Remarks:
Client has experienced ongoing problems with substances since adolescence. He has recently spent five days
detoxing in Gorman House and indicated that he plans to go back in to Gorman House and from there go to a long-
term residential rehab.

Client reported that he has suffered from ongoing bouts of depression since his teens and has been treated with
various kinds of psychotropic medication and been case-managed by local mental health teams.

The findings and recommendations of this assessment are based on information reported by the client at interview,
observations made by the assessor during the interview, information contained within previous Job Capacity
Assessment report, prepared by Christine Sheey, Registered Nurse, HSA, dated 10/7/07 and a medical certificate,

prepared by client's treating doctor (medical certificate was unable t be located by assessor), dated 12/11/08 for
condition, namely drug dependence. It is noted that there have been a number of medical certificates submitted for
this condition and also for depression on the Medical screen of the client's Centrelink record.

For information related to the client's work capacity, please see relevant sections of the report.

The client presented on time and was cooperative and forthcoming with information. He appeared somewhat
agitated which he said was due to the fact he was withdrawing from substances. He indicated that his main goal was
to be rehabilitated off substances.

The client reported a number of personal factors, including family/relationship breakdown, lack of formal education,
accommodation issues and literacy/numeracy issues, all factors which are likely to have a significant impact on his
capacity to work, obtain work and look for work and which the client will disclose in the appropriate setting. In spite
of barriers, the client reported that he has completed a Certificate III in Aged Care.

Based on information from the above sources, the client would benefit from a referral to the Personal Support
Program, to address his multiple non-vocational barriers, with high and urgent need.
