[CAPSTONE] AN ANALYSIS OF GAMING

ACTIVITY, STATE OF HAPPINESS, AND


MENTAL WELL-BEING AMONG UPRHS SHS
STUDENTS
Greetings! We are Marc Quilloy, Vohn Redulla, and Isaiah Reyes. We are here to conduct a
survey titled: “AN ANALYSIS OF GAMING ACTIVITY, STATE OF HAPPINESS, AND MENTAL
WELL-BEING AMONG UPRHS SHS STUDENTS'' and based on its title, this study would be in
regard to the effects of gaming on your personal happiness and mental well-being. As a
capstone requirement, we would like to gather information on what is your mood when playing
games, how playing games affect your mood and well-being throughout the day, and your
overall engagement when playing.

As such, we are politely requesting for your participation in the survey as this would only take
around seven to ten minutes. Participation from this questionnaire is completely voluntary and
withdrawal can be taken at any time.

We are hoping that the responses you give us are true for the sake of our research project. Rest
assured that all the data we collected from this survey will be used for this study and
educational purposes only; and the data will be held with utmost value and confidentiality in
accordance with the "Data Privacy Act of 2012".

Happiness: According to Merriam-Webster, happiness is the state of well-being and


contentment. In the words of Kerns, "It is the experience of frequent, mildly pleasant emotions,
the relative absence of unpleasant feelings, and a general feeling of satisfaction with one’s life.”

This section of the survey was adapted from the Oxford's Happiness Questionnaire written by
Dr. Steve Wright.

On a scale of 1-6, answer the following questions with 1-Strongly Disagree, and 6-Strongly
Disagree.
