    Dear Dr. Oca,


    I’m writing on behalf our client name of seafarer. Complainant was
    employed by the respondent company as position of seafarer and was deployed on
    date of deployment. He was repatriated on date of repatriation. He was diagnosed
    of illness/injury of seafarer. He was given a disability grading of put disability
    grade but the second opinion of his independent physician stated that he is
    permanently unfit to resume seafaring duties.
    Mr. name of complainant, is now claiming total and permanent disability
    based on his CBA. May I respectfully request for the assistance of AMOSUP so
    that a Grievance Proceeding may be set for the case of name of client, against
    name of company, The SPA executed by Mr. name of seafarer in favor of our Firm
    is hereto attached and marked as ANNEX A.
