treatment programs where I am accountable data collation,entry,validation,program monitoring and
reporting in both DATIM and 3PM online sites as well as implementing and strengthening monitoring and

against targets as well as providing feedback to the team on identified data quality gaps and poor

Duties and Responsibility
 Spearhead Continuous Quality Improvement activities within the project to keep track of stocks.
 Manage facility databases by ensuring quality entry of patient line data of pediatrics, adolescents
and adults.
 Ensure data integrity is upheld by doing regular data cleaning and validation.

 Preparation of timely and accurate routine, monthly, quarterly and annual facility reports and
submission of all relevant stakeholders.
 Organizing and facilitating quarterly, semiannual and annual project reviews meetings with facility staff.
 Developing summary report and power-point presentations on key issues emerging from projects
monthly reports, evaluation reports, and feedback reports.
 Identification, documentation, and dissemination of project experiences, lessons learned,
and success stories.
 Design data quality assurance program and lead data quality assessments for high quality reporting.
 Ensure necessary Ministry Of Health, customized donor registers and summary tools are in
place and in use at service delivery points.
