    CREDIT SALARY VIA GCASH/ PERSONAL BANK ACCOUNT

    Concentrix aims to timely and regularly release the salaries and allowance of all its employees. In this
    regard, we remind all employees of their responsibility to ensure the timely completion and activation of
    their payroll account opening and enrollment. Concentrix will not be responsible for any delay on the part
    of employees in completing this responsibility.

    Nevertheless, should there be some delay in the said creation and activation of your company payroll
    account, Concentrix will extend accommodation to you to ensure your receipt of your salaries and
    allowances by making the payments/salary credit to your elected Personal Account subject to limitations
    and terms indicated herein. While Concentrix will accommodate the processing of payments to your
    Personal Account, Concentrix will maintain this accommodation and cover Instapay fees only for the first
    two (2) payouts to your Personal Account. Again, it is your responsibility to ensure that you complete your
    company payroll enrollment and activation. Should you fail to complete your company payroll account
    enrollment and/or continue to be paid through your Personal Account after the said 2 payouts, you will
    have to cover for the applicable Instapay fees and any facilitation fees. These corresponding fees are due
    from you and you agree for Concentrix to deduct the same from your payroll to facilitate the payments to
    your Personal Account.

    To clarify further, Concentrix is not responsible for maintaining your Personal Account, and will not be
    involved in its management including any requirement for maintaining balance and charging of fees by
    your bank.

    Should you fail to provide the Company with a valid Personal Account to be credited with your pay, the
    Company will be constrained to hold on to any amount due you until you provide a Personal Account or
    completed your company payroll enrollment.

    In relation to the foregoing, the personal bank account or GCash (“Personal Account”) details are being
    collected below for the purposes of your identification, processing the payment of your salaries,
    allowances and benefits, and management of payments made to you (in case of delay in your payroll
    enrollment/activation). The herein collection, processing and storage of your personal data will be in
    accordance Concentrix Privacy Policy.

    You will continue to have the rights of a data subject under the law, including the right to be informed,
    object, withdraw consent, access, erasure, blocking, and file a complaint. Should you have any concerns
    or requests regarding privacy, you may email your concern to dataprivacy.ph@concentrix.com. Note
    however that your failure to provide your personal data, withdrawal of consent or exercise of your right
    to erasure and blocking may result to Concentrix being unable to process your payment to you.

    CONSENT AND AUTHORITY:

    I certify that the GCash or personal bank account (“Personal Account”) details provided below are true
    and correct. I authorize Concentrix to credit my salaries, allowances and payables to the Personal Account
    below. I agree to indemnify and hold Concentrix free and harmless from any damages or claims relating
    to my use of my Personal Account, maintenance charges and fees by my bank, including charges in case
    my Personal Account go below maintaining balance, and/or my non-receipt of any amounts credited by
    Concentrix to the said Personal Account. I also agree to cover any fees relating to the said crediting to my
    Personal Account after the first 2 payouts, and authorize Concentrix to deduct the same from my payroll
    or any amount that is payable to me. In case I am unable to provide a valid Personal Account where my
    pay can be successfully credited, I authorize Concentrix to hold on to any amounts due to me until I am
    able to provide a Personal Account or complete my company payroll enrollment.
