EXEMPLAR 2:
CARDIFF METROPOLITAN UNIVERSITY APPLICATION FOR
ETHICS APPROVAL
When undertaking a research or enterprise project, Cardiff Met staff
and students are obliged to complete this form in order that the
ethics implications of that project may be considered.
If the project requires ethics approval from an external
agency such as the NHS or MoD, you will not need to seek
additional ethics approval from Cardiff Met. You should however
complete Part One of this form and attach a copy of your NHS
application in order that your School is aware of the project.
The document Guidelines for obtaining ethics approval will
help you complete this form. It is available from the Cardiff Met
website.
Once you have completed the form, sign the declaration and forward to
your School
Research Ethics Committee.
PLEASE NOTE:
Participant recruitment or data collection must not
commence until ethics approval has been obtained.

A3 Describe the research design to be used in your project
I have decided to take online surveys for my research project. Because my project
is mainly based on the music industry in Bangladesh, so its not possible to take
interviews of persons from related field.
I have used random sampling as everyone listen music and everyone has their
different choices. Sample size will be 50 people. I have chosen these participants
from my friends, colleagues and some known persons related to the music
industry in Bangladesh. It will take approximately 4 weeks to get all responses.
We will take careful steps to make sure that the data cannot be directly identified
from the questionnaire form; there is no information on these questionnaires that will
identify the participants. The personal details of the participants (e.g. signature on the
consent form) and the questionnaire will be kept in secure locations by the research
team. When we have finished the study and analyzed all the information, all the
documentation used to gather the data will be destroyed.- I have used Excel to
analyzed the data. Because it is easy to use and convenient
-------------------------------------------------------------------------------------------------------------------------------
If your data collection will involve the use of a questionnaire, you should provide a cop
of the questionnaire with your application. In cases where the questionnaire has not y
yet been finalised you should give examples of the kinds of questions which
participants will be answering. Similarly, if your data collection will involve the use of
interviews or focus groups, you should provide details of the topics which will be
discussed and a Participant information Sheet and Participant Consent Form.
If you intend to undertake the data collection within an organisation or at an event you
must also provide a (draft) letter to the organisation or event managers asking for
permission to do the research on their premises.
An Approved Protocol is one which has been approved by Cardiff Met to be used
under supervision of designated members of staf; a list of approved protocols
can be found on the Cardiff Met website here

13. Are there any other events that you would like to attend at xxx?
Dear Guest
I am very glad you are a customer of XXXXX, but I would like to find out more
about you, what you would like the hotel to provide and how well we are meeting
your expectations. I have asked a team of undergraduate students to undertake
research with you to try to find out if there is a gap between what you would like and
what we currently provide.
Participation is voluntary and anonymous and we will understand if you choose not to
take part. However the students and I would be very grateful if you could complete it
and return it to the collection box on the reception desk.
Thank you for your time,
XXXXX
