CASE SCENARIO:
Prematurity
A newborn baby boy was delivered via NSVD to a 40 years old G1P1 mother, 30 weeks AOG, weight 1500 grams.
mother has a previous history of admission due to urinary tract infection.
As patient arrives in the NICU, patient was placed under radiant warmer, attached to monitor and vital signs were taken:
Temperature : 35.5 degree Celsius
Pulse rate : 156 beats per minute
Respiratory rate : 72 breathes per minute
Oxygen saturation : 89-90% in room air

The Patient was limp, pale and mild cyanosed and in distress with labored breathing. Upon auscultation, lungs were with
moderate subcostal, intercostal, and supraclavicular retractions, with decreased air entry on both lungs.

Laboratory results:

Complete Blood Count (CBC)

Hemoglobin 12.4 g/dL
Hematocrit 40.5 percent
White Blood Cell Count 20,000 cells/mcL
Platelet Count 280,000/mcL

Chest x-ray result:

● Po2 70
● Pco2 80
● Hco4 18
● Base Excess -2

Because of the hypoxia, he is given the following interventions:


1. Attached to Mechanical Ventilator with Settings: Fio2=100%, VR=60pm, PC above Peep 22, Peep 5, IT=0.5s
2. A 500 cc D10 water infusion at 5ml/hr (80ml/kg/day)
3. A blood culture is obtained.
4. Was ordered to start an Umbilical Vein Catheterization.
5. Arterial Blood Gas was requested.
6. He was started on Intravenous ampicillin 150mg q12 and amikacin 22.5mg od
7. To administer Survanta 6ml (4ml/kg)
