    AFFIDAVIT OF LOSS

    I, ALAIN JUDE R. VIDAL, of legal age, single and with residence and postal
    address at Barangay Looc, Sibulan, Negros Oriental, Philippines, after having been sworn to
    in accordance with law, do hereby depose and state:

    That I have been issued a Non-Professional Driver’s License by the Land


    Transportation Office, Dumaguete City and a G-Cash Account Card, ATM Card issued by
    Metrobank and a Company ID;

    That on September 27, 2020, I placed my wallet where my Non-Professional Driver’s


    License, my GCash Account Card bearing mobile number 09452514238, my Metrobank
    ATM Card and my company ID were inserted in the side pocket of my motorcycle. After I
    while I noticed that my wallet where I inserted my valuable cards cannot be found anymore.
    Perhaps, the said wallet where I inserted my cards was stolen in my motorcycle;

    That despite diligent effort to locate said wallet which contained my Non-Professional
    Driver’s License, GCash Account Card, ATM Card and Company ID, the same was beyond
    recovery.

    That I am executing this affidavit to attest to the veracity of the above facts and for
    the Land Transportation Office to issue me the duplicate copy of my Non-Professional
    Driver’s License, for Globe Telecom to issue a new GCash Account Card, and for Metrobank
    to issue a new ATM Card and for my employer to issue a new Company ID;

    IN WITNESS WHEREOF, I have hereunto affixed my signature this 29th day of


    September, 2020 at Sibulan, Negros Oriental, Philippines.
