    I, GILBERT HAMOR, of legal age, Filipino, with residence and postal


    address at Block 1, Lot 42, Southville 1, Brgy. Marinig City of Cabuyao, Laguna,
    after being duly sworn to in accordance with law, hereby depose and state:

    That I am unemployed and have no fix income;

    That I have not gainfully engaged in any profession, business or


    transaction to earn money;

    That I have not filed any Income Tax Return (ITR) with the Bureau of
    internal Revenue (BIR) for the reason stated above;

    That I have executed this affidavit to apply for legal service with the Public
    Attorney’s Office (PAO), to affirm the truthfulness of the foregoing and for all legal
    intents and purposes that may legally serve.
