    AFFIDAVIT OF LOSS
    I, RECHELLE M. NEGRITE, Filipino, of legal age, single and a resident of Sili,Naguilian, La Union,
    Philippines, after being duly sworn to in accordance with law, hereby depose and state:

    1. That I graduated from Naguilian National High School in Naguilian, La Union on the year 2008;

    2. That I am currently processing my papers for enrollment at TESDA;

    3. That in view of said application, I am required to submit my High School Diploma;

    4. That however, my High School Diploma which was yet to be issued to me by the school got lost
    sometime on 2009 when Typhoon Pepeng struck the town of Naguilian, La Union and a great flood
    swept across ourschool premises;

    5. That because of such typhoon, some of the school’s records and pertinent documents including my
    High School Diploma were hit and washed out;

    6. That I diligently searched for my High School Diploma but as all efforts to find the same proved
    unrewarding, it is now safe to conclude that recovery of the same is an impossibility;

    7. As such, I am executing this Affidavit to attest to the truth of the foregoing facts and to support my
    request for the issuance of my High School Diploma from the Naguilian National High School and
    for any other legal purpose that this Affidavit may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 7 th day of February 2020 at Naguilian, La
    Union, Philippines.
