Abstract

The study aims to know the positive and negative impact of integrated ethics program at
rural areas in terms of nurse’s knowledge and performance. Integrated ethics is defined as ethical
concerns in medicinal services that are arranged into significant domains in which these domains
are the rehabilitation hospitals, nursing homes and healthcare systems that were intended to cover
the wide scope of ethics issues faced by all types of healthcare organizations. However, there are
instances where barriers or problems arise so that poor and effective decision-making methods are
not performed to meet all patient needs especially the needs of the pregnant mothers and even
newborn babies also the other children. Rural community are the areas where there is simple living
such as having clean and fresh air and beautiful scenery but not all good features the rural area has
because of the big problem of the rural community is about health in which does not meet all needs
of people in a community when it comes to promoting health and addressing people who are sick.
That is why having an ethics is very important in the field of medical profession because it guides
every nurse working in a different work conditions especially in rural areas where most of in the
rural community hospitals have a difficult situation where they experience a shortage of medical
equipment.

Introduction

In today’s generation the word ethics plays a major role in our society in which an ethics
is a moral principles wherein it becomes rules in everything such as at school in which how the
teachers, students and other members of the school follow it and also at work wherein having the
policy that needs to be followed can really a big helps just to have a good and positive feedback
for everyone. However, morality refers to the standards of right and wrong that an individual first
learns in a community there is also some individual define morality as choosing the right belief or
behavior. These can be bases what a person should do as obligation, right, and value especially to
every medical practitioner like nurses need a critical analysis to the complex situation. Ethics also
focuses on the principles of ethical behavior especially in the modern society at the level of the
individual, society, and interaction with the environment. There are some basic expectations of
any society or nation; these are the humanity, honesty, and trust. To achieve the goals of the health
care system, every nursing manager are required to adhere to ethical principles in decision making
most especially in complex and challenging health care settings. Additionally, ethics can be
guiding every nurse in day by day practice and settings to the hospitals and it helps to the person
who wants to be medical practitioners in the future by setting essential objects and goals. Its
capacity is to give also a brief statement of the moral commitments and duties of each person who
enters nursing profession. Also, there are many hindrances that the nurse’s experiences in time of
their duty in a rural area. One of this is the rural nurses in which every nurse who are working in
rural areas experiencing relatively difficult provision of patient needs or difficulty addressing their
needs.
Nevertheless, the feedback that some study has is divided into two in terms on what is the
impact of integrated ethics program at rural areas in terms of nurse’s knowledge and performance.
The first one is the positive impact to it wherein it helps to promote the performance and
knowledge of nurses in terms in decision-making in which it is important element in nursing
profession. While on the other hand, negative feedback there are some barriers in implementing
the integrated ethics especially in terms of ethical leadership.

Body

Integrated ethics are much important for any healthcare provider such as nurses and
doctors. According to Haddad L.M. et. Al, 2020, the word ethics is derived from the Greek word
"ethos," which means character. Additionally, it is a good rule for every professional not just a
professional but also to others because it administers how an individual act in which it focuses to
the action of a person it’s either good or bad. Also having ethical standards that provide a practical
basis that leads and give a reasonable premise to distinguishing what sorts of goals, actions and
intentions are esteemed. On the other hand, Maternal and Child Health (MCH) is a health service
that helps the mothers who are in the stage of child bearing and the children. It also provides
protection and promotes the health of a mother and child. Even though the impact of MCH is high,
there is a time that some pregnant mother or children are not provide or met by appropriate and
proper needs.

One of the reasons is that in some rural areas has a difficult lifestyle where they cannot
meet all the individuals’ needs especially when it comes to medical matters. According to Sheldon
Weisgrau (2016), rural communities are experiencing shortages when it comes to medical
equipment where it is difficult to maintain in rural or meet the appropriate needs of the people.
Also, rural residents regularly encounter or experiences different problems or barriers to human
services that limit the capacity to acquire the care needed. All together for the rural residents for
them to have an adequate access, vital and suitable social insurance health care services such as
nurses must be accessible and possible in an ideal way. Even in any event or situation especially
if the situation that are facing is quite difficult. The other barriers that a rural community has are
the workface shortages and health insurance status transportation, health literacy and stigma
associated with conditions just like having a mental health. In terms of living of those in rural areas
there are reasons for having a problem sometimes but, living in this kind of community is not
always a bad idea for everyone because a rural area or community has more clean and fresh air to
breathe and have a comfortable living. However, there are instances that some of a person who has
the disease that are difficult or not easy to address in the hospital facilities of a rural community.
That is why rural nurses should be better prepared and aware to sometimes difficult situations that
can be encounter. And because of that there is integrated ethics that helps the rural nurses to
immediately find a proper and effective solution with the help of it by having positive ideas for all
the nurses.

Additionally, having ethics in all fields of works especially medical professionals here will
know that a nurse is able to meet all the needs of a patient especially in areas that lack in medical
equipment such as in the hospitals of rural areas. According to Luke Kikukawa (2017), rural
healthcare can be limited and is often fragmented. Many rural areas lack health care providers as
well as the resources to meet the needs of the populations in the community and some countries
lack even the most basic health resources such as clean water, doctors, antiseptic techniques, or
immunizations. Most especially to the mother who is bearing the child, or the pregnant mother and
the children are the most people who are in needed in terms of medical care. According to
Shahriari, Mohsen et al. (2016), there are 10 nursing ethical values included here is the human
relationship, sympathy, honesty, precision and accuracy in caring, commitment, human dignity,
privacy, justice, autonomy in decision making and individual and professional competency. And
based on the study it can help to improve the nursing practice in terms of immediately responding
to patient needs and effective methods to help a patient.

Conclusion

In general in conclusion it can be said that having an ethical standard in a field of work
has a great contribution to society because with its help even though there is a time of lacks of
medical needs it is met promptly because everyone or each of nurses thinks the decision wisely to
where it is right or wrong, is it bad or good for the patient and is it effective or ineffective. With
this it is shown that every nurse should have awareness in terms of sensitive making ethical
decisions. In addition, based on the research done it is concluded that ethics has an impact on a
nurse’s knowledge and performance because with this it helps to determine that a nurse is very
effective in the field of medicine.
