DEN Clinical Competency Prompts

These are some examples to help prompt you to think of how your practice
links to each competency (taken from the NZNC competency tool).

Please also refer to the Nursing Council of New Zealand Competencies for
Enrolled Nurses:

Competency 1.1
Accepts responsibility for ensuring that his/her nursing practice and conduct meet the
standards of the professional, ethical and relevant legislated requirements.

 Demonstrates knowledge of the code of conduct and HDC act within practice
 Upholds patient privacy
 Ensures practices within scope of practice

Competency 1.2
Demonstrates the ability to apply the principles of the Treaty of Waitangi/Te Tiriti o
Waitangi to nursing practice. (This competency relates only to your experiences with
Māori patients)

 Applies the principles of the Treaty of Waitangi to nursing practice


 Ensuring equity
 Being aware of disparities in health and linking this to clinical practice 

Competency 1.3
Demonstrates understanding of the enrolled nurse scope of practice and the registered
nurse
Responsibility and accountability for direction and delegation of nursing care.

 Practices within the scope of practice for a student enrolled nurse


 Demonstrates understanding of the registered nurses role to direct, delegate,
monitor and evaluate nursing care
 Is accountable for following direction from Registered Nurse
 Seek guidance from a registered nurse when encounters situations beyond own
knowledge, competence or scope of practice

Competency 1.4
Promotes an environment that enables health consumer safety, independence, quality
of life, and health.
 Reads infection control policies and procedures
 Upholds infection control policies and procedures
 Is aware of processes requiring an incident form to be completed
 Participates in the checking of emergency equipment
 Supports the health consumers right to maintain independent lifestyles with
dignity in their own environment

Competency 1.5
Participates in ongoing professional and educational development.

 Researches best practice guidelines for health education resource


 Reads local policies and best practice guidelines prior to carrying out nursing
activities
 Being proactive in learning opportunities
 Some type of formal education - e.g. a module which has been recommended by
the clinical placement and is relevant to the area 

Competency 1.6
Practices nursing in a manner that the health consumer determines as being culturally
safe.

 Respectful when entering people’s home environments


 Asking permission before carrying out any nursing assessments
 Using appropriate language with all clients
 Demonstrates the ability to provide culturally safe care to meet health
consumers’ needs beliefs and values  

Competency 2.1
Provides planned nursing care to achieve identified outcomes.

 Administers interventions, treatments and medication under RN supervision


 How you involve your client in their care using the nursing process. (Develop the
care plan in partnership with client/family, ensure client/family understand all
interventions or diagnostic tests required to give patient reviews on expected
progression).
 The organization policy/ process for informed consent for your client.

Competency 2.2
Contributes to nursing assessments by collecting and reporting information to the
registered nurse.
 Completes assessments as delegated by the registered nurse
 Assists with routine examinations and routine diagnostic investigations

Competency 2.3
Recognizes and reports changes in health and functional status to the registered nurse
or directing health professional.

 Observes for changes in the health consumers health and reports this to the
registered nurse or delegated supervisor
 Communicates observations to the registered nurse and appropriate members of
the healthcare team
 Reports any concerning changes or observations outside of expected ‘norms’ in a
timely manner to the registered nurse or delegated supervisor

Competency 2.4
Contributes to the evaluation of health consumer care.

 Monitors and documents progress towards expected outcomes – is aware of


normal parameters for observations being taken
 Contributes to the nursing care plan in collaboration with the registered nurse

Competency 2.5
Ensures documentation is accurate and maintains confidentiality of information.

 Appropriate documentation in clinical notes and handover to other team


members
 Utilizes documentation tools such as SOAP or SBAR
 Completes documentation using MedTech or documentation tool being used by
placement

Competency 2.6
Contributes to the health education of health consumers to maintain and promote
health.

 Measures health literacy of clients to ensure appropriate health education is


provided 
 How you assess your client’s health literacy.
 How you assess the need for specific health education.
 What health education you provide to clients. (Understanding their health needs,
diagnostic tests, treatment plans, effective and safe management of medication,
keeping well, action plans and other).

Competency 3.1
Establishes, maintains and concludes therapeutic interpersonal relationships.

 Identifies the limitations of confidentiality when dealing with Youth


 How you establish a relationship with clients (introduce self at beginning of shift,
ask consent to be involved in care, share plan of care for day with client, ask
about any immediate concerns).
 Your use of AI2DET or other communication tools in your practice.
 What communication techniques you use in your practice (effective interviewing
skills, counselling skills, therapeutic communication skills) to demonstrate
respect, empathy and interest in the health consumer.

Competency 3.2
Communicates effectively as part of the health care team.

 Communicates verbally and in writing appropriately and effectively


 Demonstrates understanding of the need for different communication styles and
approaches to different situations
 Contributes to a positive working environment
 Demonstrates the ability to take on constructive feedback in a positive manner

Competency 3.3
Uses a partnership approach to enhance health outcomes for health consumers.

 Practices within the Te Whare Tapu Wha framework which incorporates family
centered care
 How you ensure you share information appropriately with clients.
 How you promote informed choice.
 How you include family’ perspectives in your care.

Competency 4.1
Collaborates and participates with colleagues and members of the health care team to
deliver care.

 Able to work collaboratively with other health professionals such as medical staff,
SLT;s, physios etc
 Participants in Case management meetings (or similar).
 How you develop a plan of care in collaboration with other health professionals.
 How you ensure continuity of care, how you effectively escalate clinical concerns
to others

Competency 4.2
Recognizes the differences in accountability and responsibility of registered nurses,
enrolled nurses and healthcare assistants.

 Clarifies enrolled nurse role and responsibilities in the contact of health care
settings
 Prioritizes the delivery of nursing care to health consumers as guided by the
registered nurse

Competency 4.3
Demonstrates accountability and responsibility within the health care team when
assisting or
working under the direction of a registered health practitioner who is not a nurse.

 Works under the direction of an identified health practitioner and reposts


observations, changes in health status and escalates concerns to that health
practitioner
