    We Spouses, IRENE OBMINA VENTANILLA and ADRIAN LITHEL TUBOSA


    VENTANILLA, both of legal ages, Filipinos, with residence and postal address at 152 Quezon St.,
    Brgy. Poblacion, Muntinlupa City, after being duly sworn to in accordance with law, depose and say:

    1. That we are the guardian of a minor named, KHALIL ANDREI OBMINA GAGAN, who
    was born on March 17, 2007 in MAUBAN, QUEZON;

    2. That the parents of the above-named minor are spouses, NAOMI BACALLA OBMINA, who
    is currently working in Saudi Arabia and FRANCISCO ENCALLADO GAGAN who is in
    Sampaloc, Quezon Province;

    3. That by reason of that, we are the acting parent or guardian of the above-named minor, and his
    care and custody was under ours;

    4. That we not an imbecile, insane, a vagrant or a vicious person or a habitual drunkard or a


    habitual criminal, and have not abandoned, neglected or refused to support said minor or
    caused his to commit offense against the law;

    5. That I execute this affidavit in order to attest to the truth of the above facts for all legal intent
    and purposes it may serve.
