    AFFIDAVIT OF CONSENT
    I, GIAN CARLO M DELA CRUZ, of legal age, Filipino, single and a
    resident of #005 Carreon St., Centro East, Santiago City, Philippines after
    having been duly sworn to in accordance with law, depose and state THAT:

    1. I am the registered owner of a parcel of land covered and embraced in Original


    Certificate of Title No. (OCT) CSC-7741, located at #27 Quirino Avenue, Victory
    Norte, Santiago City, containing an area of ONE HUNDRED THIRTY EIGHT (138)
    square meters, more or less;

    2. I hereby give my consent to my father, GREGORIO L DELA CRUZ, of legal


    age, Filipino citizen, married and resident of the same address the right to use
    the abovementioned parcel of land for the construction of one (1) storey
    residential building;

    3. I give unto him the right to act and assume all rights and responsibilities for the
    accomplishment of the foregoing task;

    4. I attest to the truth of the foregoing statements to the best of my own personal
    knowledge and based on authentic documents or records. I am executing this
    affidavit to support the application for building permit of my father, GREGORIO
    L DELA CRUZ, as well as for all other legal intents and purposes it may serve;
