PTW Systems
The purpose of safe work permit system is to ensure a
stringent control when all non-routine works are
carried out. It also allows works be performed safely
by reducing the chances of misunderstanding when
non-routine activities are to be carried out.

The work permit itself does not make the job safe but it
ensures proper consideration is given to the risks prior
to commencement of work.
Area Classification

Non-hazardous Area
An area in which a flammable atmosphere is not
expected to be present.

Hazardous Area
An area in which a flammable atmosphere is, or may
be expected to be present.
Special precautions for any work activity are
necessary.
Hazardous Area

Hazardous areas are classified into the following three zones:

• Zone 0
• Zone 1
• Zone 2

Zone 0 - Flammable gases and vapors are constantly present

Zone 1 - Flammable gases and vapors are intermittently present.

Zone 2 - Flammable gases and vapors are not expected to be


present unless conditions are abnormal

& Certificates

The safe work permit system uses four different work permits.
Each has a solid colored border.

The safe work permit system uses seven different certificates in


conjunction with work permits. Each certificate has a colored
border of diagonal stripes.
Types of Permits

Hot Work Permit (RED)

Hot work is any work activity which will or may produce a source of ignition.

Cold Work Permit (GREEN)

Cold work is any work outside of “Routine Work” activity, excluding any work

which is considered to be “Hot Work”

Electrical Work Permit (YELLOW)
The electrical work permit is used for ANY work involving electrical equipment
or system. The electrical permit is combined with the Isolation Certificate (i.e.) it
is the same document for either application

Radioactive Permit (BLACK)


Any work involving a radioactive source e.g. X-rays, some well logging, etc
requires a radioactive permit.
Although there is no fire hazard attributed to gamma ray radiography work (x-
ray), these activities are potentially hazardous work, requiring additional
planning and precautions that are inherent in hot work.
In the application process, the originator and radiographer in most situations will
most likely the same person
Types of Certificates
Isolation Certificates
Used when any equipment, electrical, mechanical or process must be isolated for
maintenance or repair or any other reason. These certificates facilitate safe
isolation, lock-out and tagging, as well as communication to people concern.

Mechanical / Process Isolation Certificate (GREEN/WHITE)

A mechanical/process certificate is required where there will be ANY isolation of

mechanical or process equipment. The certificate is valid for the period stated in

Section 5 of the certificate. The period could extend beyond the duration of the

accompany work permit, e.g. a machine undergoing maintenance awaiting spare

parts, and the Person In-charge will sign to authorize the extension.

Electrical Isolation Certificate (YELLOW)


The electrical isolation certificate is used for ANY work involving electrical
equipment including isolation for mechanical work, e.g. disconnecting an electric
motor for mechanic to repair the pump. In some instances, it may be necessary to
have both electrical and mechanical certificates in place in conjunction with the
particular work permit pertaining to the job.

Senior Authorized Electrical Person/Authorized Electrical Person is responsible for


applying for the certificate. A competent, qualified/trained person e.g. operator may carry
out the precautions required, such as closing main breaker on MCC panel and installing
lock (tag out procedure). Work associated with high voltage and extra high voltage, the
certificate will be ONLY issued by Senior Authorized Electrical Person.

Lock-out Devices and Procedures

An employee who has to repair, adjust or maintain machinery or other equipment

may run grave risks unless he is certain that the machine or equipment cannot be

started up, energized, or activated until his job is finished.

A lock-out device is a mechanism or arrangement, that allows the use of padlocks

and keys to hold and retain a switch/lever or valve handle in the REQUIRED

position. Effectively installing locks and tags at each isolation point. One lock and

tag WHERE POSSIBLE be installed by the operator and the second lock and tag

WHERE POSSIBLE be installed by the person who will be actually involved in the

work activity.

Ground Disturbance Certificate (BLUE/WHITE)

Ground disturbance certificate is needed whenever mechanical

excavation deeper than 0.5 meters is planned in areas where it is

reasonable to expect underground pipelines, utilities or other

structures.

The ground disturbance certificate is only valid for the duration of

the relevant accompanying work permit

Confined Space/Vessel Entry Certificate (BROWN/WHITE)

A confined space/vessel entry certificate is required to ensure safety

for personnel entering confined spaces or vessel, in order to carry

out inspections, maintenance etc..

Safety System By-pass/Override Certificate


For some work activities on process or mechanical equipment one
or more safety systems may need to be taken out of service. This
may be done through a by-pass, or an override of a safety system or
equipment, e.g. maintenance work on fire pumps,
testing of high level shut-downs, testing of emergency shut-down
alarms.
No by-pass/overrides are permitted without the appropriate safety
system bypass certificate is required
Types of Certificates

Scaffolding Certificate (ORANGE/WHITE)


Scaffolding certificate is needed for the purpose of safe
construction, alteration and dismantling of a scaffold.
This is to ensure that the scaffold can sustain safely all
imposed wad.
Note: Scaffolding certificate does not standalone; it
becomes invalid without an accompanying work permit
Types of Certificates

Lift Certificate (PURPLE/WHITE)


A lift certificate is required for all lifts not classed as
routine such as lifting over process equipment or
wellheads, lift with sling not made of wire rope, multiple
crane lift, lifts exceed 8 tons and lifts inside of confined
space.
