PROCEDURES GUIDE COLLECTION, IDENTIFICATION, HANDLING, SAFE


TRANSPORTATION, PROCESSING & DISPOSAL OF SPECIMENS

1. Purpose
1.1.To channelize the sample handling system
1.2.To avoid any confusion in collection, identification, handling, transportation, processing and
disposal of specimen

2. Policy
Samples shall be collected, identified, handled, transported, processed and disposed as per the
standard procedures in line with the infection control measures.

3. Scope
Staff handling samples and specimens at OPD, IPD, OT, ICU, Sample collection area, Laboratory &
Housekeeping.

4. Responsibility
Lab In-Charge, Laboratory Technicians, Nurses, Doctors, House-Keeping personnel.

5. Process Details
5.1 Sample Collection

5.1.1 Sample collection facilities are provided in patient care areas like OPD, IPD .
5.1.2 Due consideration is given for space, seating and privacy.
5.1.3 Adequate toilet facilities are made available close to the sample collection area.

5.2 Sample Identification

5.2.1 Lab requisition slip carries sufficient information to identify the patient, doctor
requesting the test and pertinent clinical details. These are to be filled by the
doctor and signed by the staff nurse/ doctor. The Lab requisition slip shall
contain the following:
5.2.2 UHID Number (Unique Hospital Identification Number). Name of the patient
including age, gender
5.2.3 Name of the Ordering Physician / Referring Doctor
5.2.4 Destination for the report to be sent to.
5.2.5 Examinations requested
5.2.6 Clinical Diagnosis of the patient.

5.2.7 Date and time of the test requested


5.2.8 Whether the test is Urgent or Routine

6 Sample Handling

6.1 All samples will be handled as per the infection control guidelines
6.2 Universal precautions are to be observed while handling samples.

7 Safe Transportation of Samples

7.1 The laboratory monitors the transportation of samples from the point of collection to the
Laboratory, ensuring the time frame within which the sample has to be processed, the
temperature to be maintained, and care to be taken to prevent spillage or contamination of the
environment or people handling the specimens.
7.2 All samples collected in vacationers shall be transported safely in sample collection boxes
7.3 Necessary precautions are to be taken depending on prevailing environmental factors
7.4 There is a defined policy for handling, storing and transportation of samples and specimens to
outsourced laboratories

8 Processing of Samples

8.1 Processing of samples is to be carried out as per the requirement of individual tests.
8.2 The examination processes are to be accurate and reliable.
8.3 Test procedures are to be selected based on standard methods that have been published in text
books or journals and have been accepted in international/national/ regional guidelines.
8.4 The procedures are to be validated for use by running of controls and calibrators. All these
records are maintained in calibration file. The procedures for selection of methods are
reviewed once a year and the modifications/changes, if any, are to be recorded.
8.5 All procedures to perform the tests are to be documented and are available at the place of
work for ready reference.
8.6 Procedure for testing is to be standardized and necessary instructions issued to all concerned
personnel
8.7 Whenever a new kit from a different manufacturer is used, it is to be evaluated against the kit
in use and compatibility with the old kit (Lot verification), range, linearity, etc. established.
The new kit is to be introduced only after it meets these criteria. In case any changes are to be
made then the corresponding information are be modified accordingly
8.8 Samples will be processed without delay, and on priority for emergency cases.
8.9 . All results are to be reviewed by authorized technical staff in the Laboratory. The results are
to be checked, evaluated and verified. The verified data are then available for viewing and
printing in hospital .When a patient requests for a printed report it is to be made available.

9 Disposal of Specimens

9.1 Disposal is to be carried out in accordance with bio-medical waste handling rules. (2016)
9.2 Precautions in accordance with the hospital infection control manual are to be observed
