Daily Diary 1

This semester we were able to experience how to be in actual work setting as

student nurses on duty at the Quezon memorial Center. Our first day at the area

was very exciting and at the same time nervous as first timers. We were first

introduced to the staffs that we are very pleased to get to know with and after that

we’ve been oriented on how the place works and were able to familiarize the area.

Our clinical Instructor thought us how to take a TPR and also how we will

communicate well with the patients. At first it is hard because of some questions

we did not want to ask but we have to, our Clinical instructor thought us that we

have to dig deeper to our patients even though it might offend them, because it is

also for them.

Our second day on OB ward, each one of us was assigned to different patients it is

not a first time for me to handle a patient though I already know how take the vital

signs and what is the things I should know when I interviewed my patient. My

patient are very friendly and accommodating she is very talkative so my

nervousness fades away easily. I also performed the BUBBLESHE, First is I did

the health teachings for the breast and advised the patient to increased fluid intake

and greenly vegetables such as malunggay and kangkong, second is Ive palpate the

patients uterus to see if it is soft or firm and contracted, the urine and stool, the

colour of her lochia or if there are any signs of bleeding, if she has any rashes or if

there are any pain she felt, I also did the homan’s sign and also if she is responsive

or not.

On our third day on OB ward we were assigned two patients each one of us it is

exciting because we were going to handle two different cases. We took the vital

signs, interviewed he patient and we did the health teachings and the bubbleshe

then we did the charting and also the NPI. Our Clinical Instructor taught us how to

use a medication card and how to make a medication card properly and I enjoyed

doing such things because it is a new knowledge for me. I am very pleased to meet

my patients because they are very talkative and very accommodating that’s why

touching others peoples life and being able to cater to their needs is an

achievement that no other thing can replace. The feeling of fulfilment I have me

makes me believe that as long as I lose what I’m doing there can be a thousand

people or even hundreds of thousand people in this world that I can offer my

undying service.

On our fourth day of our duty I was already confident on the things I did because

of the same routine I did on my patients. We took the vital signs, interviewed the

patients and did the health teachings. We also did our TPR sheet and the sample

charting also the treatment record it is very important for to make these things so in

the future we won’t forget any of it. I have improved my skills in rendering good

nursing care such as morning care, vital signs taking, providing interventions that

are appropriate to my client, giving medication and improved my charting. I was

already confident on anything but also very careful because I know I can also do

something’s that will made me a mistake, that’s why in every actions I take I

always think hard if I am doing it right.

On our fifth day of our duty on Quezon Memorial Center, it is our last day of our

duty so I did my best and assure that my patients will go home with a big smile. On

this day I remembered what we were like when we first duty on here like the

mistakes we did but learned from it and the smiles of our patients with us it is a

very nice experience we had on OB ward. As for giving and prescribing drug and

even importance of different drug we administer and treatment we did on our

patients, I learned that before going on duty we are always ready on the things we

will encounter and always ready for the emergencies we will face on the QMC.

For this experience I can say that this was the time wherein I was able to interact

more with the clients and with the family. I was really happy because 3ust being

there# caring for my clients and witnessing their progress I know somehow I

contributed to their development. All I can say with my duty is Im happy with the

outcome and I really enjoyed it.

Ako nga po pala si Wella S. Virinia, student nurse Oo, itong kamay ko medyo masakit padin gawa
po ako sa Enverga, ako po ang magiging nurse niyo nung turok don sa suero. Namamaga pa nga nung
ngayong hanggang sa madischarge po kayo. nakaraan. Pero itong ouson ko hindi naman na
Kamusta naman po kayo ma’am? May masakit masyado effective naman kasi yung gamut na
parin po ba? binigay.

Wala naman pong malakas na pagdurogo? Wala naman.

Kamusta naman po ang pagpapasuso niyo?


Okay din naman, lakas na nga sumuso ni baby e.
Mabuti naman po. Ma’am para pol along dumami
gatas niyo ay kain po kayo ng mga gulay na Opo. Yun nga din ang sabi sakin ng mga dating
maberde lalo nap o ang malunggay. Tsaka po studyanteng nagduty ditto.
Ma’am pag po lilinisin niyo po ang suso niyo, gamit
po kayo ng maliit na towel at yun lang po dapat ang
paggagamitan niyo po, lagi niyo nalang pong
lalabhan yung tuwelya. Hindi nap o bulak ang
ginagamit ma’am ha, baka po kasi may maiwan na
hibla ng bulak e makain po ni baby.

Ma’am wag din po kayo maghuhugas ng mainit na


tubig pati alcohol baka po kasi mamaga at
matanggal yung tahi. May Philhealth po ba kayo
ma’am? Meron kaso zero balance e.
