UNDERTAKING

I, ____________________________________________________, of legal age, Filipino


Citizen, married and presently residing at
___________________________________________________________________________
after having been sworn in accordance with law, hereby depose and say that:

1. Being the ________________________________, (please provide your


designation) of establishment under the declared business name of
____________________________________________________________________________
located at ____________________________________________________________________
(the „‟Business‟‟), with the authority to execute this Undertaking and be held liable for the same.

2. The Business is duly owned by ______________________________________, a


single proprietorship/partnership/corporation (pls. underline appropriate type of entity), duly
registered or organize under the laws of the Philippines with principal address at
_____________________________________________________________________________.
The Business is duly registered and licensed to do business by the City of Las Piñas, having
obtained a Business License & Mayor‟s Permit dated _______________ under Account No.
_________________________ (hereinafter referred to the as „‟Permit‟‟).

3. In behalf of the entity I represent, I hereby acknowledge that the issuance of a


Permit by the City of Las Piñas to the Business is a privilege subject to the faithful and full
compliance of the terms and conditions as enumerated hereunder, to wit:

- The Permit shall be posted conspicuously at the place where the Business is being
conducted and valid only at the place indicated therein. It shall be presented to competent
authorities upon demand.

- The Permit is revocable and may be cancelled or voided should the taxpayer, owner or
holder, or any of its officers, operators and/or employees, violate any applicable laws,
ordinances, rules and / or regulations, including any of the terms and conditions of this
Undertaking, or should the taxpayer, owner or holder fails to pay any tax or charges as
they become due.

- The Permit must be surrendered to the Office of the City Treasurer upon closure or
retirement of the Business.

- Fire Safety Inspection Certificate (FSIC) for the Business should be valid and/or updated
for duration of the validity period of the Permit (por the “Permit Period”). Should the
validity of the FSIC expire within the Permit Period, the taxpayer, owner or holder of the
Permit should immediately secure an updated or renewed FSIC and submit a copy thereof
the Business Permits & Licensing Office (BPLO). A temporary FSIC that is initially
granted should be made permanent within the Permit Period and duly submitted to the
BPLO.

- Sanitation Permit for the Business should be valid and/or updated within the Permit
Period. A temporary Sanitary Permit that is initially granted should be made permanent
within the Permit Period and duly submitted to the BPLO.
- Should the Business require permit/s from any other government office or agency, such
as the Food and Drug Administration, Department of Health, Department of Education,
Culture and Sports, etc., for the operation of the Business, such permit shall be presented
to the BPLO. Revocation or cancellation of such permits shall cause the revocation or
cancellation of the Permit.
- Waste Discharge Permit for discharge, handling, maintenance and/or treatment of liquid
and solid waste, including septic wastage and maintenance for the Business should be
obtained from the Department of Environment and Natural Resources (DENR), in
accordance with existing laws, rules and regulations, and to be submitted to the BPLO on
or before June 30, 2021.

- Notwithstanding the payment for Barangay Clearance, the Permit may be revoked or
cancelled upon withdrawal of such clearance by the appropriate Barangay where the
Business is located. Giving notice to the BPLO by the Barangay of complaints pertaining
to the operation of the Business, non-compliance or violation of any existing laws and
ordinances, including Barangay Ordinance, and/or request for appropriate action of
complaints pertaining the Business, are deemed as withdrawal of the Barangay Clearance
by the appropriate Barangay.

4. Should the Business require compliance with additional conditions as imposed by


the BPLO which are appropriate to the nature of the Business (such as junk shops, spas,
vulcanizing shops, etc.), I shall take due notice thereof and undertake to duly and faithfully
comply with the same.

5. I hereby expressly acknowledge that any violation or non-compliance of the


conditions of this Undertaking is a valid cause for the revocation and/or cancellation of the
Permit, and the closure of the Business in accordance with the existing ordinances of the Las
Piñas City, without prejudice to the filing of appropriate cases, whether criminal, civil and/or
administrative, against the taxpayer, owner or holder of the Permit.

6. I am executing this affidavit to attest to the foregoing in connection with my


application for the issuance of Business License and Mayor‟s Permit for the current year.
