    AFFIDAVIT OF LOSS

    I, KENNY JUN SERVAN, Filipino citizen, single and a resident of Poblacion


    Norte, Mayantoc, Tarlac, after having been duly sworn to in accordance with law, do
    hereby depose and state:

    1. That I am a holder of my Professional Regulatory Commission


    Identification Card with Registration No. 0866483;

    2. That the said ID was accidentally misplaced by me on September 26, 2018


    and despite my diligent efforts, exerted on my part to locate the same
    proved futile and in vain;

    3. That the probability of recovering the same is practically nil and I am now
    declaring its loss;

    4. That I am executing this Affidavit of Loss to attest to the correctness and


    truthfulness of the foregoing statements and for the application and
    issuance of new GSIS ID.

    IN WITNESS WHEREOF, I have hereunto affixed my signature below this
