    AFFIDAVIT OF LOSS

    I, MYLENE DUMAS POSTRERO, Filipino, of legal age and a resident of Brgy. 100, San
    Roque, Tacloban City, Philippines after having been duly sworn to in accordance with law do hereby
    depose and say:

    1. That my grandfather, INOCENTES CABAOBAO HOMERES is a bonafide and active


    member of the Senior Citizen Association (OSCA) of Tacloban City with ID No. 3602336;

    2. That as such he was issued a Senior Citizen ID by the LGU of Tacloban City;

    3. That unfortunately said ID was lost without his knowledge and believed to have
    been misplaced;

    4. That for all intents and legal purposes, it is considered it lost and beyond recovery;

    5. That I am executing this affidavit to attest the truth and veracity of all the foregoing
    statements.

    IN WITNESS WHEREOF, I have hereunto affixed my signature this 6th day of January
    2022 at Tacloban City, Philippines.
