    GREETINGS:

    Under and by virtue of the authority vested in me by law, you are hereby directed to submit your counter-
    affidavit in seven (7) legible copies and other supporting documents or affidavits of your witnesses, if any, within
    ten (10) days from receipt hereof. Your pictures taken in four (4) manners: front-view half body, left-side view
    half body, right-side view half body and front-view whole body, in four (4) copies each and printed in 4R size
    shall be attached to your counter-affidavits.

    Furthermore, you are directed to furnish a copy of your counter-affidavit and controverting evidence to
    the complainant, with proof of service thereof to be submitted to this office.

    Attached is a copy of the complaint and other evidence submitted by the complainant.

    You are hereby WARNED that failure on your part to comply with the subpoena shall be considered as a
    waiver of your right to present your defense and the case shall be considered submitted for resolution based on the
    evidence on record.
