Medical Management: Treatment Measures Include The Following
Main objectives are to improve circulation to the extremities, prevent the progression of the disease, and protect the extremities from trauma and infection. Treatment measures include the following: Completely stopping use of tobacco. Regional sympathetic block or ganglionectomy produces vasodilation and increases blood ow. Conservative debridement of necrotic tissue is used in treatment of ulceration and gangrene. If gangrene of a toe develops, usually a below-knee amputation, or occasionally an above-knee amputation, is necessary. Indications for amputation are worsening gangrene (especially if moist), severe rest pain, or severe sepsis. Vasodilators are rarely prescribed (cause dilation of healthy vessels only).

Nursing MGT:
Nursing intervention: Lower the extremities below the level of the heart Encourage moderate amount of walking or graded extremity exercises
Nursing Dx: Chronic Pain related to impaired ability of peripheral vessels to supply tissue with O2
Nursing intervention Promote increase circulation Administer analgesic agents as prescribed with appropriate nursing consideration
Nursing Dx: Risk for impaired skin integrity related to compromised circulation
Goal: Attainment/Maintenance of tissue integrity Nursing Intervention Instruct in ways to avoid trauma to extremeties. Encourage wearing protective shoes and padding for pressure areas; wear new shoes for short period of time and inspect feet for signs of injury. Encourage meticulous hygiene: bathing with neutral soaps, applying lotions and carefully trimming nails. Caution to avoid scratching or vigorous rubbing. Promote good nutrition; adequate intake of vitamins A and C, protein and zinc and control of obesity.
