Metatarsalgia

Introduction

• Normally about one-third of the body weight falls at the


forefoot and that weight distributed between the pads of the
toe and MTP joints.
• The metatarsalgia comes from the names of the bone that
are in this part of the foot; metatarsals.
• Pressure on the ends of the metatarsals bones causes
symptoms in the ball of foot.
Defination:
 It is described as condition that cause plantar pain in the forefoot at the MTP
joints.
 It is best characterized by pain under the metatarsal head exacerbated by
weight bearing.
 There is marked pain and inflammation in the ball of foot.
 This ball of the foot pain is often located under the second, third and fourth
metatarsal head.
 And this metatarsalgia pain experience during activity that involve running
and jumping.
 Tenderness is felt over the plantar aspect of capsule of MTP joint.
Common factors:
 Pes plantaris
 Pes cavus
 Muscular weakness
 Wearing high heels
 Tight aponeurosis
 Hammer toe
 Intense training or activity
 Bunion
 Excessive weight
 Stress #

Contd:
 Muscular weakness- weakness in toe flexors, during inactivity or
immobilization can also rise to metatarsalgia on initiating sudden weight
bearing.
 Wearing heels- It also cause constant pain over the forefoot giving rise
to metatarsalgia.
 Tightness of plantar aponeurosis- due to the tight aponeurosis.
Aponeurosis get shortened pulling the metatarsal head downward causing
curling of the toes
 Bunion – swollen painful bump at the base of big toe which put extra
stress on the ball of the foot
 Stress #- small breaks in the metatarsals and toe bones can be painful and
injury may lead to metatarsalgia
Sign and Symptoms

 Sharp aching and burning pain in ball of foot or metatarsal head


 Pain around 2,3,4th toe
 Pain gets worse when stand, walk and run improves while in rest
 Numbness or tinging in toes
 Pain gets worsens when flex feet
Differential diagnosis

 Degenerative changes like arthritis


 Morton’s neuroma
 Loss of metatarsal fatpad
 Metatarsal stress #

Morton’s metatarsalgia

 In Morton’s metatarsalgia there is sudden onset of pain with burning


sensation over the outer border of the forefoot and between the third
and fourth toes while walking. Pain subsides after rest and massage

 The causative factor is thickened digital nerve (often a neuroma) between the
third and fourth metatarsal bones.

Conservative treatment consists of:


The alignment of the metatarsal heads by providing a small pad which elevates the
heads of metatarsals.
This prevents painful nipping of the digital nerve.
Test And Diagnosis

 X-ray to identify and rule out a stress fracture or other foot problems.

Complications
1. If Metatarsalgia left untreated, it may lead to:

2. Pain in other parts of the same or opposite foot.


3. Pain elsewhere in the body, such as the low back
or hip, due to limping (altered gait) from foot
pain.
Drugs:
 Ibuprofen (Advil, Motrin, Others)
 Naproxen (Aleve, Others)
 Aspirin to reduce pain
Physiotherapy Management:
1. Rest : protect the foot from further injury by not stressing it.
Low impact exercises such as swimming and cycling continues
with stretching.
2. ICE the affected area: ice the affected area when associated
with swelling with leg in elevation for 20 min ,

3. PAINFUL STAGE: Warm water, contrast bath, TENS,


Ultrasound
4. Ankle and toe movement to reduce edema and to improve
circulation.
5. Faradism under pressure with leg in elevation.
6. Faradic foot bath with voluntary intrinsic exercises.
SHOE MODIFICATIONS

 Wearing proper shoes with appropriate shoe wedge.


 Use metatarsal pads.
 Try shock absorbing insoles these are made of plastic ,
rubber or gel like substance ,fit inside the shoes and works
as a cushion.
 Consider arch support, it minimize stress on the
metatarsal head and improve foot function.
