Lourdes Dela Cruz who was assigned to our school to secure safety and orderliness. Due to her
poor health she can no longer serve the duties assigned to her.

C. MAURICIO who was previously assigned in this school.
1. The school necessitates new OPOS Guard that would give maximum security and
guarantee that it is protected carefully with great concern,

I have the honor to inform your good office of the Electrical issues that the above-mentioned school is
presently experiencing. Due to insufficient power source supplied by the only Meter base that the school
has installed years ago, inevitable power shortage and power outages that sometimes cause fire on the
wire lines happen.
Hence, this office already submitted request for the inspection and seek advice from the DepEd
Engineers. And base on the findings of the inspection by Engr. Erwin D. Pumay and Engr. Darryl Oseña,
it needs to allocate an amount of four (4) million pesos to have the installation of the additional electrical
meters and wirings for the newly-constructed 2 units 4-storey 12 classroom building. They also advised
that the installation of the additional electric meters will be included in the 2022 National budget.
Thus, the forthcoming 2022 Elections demands number of classrooms to be utilized during the activity.
The school sought to have the success of the said activity by having a sufficient and stable power supply.
Therefore, we ask for your assistance for an immediate solution to this matter. To be one of the priority
projects for CY:2022. This will not only serve the school and its staff but the whole community during
the elections and other similar activities especially when face-to-face classes will be implemented back.

This is in reference to the letter received from Engr. RUEL V. UMALI, District Engineer of the
Department of Public Works and Highways District Engineering Office regarding their response to the
contractor’s unfinished obligations and rectification works needed for the newly-constructed school
building under BEFF 2018 Batch 3 project. As stated, the agency informed the concerned contractor and
advised that actions will be taken immediately to resolve the raised concerns.
Therefore, this office seeks the immediate action on the matters that intervene the proper usage of the
building and its classrooms. With the underlying condition of the building (water leakage from the
ceiling, diminishing flush doors and others), safety of the learners would be at risk if face-to-face classes
will be implemented not only because of the Pandemic but because of the impending danger.
May I also inform your good office that these school buildings will be used for the scheduled 2022
Elections which will be of great importance if these concerns be addressed as soon as possible.

budget as per recommendation by the DepEd Regional Engineers.
As inspected by Engr. Erwin D. Pumay and Engr. Darryl Oseña, it needs to allocate an amount

School, Tala, Caloocan City. They also advised that the installation of the additional electric

After series of inspection done by the DepEd Regional Engineers on the proposed
installation of additional electric meters and wirings, Engr. Erwin D. Pumay and Engr.
Darryl Oseña recommended that the needed budget for the said installation is Four
Million pesos.
Therefore, they suggested and promised to include this installation in the 2022 National
Budget since the school’s MOOE is lesser than the needed amount for the additional
installation of electric meter and wirings.
The finance officer, BAC members with the school principal, sought necessary
realignment of the budget for repairs and installation to the needed materials for the
SBM room installation, the Grade Six Graduation and Kinder Moving Up Expenses for
these are the timely needed project.
It is agreed by the BAC members that the allotted budget of Php 307, 704.83 for the
Installation of additional electric meters for June, 2021 will be used for the Installation
of SBM Room, Grade Six Graduation and Kinder Moving Up Expenses.
It is understood that only the allotted budget for additional electric meter installation
will be realigned and used.
Except as otherwise expressly stated herein all of the terms and conditions of this
amendment remain unchanged an in full force and effect.

be momentarily unavailable due to the sudden suspension of work in the Land Bank
Novaliches branch because of COVID-19 related issues.
If it were be asked from a different branch, it will be the cause of delays for the needed
liquidation report for the reason that the next branch which is the LBP PhilCOA is located far
from the requesting school.

I have the honor to explain that the compensation for the services rendered by
______________________ as ___________________ from Monday to _____________ as charged from
the Maintenance and Other Operating Expenses of the above-mentioned school was granted
earlier.
The school has decided to give their monthly salary from the MOOE to compensate their
services earlier as expected so that the report on liquidation will be given much focus and time
and be submitted before its deadline.
As per the signed contract, the services rendered by them is continuous and cannot be delayed.
Therefore, the school decided to give their April and May salary last May 17, 2021 to give way
for the liquidation process.
The school has seen nothing wrong with the early payment of wages to the workers as this
would encourage them more and would make the reporting of liquidation be easily done.

Madam:
I have the honor to explain that the compensation for the services rendered by Mrs. Joventina
F. Pacanut as school guard on duty at night from Monday to Sunday was charged from the
Maintenance and Other Operating Expenses of the above-mentioned school from March 1,
2021-June 30, 2021.
The school has decided to get portion of the MOOE to compensate the designated guard
because her agency has their on-going re-alignment and adjustment on the contract needed
not only to Mrs. Pacanut’s but for the rest of the agents connected to the agency. Because of
the Pandemic, the agency’s promise to give the intended compensation was delayed and
promised that on July 1, 2021 they will have their remuneration.
She serves continuously as assigned guard even without the assurance if she will be
compensated or not just to help implement the school’s trust and policies as Security Guard
due to unavailability of DPSTM, she commits to abide by office rules and regulations and other
special rules.
Being aware of Mrs. Pacanut’s situation the school provided her the Job Order Contract from
March 2021-June 2021 which clearly states her duties and responsibilities to be compensated
with a fixed monthly compensation of NINE THOUSAND PESOS (PHP 9,000.00), subject to
lawful deductions and usual budgeting, accounting and auditing rules.
Affected by the ongoing situation brought by the Pandemic, re-alignment and revisions of
PPMP, MCP, AIP, SIP, APP, SIP, WFP and Breakdown of expenses were made and submitted to
the Division Office dated March 22, 2021 which is prior to the information given that the guard
on duty, Mrs. Pacanut need to be compensated by the school that is why her monthly wage
was not included in the revised copy of the MCP, PPMP, APP, AIP, SIP, WFP.
As stated in the Division Memo dated February 24, 2021 with Special Order No:03-21-9213, re:
Mission Order with Permission and/or Authority from the DepEd Division of City Schools
Caloocan (SEF), that Mrs. Pacanut was designated to continue her service as Security Guard
under the SEF Budget as agreed in the meeting last February 23, 2021.
Hoping that the agency’s promise to Mrs. Pacanut and the rest of the security guards through
the help of your office will be granted and guaranteed to be compensated on July 1, 2021.

Electrical Plan of A. Mabini Elementary School from the licensed Electrical Engineer as per requirement in
the installation of additional Electrical base of the newly-constructed 4-storey, 24 classroom building.
The needed additional electrical base will hold the power source for the remaining buildings which will be of
used as the rooms in the said buildings are the venues of the scheduled School Learning Clinic of Kinder,
Grades 3, 4, 5 and 6 observing the same Safety Protocols and in preparation for the face-to-face classes.
It is requested that the new electricity meter connection be granted to avoid power outage which sometimes
caused series of damage to our electric transmission, short circuit and breaker operation failure which has
been experienced by the school since we only have one power transmission which can’t hold and supply 7
buildings due to its limited capacity of source and the material used can not hold huge amount of electric
power voltage.
