    WARRANT OF ARREST

    TO: ANY OFFICER OF THE LAW

    You are hereby commanded to arrest ____________________, who is said to be found at ___________________________
    or where ever he may be found in the Philippines who have been charged before me with the offense of _____________
    and bring him/her forthwith before me to be dealt with according to law.

    Furthermore, pursuant to A.M. No. 21-06-08-SC, you are required to use at least one (1) body-worn camera
    and one (1) alternative recording device or, if for justifiable reasons a body-camera is not available and upon
    motion, at least two (2) alternative recording devices, or such number as may be necessary to capture and record
    the execution of the warrant which devices shall be turned off only on circumstances allowed under Section 10, Rule
    4 of the same rule.

    The bond for the release of the accused in this case is set at ________ Pesos. The bail bond maybe furnished
    by the said accused either:

    1. By depositing the amount thereof with the nearest collector of internal revenue or Provincial, City or
    Municipal Treasurer, or clerk of court where the case is pending. Upon submission to a competent Court
    of the proper certificate of said deposit and a written undertaking for the compliance with the
    conditions of bail under Section 2, Rule 114 of the 1985 Rules on Criminal Procedure, the accused shall
    be discharged from custody;

    2. By furnishing a property bail bond undertaken and subscribed by one or more solvent sureties who are
    resident owners of real estate, whose solvency are shown by their land tax receipts and certificate of
    title, free from any encumbrances, justified and approved by the judge; or

    3. By a bond posted by a duly licensed and currently authorized Surety Corporation.

    The bond maybe approved by any Judge of Regional Trial Court/Municipal Trial Court sitting in any
    province or city where the accused is being held or by a Metropolitan or Municipal Trial Court Judge, who shall
    transmit the same with the release papers to this Court.

    The peace officer/s making this arrest is/are hereby authorized to discharge the accused from his/her/their
    custody after he/she/they have furnished the bail duly approved and ordered released by the proper Court.

    The Head Office to whom this arrest has been delivered for execution shall cause the warrant to be executed
    within ten (10) days from receipt hereof. Within ten (10) days after the expiration of such period, the officer
    assigned to execute this Warrant of Arrest shall make a return thereof to this Court accompanied by affidavits of the
    officers whose body-worn cameras or alternative recording devices were used to capture the execution of the
    warrant and simultaneously deposit said all recordings stored in an external media storage. In case of his/her/their
    failure to execute the warrant, he/she/they shall state the reasons thereof. (Sec. 4, Rule 113, Rules on Criminal
    Procedure and Sec. 4, Rule 1, A.M. No. 21-06-08-SC).
