You can file a RTI application to your Gram Panchayat Secy or directly to BDPO and ask them

information related to Gram Panchayat Fund.

However I suggest that you should ask the information only last few years. Asking information
for 15 days will become a huge information for which the PIO may make an excuse.

In Haryana RTI fee is Rs. 50/-. You can either enclose a Indian Postal Order of Rs 50/- in fovor
of Secretary Gram Panchayat and send it by registered post or pay the fee in cash.

Ask following questions

Please provide following information under Right to Information Act 2005.


1. How much amount has been received by Gram Panchayat from State and Central Govt. under
various schemes in years 2014 and 2016? Please provide details as per following format:
a. Name of Scheme/fund
b. Amount received
c. Date of receipt of amount
d. Amount spend so for
e. Bank account No.

2. How much amount has been received by Gram Panchayat from other resources in the years
2014,2015 and 2016? Please provide details as per following format:
a. Name of source of Income
b. Amount received in 2010
c. Amount received in 2011
d. Details of bank account in which the amount was deposited.

3. Please provide details of works carried out by Gram Panchayat in the year 2014,2015 and
2016 as per following format:
a. Name of work
b. Name of scheme
c. Total amount spent on work
d. Date of completion of work
e. Name of contractor

4. On which date the account of Gram Panchayat was last inspected?

5. Please provide me photocopies of Bank Statement of all the accounts maintained by Gram
Panchayat from 01 Jan 2015 to 31 Aug 2016. The photocopies should be signed and certified by
Gram Sarpanch.

6. Please provide Name, address and contact number of Junior Engineer, Assistant Engineer and
Executive Engineers who are responsible to ensure quality of works by Gram Panchayat.

7. Please provide name, designation, address and contact number of First Appellate Authority
who should be approached in case the reply received from you is incomplete or unsatisfactory.

For Budget grant

[1] certified copy of the Head-wise budget grant sanctioned for the year 2014-15 and 2015-16.

[2] certified copy of the statement of total tax, cess and/or Octroi or Local Body Tax (LBT)
collected under various heads during 2014-15 and 2015-16.

[3] Please supply me details of MP-LAD, MLA-LAD funds received by the Panchayat for
execution of works, during 2014-15 and 2015-16.

[4] Please supply me details of MGNREG funds received by the Panchayat during 2014-15 and
2015-16.

For Abstract Works and expenditure

[5] Head-wise expenses of the Gram Panchayat separately for the period 2014-15 and 2015-16.

[6] List of Plan-head works contracts and Maintenance works Contracts awarded and
expenditure incurred for such works, for the years 2014-15 and 2015-16 separately.

[7] List of works completed under MGNREG funds and the actual expenditure charged to
MGNRG Works during 2014-15 and 2015-16 separately.

[8] Certified list of works completed by Panchayat, out of funds released from MP-LAD funds
during 2014-15 and 2015-16.

[9] Certified list of works completed by Panchayat, out of funds released from MLA-LAD funds
during 2014-15 and 2015-16.

For Work Expenses

[1] certified copy of Letter of Acceptance of all works awarded from 1/4/2014 to 31/1/2016

(LoA shall contain exact description of work awarded on contract, name and address of
contractor to whom the work awarded, accepted contract value, date of commencement, target
date of completion.)
[2] certified copy of latest paid On-account bill (OR running bill) in respect of all on-going
works.

(On-Account Bill/Running Bill shall contain details payments made in the past)

[3] certified copy of Final Bills of all contracts completed from 1/4/2014 to 31/1/2016

(Final bill will show all details of amount paid, recoveries due and made, escalation in rates or
contract value allowed, final Completion Cost etc.)

(4) Details of Service Tax deducted form each of the contractor and remitted to Central Excise
and Customs Department during the period 1/4/2014 to 31/1/2016 (The amount of Service Tax
can be used to assess the correctness of payments released to the contractor )

If you still have doubt about the quantum of physical work executed vis-a-vis the quantum
of work awarded on contract, and payment released, you can seek:

[5] certified copy of the Schedule of Work in respect of Letter of acceptance No. ..
dated

(Schedule of work shall show quantity of each item included in a contract. With the schedule of
work in hand, you can cross-verify physical quantum and assess discrepancy if any.)
