BREAST ABSCESS

DEFINITION

A Breast abscess is a painful collection of pus that forms in the breast.

A swollen lump develops under the skin, and is primarily caused by a bacterial
infection.

INCIDENCE

One in ten women (in the age group of 18 - 50 years) is affected by breast abscess.

CAUSES

 Mastitis

 Blocked milk duct

RISK FACTORS

 Inadequately treated mastitis

 Abrupt weaning during an episode of acute mastitis.

CLINICAL MANIFESTATIONS

 Flushed breast ( warmth and redness of breast tissue)

 Brawny edema of the overlying skin

 Marked tenderness with fluctuation

 Swinging temperature

 Breast pain

 Nipple tenderness
 Discharge from nipple

 Tender or enlarged lymph nodes in the armpit [affected side]

DIAGNOSTIC EVALUATION

 History collection

 Physical examination

 Chest x-ray

 Breast biopsy [fine needle aspiration biopsy]

 Mammogram

MANAGEMENT

Medical management

 Analgesics for pain

 Antibiotics – cloxacillin 500mg Q6hrs for 7-10 days

Erythromycin

Penicillin

 Adequate breast support

Surgical management

Abscess drainage

 Incision and drainage

 Needle aspiration with concurrent antibiotic therapy

 Ultrasound guided vacuum assisted biopsy
