源出口44显示在正X面上。另外,壳体40形成了气体储存器或收集器46。气体供应入口42可以与标准的医院O2源或任何气体源(例如气罐或压缩机)接合。
下面参考图3和图4描述的阀闸48根据其沿X轴的位置控制源流量,Q源(t)。负Z表面上的面沿着X轴在阀壳体滑动表面50上滑动。阀闸的正X轴上的阀闸面和阀壳体密封表面52之间的距离δ通过在阀壳体密封表面和在正X轴上的阀闸Y-Z面之间形成阻力通道来确定流动阻力。

弹簧柱塞58在负Z方向上向阀闸48提供预加载力。其目的是持续地保持阀闸48和阀壳体滑动表面50之间的气密密封。
垫圈60保持阀壳体的X-Z表面与阀闸48之间的气密密封。
再次参考图2,换气控制器12包括连接到气体或气流控制阀28的空气或气体输入端口30。控制阀28具有连接到端口的出口32,该端口包括吸气流连接34和呼气流端口36,呼气流端口36又连接到呼气流阀38。呼气流阀38可以通向大气中,或者被连接以除去CO2并通过气体输入端口30再循环。该系统还包括用于感测患者呼吸的呼气流量传感器或呼吸传感器40,以及用于触发阀28的来自传感器的连接。传感器可以包括空气流量传感器、温度传感器、声音传感器、CO2传感器或用于检测患者胸部运动的运动或应变传感器。

还参考图7,阀组件28通过改变流动阻力R阀(δ)来控制源气体流动。这是通过改变致动器54的长度△X来实现的,这又导致在阀闸48和阀壳体密封表面50之间的相应间隙中沿着X轴将阀闸48移动δ。储存器压力P储存器(t)被压力传感器70监测和利用,以计算控制如方程3所示的Q源(t)的所需△X命令。
气体流经气体供应入口42中内联的流量传感器,该流量传感器测量作为时间t的函数的源流量Q源(t)。该流量测量值被气源控制器和传感器/用户界面用来计算控制如方程3所示的Q源(t)的所需△X命令。
与传统呼吸机的情况一样,入口气体或气流可能需要加湿和/或加热。这是通过从控制器到加湿和加热模块72的命令来实现的,其与储存器46通信,储存器46通过水的加热和随后的蒸发、水的压电雾化或将水添加到气流中的其他常规方法来添加水蒸气,增加气流的湿度。当气体流过该模块时,该模块也可以对气体进行加热。
气体流过相对湿度传感器,该相对湿度传感器测量作为时间t的函数的气体相对湿度RH(t)。控制器利用该测量值生成作为时间的函数的期望RH命令,RH命令(t)。
温度和压力源模块测量气体温度T(t)。控制器和传感器/用户界面利用该温度测量值计算加湿和加热模块的加热命令T命令(t)以控制气体温度。
温度和压力源模块还可以测量气体出口压力P出口(t)。控制器和传感器/用户界面利用该压力计算控制如方程3所示的Q源(t)的所需△X命令。温度和压力模块的出口与气体供应管路对接,该气体供应管路终止于加压鼻呼吸机或其他患者呼吸装置,例如面罩、套管或插管。
气源控制器和传感器/用户接口包括用于控制气源流量Q源(t)、压力P出口(t)、温度T(t)和相对湿度RH(t)所需的传感器接口。它产生致动器命令△X(t)、温度命令T命令(t)和相对湿度命令RH命令(t)。它还与用户命令输入装置和状态监视器连接,接收用于气源流量Q源(t)、压力P出口(t)、T(t)和RH(t)的用户定义的命令集。气源控制器和传感器/用户接口还向用户命令输入装置和状态监视器提供传感器读数。
用户命令输入装置和状态监视器允许用户生成用于气源流量Q源(t)、压力P出口(t)、T(t)和RH(t)的命令。它还显示传感器读数。该装置可以是类似于I-Pad的界面,其以有线或无线方式与加压鼻呼吸机组件通信。
气体供应管路可以是标准O2管路。气体供应管路也可以隔热,以便在从气体源行进到加压鼻呼吸机组件时将气体热损失最小化。气体供应管路还可以包括电加热元件以保持气体温度,并且还可以包括电源和数据线组以向加压鼻呼吸机组件提供电力并从加压鼻呼吸机组件接收传感器数据。由于气体供应管路具有已知的流动阻力RGSL,因此已知Q源(t)、P出口(t)和RGSL,可以通过等式P源(t)=P出口(t)-Q源(t)RGSL来计算在进入加压鼻呼吸机气体端口的点处的压力P源(t)。
附加的传感器可以提供用于控制气体源组件的输入。这些包括但不限于从加压鼻呼吸机组件空气腔室和通过胸腔运动监测呼吸速率和潮气量的基于阻抗的装置(例如系统)采样的空气腔室压力P腔室(t)、空气腔室温度TAC、空气腔室相对湿度RHAC、ETCO2和/或O2测量值。
参考图8,总体操作如下:气体源14向换气控制器12供应加压气体,换气控制器12打开阀28,以所需的频率、流量和压力向患者22供应气体,以支持患者的呼吸。由于在结合到阀28中的空气或气体储存器46中存在加压气体或空气的供应,将加压空气或气体输送到患者22基本上是在阀打开的瞬间进行。在患者呼气的同时对空气或气体储存器48进行再充。
与传统的换气装置相比,本发明的所得到的呼吸机系统是一种低成本、相对简单的装置,其坚固、体积小而方便而且重量轻,并且在响应患者需求方面异常快速。
